1. SCOPE

The 527 EMXS Type IIA Precision Measurement Equipment Laboratory (PMEL) supports all common portable Test Measurement and Diagnostic Equipment (TMDE) for

Hill AFB and the surrounding geographical area IAW T.O. 00-20-14 Section 4.1.2

Figure 4-1.

2. DESCRIPTION OF SERVICES

The Contractor shall provide qualified personnel, equipment, materials, management, and services necessary to perform traceable calibration and related maintenance and repair capabilities to operate the Type IIA PMEL at Hill AFB, Utah. All work shall be performed In Accordance With (IAW) the requirements of this Performance-Based Work

Statement (PWS) and all publications cited herein. The Contractor shall comply with this

PWS in its entirety.

2.1 Assessments, Certifications, and Inspections: The Contractor shall comply with and participate in local, base-wide, Air Force, and any other assessments, certifications and/or inspections that involve/require PMEL participation. During any assessment, certification or inspection, the Contractor shall provide all information and access necessary to demonstrate compliance. The Contractor shall take prompt action to resolve all Contractor related deficiencies and recommendations identified during assessments, certifications, and inspections.

2.1.1 Air Force Metrology and Calibration (AFMETCAL) certification:

Immediately upon award, the Contractor shall obtain AFMETCAL certification

IAW T.O. 00-20-14 Section 7.

2.1.1.1 AFMETCAL pre-assessment, onsite assessment, and certification review shall be completed within approximately seven months after contract award. Following the initial certification, AFMETCAL assessments will occur approximately every 2 years for the life of the contract. AFMETCAL certification shall be maintained for the life of the contract.

2.1.2 Inter-PMEL Comparisons and Proficiency Testing: The Contractor shall participate in Inter-PMEL comparisons and proficiency testing IAW T.O. 00-

20-14, Section 11.

2.1.3 Logistics Compliance Assessment Program (LCAP): The purpose of the

LCAP is to provide leadership at all levels with an evaluation of a unit’s ability to perform key logistics processes in a safe, standardized, repeatable, and technically compliant manner. LCAP evaluations are conducted IAW AFI 20-

111.

2.2 Calibration and Repair of Test, Measurement, and Diagnostic Equipments

(TMDE):

2.2.1 TMDE calibration and repair shall be accomplished IAW T.O. 00-20-14

Section 3, Weapon System Calibration Measurement Summaries (CMS), T.O.

33K-1-100-1, T.O. 33K-1-100-2, applicable 33K Series T.O.’s, applicable commercial data, and AFMETCAL approved local calibration procedures.

Calibration authority shall be selected according to the order of precedence outlined in T.O. 00-20-14 Section 3.1.2.

2.2.2 Forms: The Contractor shall complete and attach the appropriate AFTO Forms and/or DD condition tags to all TMDE IAW with T.O. 00-20-14 Section 5, AFMAN 23-110Volume 2 Part 13 and TO 00-20-3 Section 3.

2.2.3 The Contractor shall enter required traceability and Maintenance Data

Collection (MDC) information into the Electronic Facility Equipment

Maintenance System (E-FEMS) or its replacement IAW T.O. 00-20-14 Section

3 and T.O. 33K-1-100-1 Section 2.

2.2.4 The Contractor shall verify TMDE against E-FEMS and notify the PMEL scheduling function of any discrepancies for correction.

2.2.5 Acceptance: The Contractor shall submit completed TMDE to the Contracting

Officer Representative (COR) for acceptance on a daily basis. After acceptance the Contractor shall deliver the TMDE to the PMEL scheduling function.

2.2.6 TMDE Returns: TMDE that fails within 30 days of receipt by the customer due to improper servicing, as determined by the COR, shall be reworked with the same priority as Mission Essential TMDE as defined in TO 00-20-14 paragraph 3.7 Input Priority System. The Contractor shall rework the item at no additional cost to the Government. Disputes regarding the COR determination will be resolved by the ACO.

2.2.7 Technical Assistance: The Contractor shall provide metrology related technical assistance to TMDE owners/users.

2.2.8 Limited Certifications: The Contractor shall document limited certifications

IAW T.O. 00-20-14 Section 3 and record in E-FEMS, limited certifications, required user notification, as well as approvals and denials. At a minimum the

Contractor records shall include the date/time, name of user contacted and approval or denial of limited certification, prior to submitting the item to the

COR for acceptance and/or evaluation.

