PAST/PRESENT PERFORMANCE QUESTIONAIRE COVER LETTER
Corporate Letterhead
Date
The 38th Contracting Squadron of Air Force Space Command (AFSPC) is in the process of selecting a contractor for the Defensive Cyber Realization, Integration & Operational Support (DCRIOS) Services contract.
One of the considerations in the evaluation process is the verification of past performance on contracts that are relevant to the proposed effort. The 38th Contracting Squadron and Cyberspace Services Flight are depending on information received from your agency, which has had first-hand experience with our efforts.
The areas of interest are summarized in the enclosed questionnaire. The acquisition schedule is extremely tight and the 38th Contracting Squadron appreciates your written response no later than proposal due date.
The questionnaire should be completed electronically and be returned via e-mail to Anthony Owens (anthony.owens.7@us.af.mil) and Donna Williams (donna.williams.6@us.af.mil). You may contact Mr. Owens at 405-734-9637 if you have any questions about this request. Your completed questionnaire will become part of the official Source Selection records.
Your help is greatly appreciated and your prompt response will be one of the keys to successful and timely completion of this Source Selection.
