All the requirements specified in the solicitation are mandatory. By the Offeror’s proposal submission, the Offeror is representing that the company/firm will perform ALL the requirements specified in the solicitation. It is not necessary or desirable to tell us so in the proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
4.1.1. Adequate Responses: Cursory responses or content which merely reiterates or reformulates sections L and M or the TRD will not be considered adequate. Evidence, which clearly demonstrates and supports the Offeror’s claims, is essential--the absence of such evidence may result in a technically unacceptable proposal.

3. Specifically, describe each process in detail, as specified in the applicable Technical Orders (TOs) and Military-Standard (MIL-STD) requirements to include all tooling, equipment, and material necessary for proper and complete remanufacture within the 60 day delivery requirement.
4.2.5. Sub-Factor Five, Qualification and Certification: The Offeror shall submit its ISO 9001 or AS 9100 compliance, or equivalent documentation. Any equivalency documentation must clearly demonstrate its equivalence to ISO or AS certifications.
5.0. Volume III (Factor Two) – Past Performance
Volume Organization: The Past Performance volume shall be organized according to the following general outline:
1. Table of Contents
2. Summary Table
3. Past Performance Information Sheets (PPISs) (Attachment 1)
4. Subcontractor/Teaming Partner Consent Letter for the Release of Past and Present Performance Information to the Prime Contractor (Attachment 2)
5. Client Authorization Letter (Attachment 3)
6. Roles and Responsibilities Worksheet (Attachment 4)
7. Glossary
5.1. Past Performance Volume Submission: Offeror is encouraged to submit its Past Performance Volume a minimum of 10 days prior to the date set forth in block 9 of the SF 1447 for receipt of proposal.
In addition to providing hard and soft copies as required in section 2 of this document, Offerors shall use the PPI Tool (EZ Source-Use latest version from FBO.gov) to submit the Past Performance Volume in accordance with the RFP. See Attachment 7 for detailed instructions.
5.2. Summary Table: The Offeror shall submit a Summary Table (Table 4). The purpose of the summary table is to capture the total scope of work that the Offeror is presenting in this Past Performance Volume. The Government may obtain and use performance information on any or none of these programs. The table shall include (as a minimum) current information as follows:

5.3. Past Performance Details: If recent and relevant past performance exists, each Offeror shall submit the following documentation:
5.3.1. Past Performance Information Sheets (PPIS): The contractor shall provide PPIS’ IAW the format contained in Attachment 1, Past Performance Information Sheets. Offerors may submit a maximum of four (4) PPISs for the prime contractor and a maximum of four
(4) for its proposed subcontractors for recency and relevancy, for a maximum of eight (8) total PPISs.
5.3.2. Completed Roles and Responsibilities Worksheet: The contractor shall provide a completed roles and responsibilities worksheet in accordance with the format contained in Attachment 4, Roles and Responsibilities Worksheet, for all proposed subcontractors performing direct labor for this effort. The Offeror has discretion in determining the most beneficial mix and quantity of contracts by prime Offeror, key partners, or subcontractors covering as many of these elements as is practical. For a description of the characteristics or aspects the Government will consider in determining recency and relevancy, see section M, Evaluation Factors for Award.
5.3.3. Past Performance Questionnaire (PPQ):Offerors shall invite the third-parties identified on the PPISs to complete and transmit the Questionnaire (Section L, Attachment 5) to the Air Force describing their experiencing regarding the Offeror. The third-party POCs are instructed (on Section L, Attachment 5) to complete the Questionnaire and transmit it directly to the Air Force evaluators in electronic format. The third-parties are further instructed NOT TO transmit the completed Questionnaires back to the Offeror but to the Air Force evaluators (identified emails on questionnaire document). Questionnaires transmitted directly by the Offeror will not be evaluated, or considered. Once questionnaires are completed they shall be considered source selection sensitive and shall not be released to the Offeror.
5.3.4. Consent Letter: The Offeror shall also submit a Subcontractor/Teaming Partner Consent Letter for the Release of Past and Present Performance Information to the Prime Contractor (Attachment 2) executed by each subcontractor, teaming partner, and joint venture partner as applicable, identified on Attachment 4, Roles and Responsibilities Worksheet, authorizing release of adverse past performance information to the Offeror so the Offeror can respond to such information. A Subcontractor/Teaming Partner Consent Letter for the Release of Past and Present Performance Information to the Prime Contractor is provided because past performance information pertaining to a subcontractor cannot be disclosed to the prime Offeror without the subcontractor’s consent. Provide with the proposal a letter from subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
Section L - pg 13 (of 33)
5.3.5. Client Authorization Letter: For each identified effort for a commercial customer, the Offeror shall submit a Client Authorization Letter (Attachment 3), authorizing release to the Government of requested information on the Offeror’s performance. A Client Authorization Letter is required because the government needs requested information on the Offeror’s past performance and the government will need consent before contacting commercial customers to assess the Offeror’s past performance.
5.4. Organizational Structure Change History: Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a “roadmap” describing all such changes in the organization of the Offeror’s company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts the Offeror identifies for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the Offeror(s), the Offeror’s “roadmap” should be both specifically applicable to the efforts identified and general enough to apply to efforts on which the Government receives information from other sources.
5.5. Contractor Relationships: If the customer for any PPIS is a potential Offeror for this solicitation, the Offeror shall provide the POCs for the next higher level customer. Example, if Contractors X and Y are both potential Offerors for this solicitation and Contractor X was a subcontractor to Contractor Y on workload performed for the government, then Contractor X shall provide both Contractor Y and the government POCs for each applicable PPIS.