2.2.9 TMDE Repair: The Contractor shall troubleshoot and repair TMDE to the lowest level possible. The Contractor shall not remove parts (cannibalize) from other TMDE to repair like TMDE unless directed by the COR.

2.2.10 Beyond Economical Repair: If the estimated cost of materials and labor for repair exceeds 75 percent of the replacement cost, the Contractor shall request approval from the COR before proceeding with the repair action.

2.2.11 Not Repairable This Station (NRTS): When an item is determined to be

NRTS, the Contractor shall record in E-FEMS, the reason for the NRTS action, enter the appropriate status code in E-FEMS, and submit the item with the required supporting documentation for COR approval.

2.2.12 Return to Owner (RTO): TMDE is considered RTO when contractor determines work cannot progress due to lack of owner supplied data or accessories. When TMDE is determined to be RTO, the Contractor shall enter the appropriate status code in E-FEMS, and submit the item with supporting documentation to the COR for approval.

2.3 Workload Management

2.3.1 Throughput Time: The Contractor shall accomplish all TMDE workload within a 20 calendar day throughput time, excluding authorized delay time.

Overall, the Contractor shall maintain a 12-calendar day average throughput time. The Contractor shall perform a root cause analysis on each item of

TMDE that exceeds the 20-day throughput time to determine what is responsible for the delay. The Contractor shall report the results of the root cause analysis to the COR and shall actively participate in the development of corrective actions.

2.3.1.1 Throughput Time Calculation: E-FEMS will track each equipment item's throughput time after the PMEL schedulers have entered it as

Received by PMEL (RBP). Day number zero (0) is the day the item is scheduled. Day number one (1) is a day to allow PMEL personnel to move the item to the Contractor's designated location/shelf. Day number two (2) is the first day the item counts against throughput time. Throughput time stops when an item is entered as

AWAITCLOSE. Time accumulated in a Government approved deferred status is not counted against throughput time.

2.3.2 TMDE Categories: TMDE will be categorized by the Government COR as follows:

Category 1: Calibrations estimated to take up to 1 hour

Category 2: Calibrations estimated to take between 1 and 2 hours

Category 3: Calibrations estimated to take between 2 and 3 hours

Category 4: Calibrations estimated to take between 3 and 6 hours

Category 5: Calibrations estimated to take more than 6 hours

2.3.3 Status Changes:

2.3.3.1 The Contractor shall enter historical data, all status changes, and other pertinent data into E-FEMS, while TMDE is in their possession. All Status changes shall be updated within four hours.

Any problems shall be reported to the COR for correction.

2.3.3.2 Follow-up on all items in deferred statuses shall be performed and documented weekly IAW AFI 21-101_AFMCSUP_I chapter

14.51.6.10.1.4.9.4

2.3.4 Maintenance Priorities: The Contractor shall accomplish TMDE on a First In

First Out (FIFO) basis according to the maintenance priority identified by the customer (routine, mission essential, or emergency). The maintenance priorities are defined in T.O. 00-20-14, Section 3.7.

2.3.4.1 The Contractor shall ensure that requests for mission essential or emergency maintenance are received in writing from the

Government. The Contractor shall notify the COR, and provide a copy of each request for mission essential and emergency maintenance the day it is received, or by 0730 the next duty day if received during other than normal duty hours.

2.3.5 Workload Records and Data: The Contractor shall allow the Government unlimited access to all workload records and data.

2.4 Supply Support and Property Control: The Contractor shall accomplish effective and economical supply support and property control. The Contractor shall manage, control, and safeguard all supplies and equipment.

2.4.1 Procurement of Materials and Parts: The Contractor shall order PMEL related materials and parts through Standard Base Supply System (SBSS) or its replacement and/or commercial suppliers.

2.4.2 Requisition of Replacement Equipment: The Contractor shall promptly notify the COR of Non-Equipment Authorized In-Use Detail (Non-EAID)

(Tables, Chairs, etc.) and Equipment Authorized-In Use Detail (EAID) items that need to be requisitioned.

2.4.3 Management of Hazardous Materials:

2.4.3.1 The Contractor shall obtain, inventory, label, and manage all

Hazardous Materials (HM) required in support of this effort under the direction of, and working through, the established Hazardous

Material Pharmacy (HAZMART) IAW AFI 32-7086_HILLAFB_I.

