1. DESCRIPTION OF SERVICES
1.1 Introduction
The objective of this contract is to provide communications cable installation support services to defense activities at all areas of Wright-Patterson Air Force Base (WPAFB), Ohio. The 88th Communications Squadron (CS) supports WPAFB Local Area Networks (LAN), Metropolitan Area Networks (MAN), Wide Area Networks (WAN) and Department of Defense (DoD) unclassified and classified networks.
1.2 Scope of Work
This contract will provide for sound management, installing, upgrading, re-routing, terminating, testing and removal of communications cable infrastructure at WPAFB as identified in the individual Task Order (TO).
1.2.1 The Contractor will provide inter-building cable plant work to include installing, upgrading, re-routing, terminating, splicing, testing, removal, cable acceptance, inspection, and labeling documentation of copper and/or fiber optic cabling running through the manhole/hand-hole, duct systems and/or directly buried of copper and fiber optic cabling.
1.2.2 The Contractor will provide intra-building cable plant work to include installing, upgrading, re-routing, removal, termination, inspection, testing, cable acceptance, inspection, labeling, documentation and installation/removal of copper and fiber optic cabling. This may also require the installation and grounding of equipment racks/cabinets and cable tray/ducts.
1.2.3 The Contractor will provide communications infrastructure to include installing, upgrading, re-routing, removal, modification, and repair of manholes/hand-holes, and communications duct systems. This may also include open trench, road crossings and boring of duct system, and placement of pre-made and cast-in place manholes/hand-holes.
1.2.4 The Contractor will plan and manage continuity of efforts between TOs. The Contractor will be capable of managing simultaneous TOs.
1.2.5 The Contractor will install government provided Wireless Access Points (APs) according to drawings provided. APs will be mounted with suspended ceiling mounts or wall mounts below the ceiling as identified by drawings. Short jumper cables (normally white) will be installed to connect the AP to the network drop.
1.3 Task Order Requirements
1.3.1 Regulations and Guidance. The following standards will be used as applicable in the performance of this contract. It is the Contractor’s responsibility to obtain and adhere to all industry standards and best commercial practice documentation.
1.3.1.1 Base Facility Standard (BFS) or Installation Facility Standard (IFS), current version, Wright-Patterson Air Force Base, Ohio OPR: 88 ABW/CE and 88 CS/SCX.

1. Telecommunications Industry Association (TIA)/Electronic Industries Association (EIA) Standards (See Appendix B).
1.3.2 Technical Interchange Meeting (TIM). A Technical Interchange Meeting (TIM) between the Contractor and the Government prior to each TO proposal submission will be conducted to enable the Contractor to verify the Government design, the physical parameters (e.g., quantities, cable lengths, manhole/duct availability) and conduit serviceability (e.g. test mandrel, conduit runs via manhole to manhole). The TIM will be scheduled within 7 business days of the RFP being released. If the contractor or the government needs to cancel or reschedule a TIM it must be done no later than 4 hours before the start of the TIM.
1.3.2.1 If during the TIM it is determined that there is a scope change or changes/modifications are required to the proposed design, the Contractor will request that the documentation be updated to reflect the change within.
1.3.3 Proposal. The Contractor will utilize the Government package and information obtained from the TIM to develop a complete technical and cost proposal for implementation and integration. The Contractor will, as a part of the TO proposal, prepare a schedule showing the order in which the Contractor proposes to perform the work and the dates on which the Contractor contemplates starting and completing the salient features of the work.
1.3.3.1 Contractor will provide proposals according to the schedule specified in Appendix D.
1.3.3.2 A schedule must be prepared as part of the proposal. The schedule will be in the form of a progress chart of suitable scale to indicate appropriately the percentage of work scheduled for completion by any given date during the period of performance.
Any change to the schedule will be incorporated into a revised schedule and submitted to the COR and PM in writing not less than 2 business days prior to the change.