2.4.3.2 HM will be used in the process and manner for which it is intended by the manufacturer and as authorized by the HAZMART. The contractor shall follow the manufacturer’s guidelines and recommendations for use, handling and storage of hazardous materials and flammable and combustible liquids as identified in AFI

32-7086 and NFPA 30.

2.4.3.3 The contractor shall practice housekeeping, storage, and material/work residue methods and procedures that will ensure HM does not migrate out of the immediate work area to become a fire or

FOD hazard.

2.4.3.4 HM shall remain in the manufacturer’s container whenever possible, must be in the employee’s possession during use and shall be stored in an approved HM locker at the end of each shift. All containers for

HM, whether manufacturer or day use, shall have a legible yellow tracking label issued by the HAZMART affixed to them. Containers shall also be labeled as to contents and hazards IAW AFI 90-821 and

Hill AFBI 48-106.

2.4.3.5 Spent or used HM and empty containers shall be collected and disposed of IAW the Hill AFB Hazardous Materials Management

Plan (HMMP) and Hill AFB Hazardous Waste Management Plan

(HWMP).

2.4.3.6 Contractor personnel shall complete the Basic Hazardous Material

Management Process training (Course #587) prior to transporting or using any potentially hazardous materials on the installation.

2.4.3.7 The contractor shall have knowledge of and comply with all applicable Interstate, Federal, State, Local and Hill AFB laws regulations and requirements regarding environmental compliance and pollution prevention. In the event environmental laws or regulations change during the term of this contract the contractor is required to comply with such changes as they come into effect. If there is a change in cost as a result of the change the contractor shall inform the ACO pursuant to notice requirements. The contractor is responsible for all fines and penalties resulting from environmental compliance actions imposed by regulators.

2.4.3.8 If the contractor spills or releases any substance contained in 40 CFR

302 into the environment the contractor or its agent shall immediately take action to contain the spill and shall report the incident to the Hill AFB Fire Department at 777-1911. 75CEG/CEV can provide a list of approved spill cleanup contractors. Liability for the spill or release of such substances rests solely with the contractor.

2.4.3.9 The Contractor shall appoint a Hazardous Waste Site Manager and alternate site manager. The Site Manager and alternate shall obtain the required Hazardous Waste Site Manger Training. The Site

Manager and alternate will manage the hazardous waste generated by the 527 EMXS PMEL IAW the Hill AFB Hazardous Waste

Management Plan.

2.5 Quality Control: The Contractor shall develop, implement, and maintain a comprehensive management system that assures compliance with all requirements of this PWS. As a minimum the management system shall implement a Quality

Program IAW with T.O. 00-20-14 Section 9 and include a quality manual IAW T.O.

00-20-14, Section 10.2 and any other policies or procedures required by TO 00-20-

14 and other laws, regulations and guidance applicable to this effort. The Contractor shall make changes as necessary and submit changes to the COR.

2.6 Reports: The Contractor shall provide a summary of Production and Quality

Metrics to the COR by the 10th calendar day of each month. (CDRL A001)

2.7 Personnel: The Contractor shall employ and utilize qualified personnel with appropriate combinations of education, experience, knowledge, training, and skills necessary to complete the services described in this PWS. The Contractor shall be responsible for determining the size and makeup of the Type IIA PMEL workforce with the following minimum requirements:

2.7.1 Program Manager: The Contractor shall provide a full time on-site Program

Manager who shall be responsible for the performance of all work required by this contract. Prior to the performance start date and as changes occur, the

Contractor shall provide in writing to the ACO, telephone numbers of the

Program Manager and alternate where they can be contacted during and outside of normal duty hours. The Program Manager shall have sufficient knowledge to understand, communicate, and manage the terms of this contract, have full authority to obligate the Contractor on all matters relating to the execution of this contract and be available during normal duty hours to meet the Government Program Office or ACO to discuss contract issues. The

Program Manager shall be available in emergency situations.

2.7.2 PMEL Manager: The Contractor shall provide a PMEL Manager who shall be responsible for the daily performance of work required by this contract. The

PMEL Manager shall have knowledge of electronic and physical/dimensional principals; troubleshooting techniques, calibration traceability, metrology techniques and Air Force PMEL laboratory practices, policies, and procedures.

The PMEL Manager or designated alternate shall be available on site at the

Hill AFB PMEL facility during normal duty hours.

2.7.2.1 As a minimum, the PMEL Manager shall have experience in the following areas: planning, directing, and supervising employees in functions such as inspecting, troubleshooting, repairing, modifying, overhauling, aligning, and calibrating (automatic and manual calibration) complex and intricate TMDE and laboratory measurement standards; developing policies, instructions, procedures and operational guidance for PMEL operations; establishing daily and long range plans for various projects and work operations;

advising management on and coordinating training programs and requirements, manpower and personnel, facility, and budget requirements; safety, quality control and assurance programs;

reimbursement procedures; equipment scheduling, production, and materiel control; submitting maintenance data; support equipment management.

2.7.3 Quality Manager: The Quality Manager shall have knowledge and experience in Air Force PMEL quality management concepts and practices outlined in

T.O. 00-20-14 Section 9, and shall have overall responsibility for the

Management system and its implementation.

2.7.4 Training Manager: Training Manager shall be responsible for overseeing training, and coordinating joint curriculum development and documentation with the Government PMEL Training Manager.

2.7.5 Additional Personnel Requirements:

2.7.5.1 All personnel shall read, write, speak and understand English and be legal citizens of the United States.

2.7.5.2 The Contractor shall maintain a current listing of all personnel assigned to the contract and shall provide a list of those names to the

Government COR upon request.

2.7.5.3 Personnel shall at all times, wear some form of visible identification showing their status as a Contractor Employee. Contractor identification shall include conversations, mail, email, teleconferences, video teleconferences, faxes, and/or other electronic communication whether with Government personnel, other contractor personnel, or with the public when supporting this contract where their status as Contractor employees might not otherwise be apparent or where they might be mistaken for civil service employees.

2.8 Program Management Reviews (PMRs): The Contractor shall conduct formal

PMRs, which at a minimum will address Training, Quality, Throughput, Backlog, Deferred Status, Contractor Deficiency Reports (CDRs). PMRs shall be conducted with the Government on a quarterly basis unless otherwise directed by the ACO.

2.9 Travel: The Contractor may be required to travel in support of the performance of this PWS. All travel shall be IAW FAR 31.205-46. The Contractor shall be responsible for making travel and lodging arrangements. The number of trips and personnel shall be limited to the minimum required to accomplish the work. The

Contractor shall obtain prior written approval from the ACO for all travel.

2.9.1 The Contractor may be required to attend project meetings, technical conferences, trainings, etc, at offsite locations in the local area.

3. GENERAL REQUIREMENTS

3.1 Training

3.1.1 Training Responsibility: On the performance start date, the Contractor shall provide trained personnel to meet the requirements of the PWS. The

Government may provide technical training for new Government equipment requirements, as determined by the ACO.

3.1.2 Government Provided Training: The Contractor shall ensure all employees complete training required for base access, network access, safety, etc.

Appendix G lists the training that will be given through the Center Training

Office (CTO) at HAFB at no expense to the Contractor. The training list is not all-inclusive and additional training may be required. Course numbers may change as training courses are updated.

3.1.3 Training Records: The Contractor shall maintain records on file in the

Contractor PMEL office or electronically for each employee on the relevant qualifications, training, skills, and experience of technical personnel. These records shall be of sufficient detail to provide confidence in the task competence of assigned personnel. The Contractor shall provide the

Government access to these records upon request.

3.2 Security

3.2.1 General Security Requirements:

3.2.1.1 The Contractor shall complete a DD 1172-2 to obtain a Base

Common Access Card (CAC) card for all of their employees and submit to the appropriate security manager. The Contractor shall obtain a Building ID and access card from the appropriate Building

Security Manager for all of their employees.

3.2.1.2 The Contractor shall complete an Unescorted Entry Authorization

Certificate (AF Form 2586) for each employee requiring entry into controlled areas. The request shall be submitted to the HAFB Pass and ID Office. The Government will provide an AF Form1199, Air

Force Entry Control Card. The credentials shall be properly displayed while in an authorized controlled area and surrendered as directed when access to controlled areas is no longer required.

3.2.2 Computer Security: Contractor personnel shall comply with the current Air

Force requirements for access to Air Force computer resources and network access, to include completing the current on-line Air Force computer security training. This training must be completed in order for an employee to receive an account on the system.