1.3.4 Drawings. The Government PM will provide the Contractor with 2 copies C sized drawings to include but not limited to communications room, rack diagrams, cable diagrams, manhole or hand-hole and infrastructure work area drawings as required for each TO. The drawings provided are For Official Use Only and the contractor will not dispose or destroy them in any manner. These drawing will be listed in APPENDIX E.
1.3.4.1 The Contractor will inspect, inventory, and sign a hand receipt (AF Form 1297) for all drawings provided by the Government PM.
1.3.4.2 The Contractor will be responsible for safe-keeping of drawings and return all 2 copies to the COR 2 business days prior to the final walk-through for review. 1 of the 2 copies returned should be red lined as built and the final copy should be marked as a working copy to the Government COR.
1.3.4.3 Whenever the drawings conflict with the BFS or IFS, the drawings will take precedence.

1.3.4.4 The Contractor will follow all requirements provided on the drawings.
1.3.4.5 All drawings will be verified by a contractor and Government representative prior to the Final Inspection and turned over to the COR at the completion of the final walk through. Contractor will have no more than two (2) errors in accuracy per deliverable. (CDRL A001)
1.3.4.6 If the Government finds any discrepancies with the redlined drawings the Contractor will be notified and have 2 business days to make the appropriate correction and return them to the Government COR.
1.3.4.7 The Contractor will have access to all available Base Civil Engineering (BCE) drawings, upon request, and base communications drawings to identify existing utilities and future utilities and/or construction, which impact the project. Base communications drawings will be supplied, when required. The drawings provided are For Official Use Only and the contractor will not dispose of or destroy them in any manner. The Contractor will verify the accuracy as applicable to the requirement and notify the Government PM of inaccuracies or discrepancies.
1.3.5 Labeling. The Contractor will refer to the BFS or IFS for labeling requirements unless the drawings specify that the labeling will deviate from the BFS or IFS. The Government will provide detailed instructions on the drawings of the label deviation.
1.3.6 Pre-testing. The Contractor will notify the COR at least three (3) business days prior to any pre-testing and will make available to the COR all testing standards/criteria used to accomplish testing, and testing results prior to government final inspection. The Contractor will perform a pre-installation quality inspection of Fiber Optic (FO) and copper cable on reels and already installed government cables prior to using. Any problems or failed tests will be reported to the COR before proceeding. (CDRL A003)
1.3.6.1 The Contractor will perform end-to-end testing on all installed communications cabling. Installed copper cabling will be continuity tested for shorts, grounds, splice errors (i.e., reverses, opens, transpositions and split pairs), and loop resistance.
Installed fiber will be tested to ensure the light loss will not be greater than manufacturer's specifications for cable and properly connected connectors. (CDRL A003)
1.3.7 Post-testing. The Contractor will notify the COR at least three (3) business days prior to final post-testing and will perform end-to-end testing on all installed communications cabling with government representative present. Installed copper cabling will be continuity tested for shorts, grounds, splice errors (i.e., reverses, opens, transpositions and split pairs), and loop resistance. All deficiencies will be corrected prior to government acceptance. All horizontal cabling will be end-to-end tested after all terminations have been accomplished to ensure they pass the applicable category specifications. Fiber optic cabling will have both Optical Time Domain Reflectometry (OTDR) and attenuation testing. Installed fiber will be tested to ensure the light loss will not be greater than manufacturer's specifications for cable and properly connected

The Contractor will test all fiber optic cables and make sure that any fiber optic cabling that does not meet the fiber optic standard will be checked, troubleshot, fixed, and re-tested; any fiber optic cabling that can’t be fixed after troubleshooting will be re-installed and tested to meet fiber optic standards.
1.3.8 Test Results. The Contractor will ensure the cable ID on the test results reflect the run locations as identified on the cable labels.
1.3.8.1 The Contractor will provide 2 copies of the post-testing results via electronic media (CD OR DVD) for each TO performed 2 business days prior to the final walk-through.

1.3.8.2 All test results will be verified by a Government representative prior to the Final Inspection and turned over to the COR.