3.2.3 Operations Security (OPSEC): All personnel (contractors, subcontractors, their employees, associates, and consultants) who have access to critical information must participate in the appropriate OPSEC programs IAW AFI

10-701.

3.2.3.1 The contractor will present initial and recurring (annual) training to all personnel working on the contract or having access to critical information associated with the contract. The training must be provided prior to personnel being allowed access to critical information and annually thereafter or as individual programs may require. The 309 EMXG OPSEC program manager/coordinator will provide the training curriculum to the contractor. The training will provide listings of critical information that must be protected, the

OPSEC measures which must be used to protect the critical information and foreign intelligence and industrial espionage threats.

3.2.3.2 The contractor must track OPSEC training statistics to include personnel working on the contract, date(s) training was completed and when refresher training is due. Training statistics will be reported to the 309 EMXG OPSEC program manager/coordinator at the end of each fiscal quarter and upon demand.

3.2.3.3 Additional training may be required based on updates to critical information, OPSEC measures, intelligence threats, or fundamental changes to the OPSEC program.

3.2.3.4 All personnel must report OPSEC related incidents, issues and concerns to the 309 EMXG OPSEC program manager/coordinator.

3.2.3.5 Information to be released outside official US Government channels must be presented to the 309 EMXG Program Manager for an

OPSEC review and approval prior to seeking approval from the

Public Affairs office for release.

3.2.4 Force Protection Conditions (FPC): The Contractor shall respond to all FPCs mandated by the United States Air Force, the 75ABW Commander and the th

MXW, IAW AFI 31-101 Hill AFB Sup. The Contractor shall respond and support all Force Protection awareness exercise scenarios. The Contractor will support other Force Protection Conditions, as directed by the ACO.

3.3 Installation-Specific Safety and Health Standards

3.3.1 Voluntary Protection Program

3.3.1.1 The Air Force, has adopted the Voluntary Protection Program (VPP) as recognized by Occupation Safety Health Association (OSHA).

Consequently, VPP impacts all “applicable Contractors” operating on Air Force Installations. It is the Contractor’s responsibility to ensure its employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. Detailed information on VPP is available on the

Contractors, whether regularly involved in routine site operations or engaged in temporary projects such as construction or repair, must follow the safety and health rules of the installation or VPP site.

3.3.1.2 VPP requirements apply to Contractors whose employees work more than 1,000 hours per quarter. The Contractor shall flow down these requirements to applicable Subcontractors.

3.3.1.3 Contractors are required to provide from their OSHA 300 information the Total Case Incident Rate/Days Away Restricted or

Transferred (TCIR/DART) rates. (CDRL A002)

3.3.2 Safety Program

3.3.2.1 The Contractor shall implement a safety program that ensures protection of Government personnel and property. (Appendix C)

3.3.2.2 As part of the Safety Program the Contractor shall establish a Safety

Plan and checklist IAW Appendix C of this PWS. The Contractor shall submit any changes to the Safety Plan to the ACO. (CDRL

A003)

3.3.2.3 In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence. A written or email copy of the mishap / incident notification shall be sent within three calendar days to the Government Representative, who will forward it to 309 MXW safety. (CDRL A004)

3.4 Contractor Furnished Items: The Contractor shall furnish everything required to perform this contract, except items listed in the GFE/GFP/GFM section and

Appendix E of this PWS. At a minimum the Contractor shall furnish the following items:

3.4.1 Office supplies (paper, ribbons, tape, pens, pencils, stamps, etc.) as required for its own operations.

3.4.2 Office equipment not specifically listed as GFE, such as copy machine(s), FAX machine, or personal computer equipment as required for its own operations. The Government will provide computers and equipment needed to interface with Government systems.

3.4.3 Computer supplies (paper, printer cartridges, mouse pads, etc.) to be utilized on Government provided computers and equipment as required for its own operations.

3.4.4 Janitorial supplies as required to maintain a clean, safe, and sanitary environment. Chemicals purchased to meet this requirement shall be managed

IAW AFI 32-7086 Hazardous Materials Management.

3.5 Publication Responsibilities

3.5.1 T.O. Deficiency Reporting: The Contractor shall complete AFTO Form 22s, Technical Manual (TM) Change Recommendation and Reply, IAW T.O. 00-

20-14 paragraph 3.1.8 and T.O. 00-5-1 Chapter 9.

3.5.2 T.O. Verifications: The Contractor shall perform Calibration Technical Order

Verifications (TOV) as requested by AFMETCAL and IAW T.O. 00-20-14

Paragraph 3.1.7.3. TOVs shall be completed by the suspense date except for

AFMETCAL approved extensions.

3.6 Hours of Operation: The Contractor shall maintain the following minimum customer service hours: 6:00 a.m. through 3:30 p.m. local time, Monday through

Friday utilizing a 5, 4, 9 work schedule, excluding Federal holidays. Contractor shall observe recognized federal holidays.

3.7 Government Closures: The Contractor shall not work in Bldg 214, nor should any deliveries be made to Bldg 214 unless authorized by the ACO on any of the following holidays:

Martin Luther King Jr.- Birthday – 3rd Monday in January

President’s Day – 3rd Monday in February

Memorial Day – Last Monday in May

3.8 Continuation of Essential Contractor Services: During periods of crisis, the

Contractor shall be prepared to continue providing services designated as essential in accordance with requirements listed in Appendix H.

3.9 Conservation of Utilities: The Contractor shall be responsible for operating under conditions which prevent the waste of utilities. Government telephones shall be used only for official Government business.

3.10 Transition: The Contractor shall fully cooperate with incoming and outgoing personnel for assumption of and relinquishing contractual responsibilities to permit an orderly and timely changeover.

3.10.1 Phase-In: The Contractor shall interface with Government and incumbent

Contractor personnel to assume responsibility for contract performance. The phase in period shall include transition of all necessary transition tasks including inventories, personnel turnover, and transfer of documentation, supplies and equipment. The Contractor shall assume responsibility for all workload and PMEL operations on the contract performance start date. The phase in period shall be a maximum of 30 days, but may be less at the incoming contractor’s discretion.

3.10.2 Phase-Out: The Contractor shall interface with the government and incoming contractor to relinquish contractual responsibilities. During the phase-out period the contractor shall be responsible for all transition tasks including inventories, personnel turnover, and transfer of documentation, supplies and equipment in order to relinquish responsibility for contract performance. The

Contractor shall maintain responsibility for all workload and PMEL operations until the contract performance end date. The phase out period shall be a maximum of 30 days, but may be less at the incoming contractor’s discretion.

6.2.2

5. GOVERNMENT SURVEILLANCE

5.1 Contracting Officer’s Representative (COR): The COR is the authorized

Government representative(s) who will assess the Contractor’s performance.

Subsequent to contract award, the identity of all CORs, along with their duties and authority, will be provided in the Quality Assurance Surveillance Plan.

5.1.1 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the ACO and Not to the COR.

5.2 Quality Assurance Surveillance Plan (QASP): The QASP is the key Government-developed surveillance process document that is applied to Performance-Based

Service Contracting (PBSC) and is used to manage Contractor performance assessments by ensuring that systematic quality assurance methods are utilized to validate the Contractor's quality control efforts are timely, effective, and are delivering the results specified in the contract. The QASP directly corresponds to the performance objectives and standards (i.e., quality, quantity, timeliness) specified in the Performance Work Statement (PWS) and details how, when and by whom the

Government will survey, observe, test, sample, evaluate, and document Contractor performance results, to determine whether the Contractor has met the required standards for each objective in the PWS.

6. GOVERNMENT FURNISHED PROPERTY AND SERVICES

6.1 Government Furnished Facilities

6.1.1 The Government will furnish the facilities required to perform the services of this PWS. The Government furnished facility shall be used for this contract effort only. The Contractor shall be responsible for day-to-day operation of the assigned facilities to include identifying problem areas to the COR. All repairs shall be accomplished through the Civil Engineering Squadron (CES) or Plant

Services unless otherwise directed by the ACO.

No alteration to the facilities shall be made without specific written permission from CE or Plant Services and authorization from the ACO.

6.1.2 Environmental Control: The Contractor shall monitor all environmental recorders within the Type IIA Facilities IAW T.O. 00-20-14 Section 8.2. When the environmental conditions are "Out-of-Tolerance", the Contractor shall adjust and/or curtail performance IAW T.O. 00-20-14 applicable TMDE technical orders, and commercial data. The Contractor shall notify the COR and the facility manager within 30 minutes of an out-of-tolerance condition as defined in T.O. 00-20-14, Section 8.2.