1.3.8.3 If the Government finds any discrepancies with the test results the Contractor will be notified and have 2 business days to make the appropriate correction and return them to the Government COR. Contractor will have no more than two (2) failed fiber or copper connections at time of final testing per TO.
1.3.9 Final Inspection. The COR, QAE, engineer and Government PM will perform a final inspection with the Contractor Site Lead, confirming all TO requirements were met.
Exceptions will be documented, fixed, and re-inspected for acceptance. The QAE will make the final decision on whether the TO requirements have been met and can be accepted. The Contractor will have no more than one (1) TO fail to pass final inspection on first attempt, each quarter.
1.3.10 Communications Equipment Room (CER) Preparation. The Contractor will be responsible for assembling and disassembling racks for computer/network equipment, hanging and painting fire retardant plywood, installing cable trays, installing all cabling, relocate specified existing cables, and other CER preparations, as identified in each TO.
1.3.11 Service Outages. The Contractor will be responsible for preventing any unscheduled interruptions of service. If the implementation necessitates disruption of service (e.g., communications, electrical, or other utilities) the Contractor will notify the Government PM at least 12-working days in advance of planned outages. The Contractor will provide the Government PM with all the information needed to schedule the outage.
1.3.12 Differing Site Conditions. The Contractor will immediately notify the COR in writing in the event of subsurface or latent physical conditions at the site which differ materially from those indicated in the order, or unknown physical conditions at the site which differ materially from those ordinarily encountered and generally recognized as being inherent in work of the character provided for in the order.
1.3.12.1 Any discrepancies between the Performance Work Statement (PWS), the provided drawings and the actual site conditions will be reported to the Contracting Officer Representative (COR) within one business day. If a modification is needed to the drawings, the COR will contact the PM and Engineer to create updated
7 | P a g e drawings. The updated drawings will be sent to PZ via the COR and then sent onto the contractor via PZ.
1.3.13 Secured Work Areas. In secured work areas requiring special access, the Contractor will provide the PM with all information requested and/or needed to get proper access.
1.3.14 Base Civil Engineering Work Clearance. The Government PM will provide the Contractor with a signed Base Civil Engineering Work Clearance Request (AF Form 103), per Air Force Instructions (AFI) 33-104, Base-Level Planning and Implementation. The Contractor will ensure they have a signed/approved, AF Form 103, prior to any digging, trenching, room modifications, or electrical power modifications are started. The Contractor will have the approved AF Form 103 on-site at all times.
1.3.15 Environmental and Hazardous Material. All environmental work will be conducted in accordance with WPAFB specification section 01 02 20. (CDRL A004).
1.3.15.1 The Government PM will provide the COR and the Contractor with an Ohio Environmental Protection Agency Project Approval for any outside plant project.
1.3.15.2 The Government PM will provide the COR and the Contractor an approved Miami Conservancy District Storage Compensation Agreement if work is completed within the 100 Year Flood Zone/Retarding Basin for any outside plant project.
1.3.15.3 The Government PM will provide the COR and the Contractor approval from the State Historic Preservation Office (SHPO); if work is to be performed within or near any known historical/archaeological site; if work is to be done within an eligible historic district; or if work will be done in an area that is not known to be previously disturbed for any outside plant project.
1.3.15.4 The Contractor will confirm that no excavation/soil disturbance will be done to any creeks/waterways for any outside plant project.
1.3.15.5 The Contractor will calculate the amount of land disturbance to include, but not limited to, the following: clearing, grading, and excavation activities. If the project requires less than one acre of soil disturbance (including, but not limited to, clearing, grading, and excavation activities), the Contractor will submit an Erosion Control Plan in accordance with Part 3.4, paragraph B of WPAFB Specification 01 02
20. Any construction activity that will disturb one acre or more land requires the contractor to prepare and submit a Notice of Intent and Storm Water Pollution Prevention Plan (SWPPP) in accordance with Part 3.4, paragraph C of WPAFB Specification 01 02 20. When a sanitary line is modified or a new one installed, the contractor will submit a Permit to Install (PTI) in accordance with Part 3.4, paragraph D of WPAFB Specification 01 02 20. For any construction activity that requires making modifications to drinking water systems including tapping into or rerouting an existing water main, the contractor will submit a Plans Approval package in accordance with Part 3.4, paragraph F of WPAFB Specification 01 02 20 for any outside plant project.