6.1.3 Housekeeping and Custodial Service: The Contractor shall be responsible for cleaning all items and work areas in its assigned laboratory, office, and storage areas IAW T.O. 00-20-14, Section 3.10.2.s.

6.2 Government Furnished Equipment (GFE)/Government Furnished Property

(GFP):

6.2.1 The Government will provide the Contractor with the GFE and GFP listed in the appendices. The Contractor shall maintain and calibrate, all GFE/GFP as required to perform the requirements of this PWS.

6.2.2 Property Control: The Contractor shall establish and maintain acceptable property management plans and systems IAW applicable contract clauses. The

Contractor shall submit a request for the addition or deletion of GFP or GFE to the ACO through the COR. Failure to maintain an acceptable property management system, as defined, may result in disapproval of the system by the

Contracting Officer and/or withholding of payments.

6.2.3 Equipment Inventory: The Contractor and a Government representative

(identified by the ACO) shall conduct a joint incoming inventory of all items listed in PWS Appendix E no later than five (5) calendar days before full performance and within 10 calendar days of the start of any option periods.

Upon inventory completion, the Contractor shall sign a receipt for all equipment provided by the Government.

6.2.4 Baseline Calibration Limitation (BCL) Listing: The BCL Listing shall establish a baseline for GFE limitations and shall be maintained throughout the life of the contract. During the inventory, the Contractor and Government representative shall prepare a BCL Listing, in electronic format, identifying all

GFE with calibration limitations as reflected on AFTO Forms 99 or 398. As a minimum, the BCL Listing shall include the equipment model/part number, serial number, ID number, and a description of the calibration limitation exactly as it appears on the AFTO Form 99 or 398. Any changes to the BCL

Listing shall be approved by the ACO.

6.2.4.1 At the end of the contract period GFE that is missing or found with calibration limitations that were not identified on the BCL Listing or directed by Air Force technical order shall be replaced and/or repaired/calibrated at no additional cost to the Government.

6.2.5 Custodian Authorization/Custody Receipt Listing (CA/CRL) and

Equipment Authorization Inventory Data (EAID): The Contractor shall furnish a primary and an alternate equipment custodian to act as the focal point for all equipment identified in Appendix E in accordance with AFMAN 23-

110, USAF Supply Manual. The Contractor shall acknowledge receipt for equipment inventoried and applicable to the EAID records on the CA/CRL.

The Contractor shall conduct and document the CA/CRL inventory as required

IAW AFMAN 23-110, Volume 2, Part 13. The Contractor shall justify and submit requirements to the CFM for additional equipment and/or turn-in of

EAID equipment on AF Form 2005, Issue/Turn-In Request and/or AF Form

601, Equipment Action Request.

6.2.6 Working Measurement Standards: The Contractor shall repair, calibrate, and perform preventive maintenance on all working measurement standards

IAW T.O. 00-20-14 Section 3, except for those working measurement standards designated in T.O. 33K-1-100-1 or -2 to go to the Air Force Primary

Standards Laboratory (AFPSL), an AFMETCAL approved vendor, or other servicing agency for calibration. The Contractor shall turn the AFPSL, vendor or other agency items over to the PMEL scheduling function for proper disposition.

6.2.7 Shared Equipment: As workload permits, the Contractor shall allow

Government personnel the use of the equipment listed in PWS Appendix E.

The Contractor shall implement a process to track the loaned equipment.

6.2.8 Certification Stamps ("K Stamps"): The Government will provide certification stamps ("K Stamps") to the Contractor. The Contractor shall manage these stamps as a controlled item and shall be responsible for issuance, inventory, and control. All certification stamps ("K stamps") shall be accounted for and returned to the Government upon contract completion.

6.2.9 Hand Tools: The Government will furnish to the Contractor all hand tools required to perform the services of this PWS IAW AFI 21-101 AFMC

Supplement I Chapter10 and local tool policies.

6.2.9.1 The Contractor shall control all tools, accessories and fixtures IAW

AFI 21-101 AFMC Supplement I Chapters 10.11 through 10.13 and local tool control policies.