1.3.15.6 The Contractor will monitor for methane gas during ground disturbance activities within landfills and within 100 feet of landfill boundaries. The Contractor will use properly calibrated methane gas meters and will provide the Government a monitoring plan for approval. The plan will include but not be limited to: calculated action levels, procedures to measure/record background readings, define monitoring frequency and location of areas monitored (i.e. in the open trench, at ground level above the trench, etc.), and describe a contingency in the event the action level is reached or exceeded for any outside plant project.
1.3.16 Hazardous Waste or Debris. Trenching through Earthfill Disposal Zone 1 (EFDZ1) may encounter construction debris.
1.3.16.1 Any boring, trenching or potholing may encounter buried hazardous waste or debris.
1.3.16.2 The Government is responsible for properly handling the hazardous waste, as well as characterizing and disposing of any hazardous waste excavated.
1.3.16.3 The Government will determine if the debris (construction material or trash) will be reburied or disposed of in a dumpster.
1.3.16.4 All disturbances to the soil covers must be repaired to at least the same protective standard of the landfill contents as before the disturbance.
1.4 Government Program Management
The Government Program Manager (GPM) and COR will serve as the primary liaisons between the 88 CS and the Contractor and between the Contractor and serviced organizations.
1.4.1 The GPM and the COR will review customer feedback received, validate any alleged complaints, and inform the Contractor of valid customer complaints. The feedback and complaints will be reviewed to assess the contractors’ performance and to take necessary action to improve processes.
1.5 Contractor Personnel Management
The Contractor will manage the delivery of services in a manner to balance workload, meet performance objectives, and improve customer service in a continuous process improvement approach.
1.5.1 The Contractor will ensure all contractor personnel maintain current security clearances, qualifications, and certifications.
1.5.2 The Contractor will provide qualified personnel who meet the required skill sets to successfully fulfill each TO.
1.5.2.1 The Contractor will designate the Contractor’s on-site team leader as the site point of contact (POC) in writing. The Contractor site POC will be responsible for the performance and quality of work by all participants and implementation of the Contractor’s quality control and safety programs through all phases of the project at

The Contractor will submit a Quality Control Plan and the Contractor POC will ensure services are performed in accordance with accepted commercial practices and the current WPAFB Installation Facility Standard. It is the Contractor’s responsibility to implement procedures to identify, prevent and ensure non-recurrence of defective services.
1.5.2.2 The contractor will have one employee physically located on Wright Patterson AFB with a Building Industry Consulting Service International (BICSI) technician level certification.
1.6 Deliverables
The Contractor will prepare and deliver Contract Data Requirements Lists (CDRLs) as listed in Appendix A. The Contractor will propose format(s) of deliverables unless otherwise stated in the CDRL. Delivery is to be electronic in a format compatible with Microsoft Office.
1.6.1 The Contractor will provide 2 drawings that the government has provided per each TO 2 business days prior to the final walk through as stated in CDRL A001.
1.6.2 The Contractor will provide a Status Report every Monday as stated in CDRL A002.
1.6.3 The Contractor will provide test results to the Government per each TO 2 business days prior to the final walk through as stated in CDRL A003.
1.6.4 The Contractor will not include proprietary markings on any of the deliverables (except for the financial deliverable, if applicable) that would preclude unlimited use and/or distribution by the Government. Contract deliverables will not be separately priced.
1.6.5 Enterprise Contractor Manpower Reporting Application (ECMRA). The Contractor will report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Client Technician Support via the ECMRA secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data will be reported no later than 31 October of each calendar year.

3. GOVERNMENT PROPERTY (GP) AND SERVICES
3.1. The Government may provide property, equipment, supplies, information, and services to accomplish the tasking. The Government’s intent and listing of such items will be specified in APPENDIX F of the TO if applicable.
3.2. The Government reserves the right to supply any property identified by the Contractor as necessary for accomplishment of the TO. Upon arriving on site for implementation, the Contractor and in coordination with the PM, will jointly inventory all GFP and sign an AF Form 1297, Temporary Issue Receipt. The Contractor will report to the COR and PM any GFP found damaged, malfunctioning, or unusable after the initial inventory/inspection as soon as possible, but no later than five (5) working hours after discovering the discrepancy.
3.3. The Contractor will protect GFP from lose, damage, improper use or disposition.

3.4. The Contractor will determine and report to the COR the extent, probable cause and usability of GFP that is damaged during or after installation.
3.5. The Contractor will be responsible for storage and protection of GFP upon receipt. Unless otherwise specified, the Contractor will be required to provide transportation of GFP. The Contractor will take precautions to assure adequate storage conditions to guard against damage from handling, deterioration, and theft.
3.6. The Contractor will be responsible for the storage and protection of all Contractor/ Government supplies, materials and equipment during the Period of Performance (PoP) of the
TO.
4. CONTRACTOR FURNISHED PROPERTY/MATERIAL
4.1. Contractor Furnished Property (CFP). The Contractor will provide all personnel, tools, transportation, supervision, and other items and services necessary to accomplish the requirements of the contract and each TO. The Contractor will clearly mark all CFP with the Contractor’s name.
4.2. Contractor Furnished Material. Unless otherwise specified, the Contractor will provide and transport all supplies and materials required to support implementation (e.g., cables, terminals, hardware, etc.).
5. GENERAL INFORMATION
5.1. Normal Hours of Operation. All contractor personnel will perform the services required under this contract during normal duty hours of operations of the site where work is being performed. All work will be performed Monday through Friday, 0730 to 1700, unless otherwise specified in the TO.
5.1.1. OBSERVANCE OF NATIONAL HOLIDAYS

Additionally, the Government will observe any other day designated by Federal statute, Executive Order, or Presidential proclamation.
5.2. Commercial Vehicle Entry. As a standard, all vehicles 15-pax van and larger [i.e., United Parcel Service (UPS), Federal Express (FEDEX)], step van/panel truck to include but not limited to tractor/trailer, box and flatbed containing cargo, gas/liquid/solid tanker trucks, box trucks, tour buses, garbage/recycled waste trucks, mixing trucks, dump trucks, cranes, recreational vehicles, and postal/mail trucks) will enter WPAFB via CVDG, Gate 16A. All others will be searched and processed at the gate where the driver is attempting to enter WPAFB.
5.2.1. The WPAFB CVDG is located on Communications Boulevard. Commercial vehicles will use Gate 16A off of State Route 444 to access Communications Boulevard. No other route of travel is authorized for arriving at the above location. Duty hours for Gate 16A are:
Monday through Friday from 0600-1800, and Saturday from 0600-1400.
5.2.2. All irregular deliveries will be sent to the CVDG for inspection. All visitors, which include contractors and commercial vendors without proper background checks and contractor passes, will not be allowed on WPAFB unless escorted by the receiving agency. This will take place after the individual(s) are issued a pre-clearance form at the CVDG. After successful completion of the vehicle inspection, the receiving agency will escort the individual to and from their destination on WPAFB. An inspector at the CVDG will contact the receiving agency, informing them to send an escort to the CVDG. Escorts will remain with the vehicle at all times while it is on WPAFB.
5.2.3. The COR must be provided a list that includes: delivery driver names, driver’s license numbers, and state in which their driver’s licenses were issued, and which gate they will use for delivery. If entry is requested on a day following a weekend or government holiday, the list must be submitted by 1300 on the last working day prior to the day of delivery. If the driver’s name is not on the list, they will not be permitted access.
5.3. Security and Privacy
5.3.1. The majority of this contract is UNCLASSIFIED but SENSITIVE; however, there may be times when work will be performed in a classified area/environment up to and including SECRET and/or within controlled areas. All individuals will be required to have a SECRET clearance prior to working on a task in a classified area.