6.3 Government Furnished Materials

6.3.1 The Government will pay for materials used by the Contractor in performance of the PMEL services for the duration of the contract unless otherwise specified. The initial stock of materials shall be inventoried by the Contractor and a Government PMEL representative no later than five (5) working days before the contract performance start date. Any missing items shall be annotated and the ACO immediately notified. Any disagreements between the

Contractor and the Government PMEL representative on the materials inventoried shall be resolved by the ACO. The Contractor shall be responsible for keeping enough materials on hand for the performance of the contract IAW its terms. If additional materials are required, the Contractor shall request such additional materials by providing a written request to the ACO. Upon approval by the ACO, said additional materials will be provided at Government expense.

At the conclusion of the contract period the Contractor shall return all residual materials to the Government IAW the applicable clauses cited in the basic contract. A closing inventory will be conducted by the Contractor and a

Government PMEL representative, and the report will be given to the ACO prior to contract completion date.

6.4 Government Furnished Records, Files, Documents, And Work Papers

6.4.1 All records, files, documents, and work papers, including all electronic records, files and communications, provided or generated in support of this contract are

Government property and shall be maintained and disposed of IAW Air Force

Records Information Management System (AFRIMS). At the time of disposition the Contractor shall box, label, and deliver all records, files, documents, and work papers to the COR.

SAFETY, FIRE PROTECTION AND HEALTH SPECIFICATION

INDUSTRIAL SAFETY REQUIREMENTS

TYPE WORK: PMEL Support

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

In context with AFMC Acquisition Reform initiatives, contract language will now focus on ensuring safety objectives are met through Contractor risk management rather than the traditional method of providing specific Government guidance for risk elimination. With this new “performance-based” language, the Government will provide the Contractor with areas of concern (Protection of Government personnel and property) and the Contractor will be free to meet those objectives in a manner that is consistent with corporate standards. The Contractor will implement a safety program plan that ensures protection of Government personnel and property. The program will consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan and corresponding site specific checklist that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and will be used during the performance of the work described in the contract. The Safety Plan and checklist will be approved by 309

MXW/QPE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All Contractor personnel shall be trained and qualified to perform their duties safely.

7. The Contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

1. The Contractor shall notify 309 MXW Safety (Phone (801) 586-4176) who in turn will notify

75 ABW/SEG ((801) 777-3333) or 309 Wing Operations Center ( (801) 777-3238), or the Hill

AFB Command Post (777-3007) after normal duty hours, and the designated Government

Representative, i.e., the ACO, PCO, or QAE within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the Government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to 309 MXW

Safety. For information not available at the time of initial notification, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

2. Ensure Government contract lease agreements determine convening authority prior to executing the contract/lease agreements, and that all contracts ensure the following, IAW AFI

91-204, paragraph 2.4.6:

The Contractor will support and comply with the safety investigation and report requirements of AFI 91-204

A record of all mishaps involving Air Force resources will be entered into the Air Force reporting system

A record of mishaps involving other DoD resources will be forwarded to the involved agencies

When a mishap involves a contract managed by DCMA, DCMA safety personnel will review the final safety report and send their response to HQ AFSC

3. Ensure contract/lease agreements for aerospace vehicles state: The Air Force is responsible for the investigation of mishaps involving aerospace vehicles. The Air Force is authorized to investigate mishaps involving non-accepted Air Force aerospace vehicles (Non-delivered vehicles for which the Government has accepted responsibility; DD Form 250. Material

Inspection and Receiving Report, HAS NOT been executed).

4. If requested by authorized Government personnel (ACO, PCO, QAE, safety representative, Fire Department, and Security Forces), the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

C. General Safety Requirements:

If the safety plan is modified, the Contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative.

Follow all applicable federal, USAF, state, and local safety directives outlined in Section

II below.

Ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels.

Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the 309 MXW

Safety Office.

For emergencies on Hill AFB contact 911 ask to be transferred to Hill AFB 911 to report any emergency.

SECTION II – SPECIFIC REQUIREMENTS

The Contractor’s prepared Safety Plan shall:

Demonstrate a management commitment to employee safety and health

Identify applicable rules and regulations

Identify the roles and responsibilities of Management, Supervisors, Employees and

Safety Coordinator

Identify work to be performed and location of expected operations

Provide a description of safety program, safety monitoring responsibilities, organizational structure, and contact information for on-site personnel

Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures

Identify employee safety and health training requirements and the documentation process

Include emergency response plans and procedures that relate to protection of Government personnel and property

Include a workplace inspection frequency, to include the identity of the individual responsible for conducting the inspection