5.3.2. A facility clearance at the SECRET level is required.
5.3.3. The Contractor will be responsible for compliance with all physical security, computer security, operational security, and communications security requirements. This includes security awareness training AFMAN 16-1405, Air Force Personnel Security Program; AFI 17- 100, Air Force Information Technology (IT) Service Management; and AFI 17-130, Cybersecurity Program Management, for access to government Automated Information Systems (AIS) required to perform the contract.

5.3.4. Contractors must also meet the security clearance requirements of DOD Instruction 8500.01, Cybersecurity, http://www.esd.whs.mil/Portals/54/Documents/DD/issuances/dodi/850001_2014.pd f
5.3.5. DD Form 254. The Contractor will comply with DD Form 254, DoD Contract Security Classification Specification, attached to this contract. DoD military, civilian, consultants and contractor personnel using classified and unclassified automated information systems must have a SECRET security clearance as a minimum, in accordance with DoD Manual 5200.02, Procedures for the DoD Personnel Security Program (PSP).
5.3.5.1. Contractor personnel will hold the designated clearance level required for a position at the time of appointment/award as indicated by the Government.
5.3.6. Operations Security (OPSEC). OPSEC requirements are required in an effort to reduce program vulnerability from successful adversary collection and exploitation of critical information. The contractor will apply OPSEC in accordance with AFI 10-701, Operations Security, and the 88 CG OPSEC Plan.
5.3.7. Government Common Access Card (CAC)/Identification Cards (ID). The Contractor will ensure their employees and their subcontractors have the proper credentials. Common Access Cards (CAC) will be issued and returned in accordance with AFFARS Subpart 5352.242-9001, DoDI 5200.46, DoD Investigative and Adjudicative Guidance for Issuing the Common Access Card (CAC), and AFI 36-3026_IP V-1, Identification Cards for Members of the Uniformed Services, Their Eligible Family Members, and Other Eligible Personnel.
5.3.7.1. The Contractor will complete a DD Form 2842, DoD Public Key Infrastructure Acceptance and Acknowledgement of Responsibilities. Contractor employees will have two (2) photo identification (ID) cards, a completed DD Form 1172-2, Application for Identification (ID) Card/DEERS Enrollment, signed by the Unit Security Manager. Prior to the issuance of a Common Access Common (CAC)/ID card, a favorably adjudicated National Agency Check Inquiries (NACI) or equivalent in accordance with revised Federal Investigative Standards is the minimum investigation required for a final credentialing determination for a CAC.
5.3.7.2. If a contractor is removed from the task, the Contractor will immediately identify that employee’s departure through the out-processing procedures to ensure return of CAC and removal from information systems access. The Contractor will report personnel who no longer perform contract services and therefore no longer require Government AIS access no later than 24 hours after termination.
5.4. Non-Disclosures. Contractor personnel will sign Nondisclosure Agreements for Contractor data, Privacy Act/Personally Identifiable information (PII), For Official Use Only (FOUO), and Sensitive but Unclassified (SBU) government data.
5.4.1. Completion of a nondisclosure statement for each individual is required. The Contractor will enter into a nondisclosure agreement with other Government contractors as requested by the Government. 14 | P a g e contract services will have restricted distribution within the selected Contractor organization. The Contractor will release no data or information related to this PWS without the prior authorization of the Government.
5.4.2. Work on this project may require personnel to have access to Privacy Information.
Personnel will adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.
5.5. Safety Requirements
5.5.1. The Contractor will provide and maintain work environments and procedures which will safeguard the public and government personnel, property, materials, supplies and equipment exposed to contractor operations and activities, avoid interruptions of government operations and delays in project completion dates, and control costs in the performance of this contract.
5.5.2. The Contractor will immediately correct safety violations of contractor and subcontractor personnel and notify the COR when a potentially hazardous or unsafe conditions exists. The Contractor will immediately notify the COR of all mishaps resulting in personnel injury, government property damage, utility damage, or financial loss to the Government. The Contractor will secure the mishap scene and immediately impound all wreckage, related items, and documentation. The Contractor will fully support the Government in any mishap or hazard investigation.
5.5.3. Confined Space Program. The contractor will provide a Confined Space Program for entry into non-permit required manholes, handholes, and cable vaults. Program should outline entry procedures, type of monitoring device used to detect hazards along with current calibration procedures.
5.5.3.1. Upon entering a confined space, the contractor will use barriers, i.e., manhole guards, traffic cones and warning signs to prevent unauthorized access into the designated work area. The contractor will check the confined space for any visible hazards and remove water from the space and begin atmospheric testing prior to entry.
5.5.3.2. The contractor will check their required safety and personal protective equipment to ensure functionality and serviceability. (gloves, hard hat, steel toe boots, eye protection…etc.)
5.5.3.3. The contractor will atmospherically test all confined spaces with an Altair 5x Gas Detector or equivalent.
5.5.3.4. The contractor will continuously monitor confined spaces while employees are in the space, readings will be documented on Government provided AF Form 1024 (or equivalent) every 2hrs from the time the space is opened till the time it is closed. The contractor will retain AF Form 1024’s for 1 year and provide a copy to the COR upon request.

5.5.3.5. The contractor will exit the space if a hazardous alarm is activated during work activity, and the space will be forced ventilated for a minimum of 15 minutes then re-tested, if necessary, the confined space will be continuously forced air ventilated until all work has been completed and all employees have left.
5.5.3.6. The contractor will conduct and document Confined Space refresher training on an annual basis with all employees.
5.6. Voluntary Protection Program.
The Contractor will participate in the Voluntary Protection Program (VPP). (CDRL A005)
5.6.1. Definitions
5.6.1.1. Applicable Contractor. A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant (installation).
5.6.1.2. Days Away, Restricted, and or Transfer Case Incident Rate (DART). The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.
5.6.1.3. Total Case Incidence Rate (TCIR). Total number of recordable injuries and illness cases per 100 full-time employees that a site has experienced in a given time frame.
5.6.2. Requirements
5.6.2.1. Wright-Patterson AFB is in the process of pursuing Voluntary Protection Program (VPP) recognition or has already been recognized under the OSHA VPP. VPP impacts all “applicable contractors” operating on Air Force Installations. The contractor will be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The Contractor will follow the safety and health rules of the installation or VPP site. Detailed information on VPP is available on the OSHA website at http://www.osha.gov/dcsp/vpp/index.html
5.6.2.2. The Contractor will submit a Safety and Health Plan (CDRL A005) and corresponding site safety checklist to the CO not later than (10) calendar days after contract award. The Contractor’s plan will include appropriate measures to ensure the Contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan will:
5.6.2.2.1. Identify the roles and responsibilities of the following individuals with respect to safety and VPP: Management, Supervisors, Employees, and Safety Coordinator;
5.6.2.2.2. Identify applicable safety rules and regulations;

5.6.2.2.3. Include a worksite hazard analysis to include base-line hazard identification and required control measures;
5.6.2.2.4. Include a job site analysis to include hazards of tasks required to control measures;
5.6.2.2.5. Identify employee safety and health training requirements and the documentation process;
5.6.2.2.6. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
5.6.2.2.7. Include employee hazard reporting procedures;
5.6.2.2.8. Identify individual(s) responsible for corrective action of hazards;
5.6.2.2.9. Identify first aid/injury procedures;
5.6.2.2.10. Identify procedures for accident investigation and reporting;
5.6.2.2.11. Identify emergency response procedures; and
5.6.2.2.12. Identify the process for tracking controlled hazards in contractor work areas.
5.6.3. The Contractor will submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A, Summary of Work-related Injuries and Illnesses, annually to the contracting office for consolidation and submission as part of the installation’s annual VPP Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each calendar year (CDRL A005).
5.6.4. The Contractor’s Quality Control Plan (CDRL A005) will identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.
5.6.5. The Contractor will establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.