Provide information regarding predecessor companies, key personnel who have relevant experience or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the acquisition and will be considered in the evaluation. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts. As with all past contracts identified in the proposal, use the PPIS (Attachment 1).
5.6. Clarifying Submitted Past Performance: The Government may contact the points of contact provided by the Offeror for the purpose of ascertaining and clarifying past performance information.
5.7. Other Performance Confidence Considerations: It is not presumed that the Offeror’s past performance has been perfect. Rather, the proposal should contain evidence of the Offeror’s ability to isolate the root causes of problems and should include in their PPIS a description of programs or actions taken to resolve those causes. Problems not addressed by the Offeror, but
Section L - pg 14 (of 33) found by the Government during the evaluation of the information in this volume, will be assumed to still exist. Note: In the case of Contractor Performance Assessment Report System (CPARS), if the Offeror’s input has already been provided and the rationale/circumstances have not changed, DO NOT repeat them here. The Government will use data provided by each Offeror in this volume and may use data obtained from other sources in the development of the confidence assessment.
6.0. Volume IV (Factor Three) - Price
Volume Organization: The Price Volume shall consist of the following sections:
1. Table of Contents
2. Pricing information and supporting data, to include estimating methodology
3. Glossary
6.1. General: The price evaluation criteria consists of price reasonableness, balance, and a Total Evaluated Price (TEP) as defined in the evaluation document of the RFP. The TEP will be assessed from the Offeror’s information provided in this volume.
6.1.1. Price Reasonableness and Balance
This section is to assist you in submitting information other than certified cost or pricing data that is required to evaluate the price reasonableness and balance of your proposed cost/price.
Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal. Additionally, unbalanced pricing poses an unacceptable risk to the Government and may be a reason to reject an Offeror’s proposal. Offers should be sufficiently detailed to demonstrate their reasonableness and balance. The burden of proof for credibility of proposed prices rests with the Offeror.
6.1.2. Total Evaluated Price (TEP):
(a) Insert proposed unit pricing in TEP Workbook.
(b) The proposed unit pricing contained in TEP Workbook will be used to develop the TEP. The TEP is for award determination purposes only and will be a consideration in the best value award decision.
6.1.3. Price Information: Information beyond that required by this instruction shall not be submitted, unless Offeror considers it essential to document or support their price position.
All information relating to the proposed price including all required supporting documentation must be included in the Price Volume and Contract Document Volumes.
Under no circumstances shall this information and documentation be included elsewhere in the proposal.
6.1.4. Pricing Information Requirements: In the event that adequate competition does not emerge in response to the solicitation, the PCO may require submission of certified cost and pricing data, and a Certificate of Current Cost or Pricing Data (IAW FAR 15.403-4).
Section L - pg 15 (of 33)
IAW FAR 15.403-1(b) and 15.403-3(a), information other than certified cost or pricing data may be required to support price reasonableness. If, after receipt of proposals, the PCO determines that there is insufficient information available to determine price reasonableness the Offeror shall be required to submit other than certified cost or pricing data; if none of the exceptions in FAR 15.403-1 apply, the Offerors may be required to submit certified cost or pricing data. Information shall be provided in format that is IAW FAR 15.403-5.
6.1.5. Rounding: All dollar amounts provided shall be rounded to the nearest U.S. dollar.
6.1.6. Estimating Methodologies: Estimating methods used in proposal shall be described in a manner that will show clear traceability into the unit pricing proposed throughout the pricing volume.
6.1.7 Approved Accounting System: As portions of this contract will be other-than Fixed Price the Offeror must provide evidence of an approved accounting system for themselves and any subcontractors who will be issued other-than Fixed Price subcontracts.
7.0. Volume V - Contract Documentation
1. Table of Contents
2. Model Contract/Representations and Certifications
3. Glossary
7.1. Model Contract/Representations and Certifications: The purpose of this volume is to provide information to the Government for preparing the contract document and supporting files.
7.2. Solicitation/Contract Form:
(a) Complete blocks 13 and 27 of the RFP Section A. In doing so, the Offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. Sections A through J constitute the model contract. Offerors shall include signatures on all amendments associated with this RFP.
(b) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections C through I, the Offeror shall submit only those pages that require a fill-in.
(c) Offeror shall provide a Subcontracting Plan – If the Offeror is other than a small business, the Offeror shall submit a Small Business Subcontracting Plan in accordance with FAR 52.219-9, Small Business Subcontracting Plan that separately addresses and specifies the extent of Offeror's commitment to the participation of small businesses (SB), Veteran-owned Small Business (VOSB), Service Disabled Veteran-Owned Small Business (SDVOSB), HUBZone Small Business (HUBZONE), Small Disadvantaged Business (SDB), and with Woman-Owned Small Business (WOSB) concerns with a separate part with the basic contract and separate part for each option (if any). The plan shall be included in and made a
Section L - pg 16 (of 33) part of the resultant contract. Failure to submit and negotiate the subcontracting plan shall make the offeror ineligible for award of contract. If applicable, submit a copy of your approved master plan. In the event the offeror has negotiated a comprehensive subcontracting plan pursuant to DFARS 219.702, the small business contracting program, statutory requirements, the offeror must submit the information that identifies and specifies the extent of their commitment to the participation of SB, VOSB, SDVOB, HUBZONE, SDB and WOSB. These targets will be provided in the small business subcontracting plan
IAW FAR 52.219-9.
7.3. Exceptions to Solicitation Requirements
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub-factors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, price, and specific requirements of the solicitation. This information shall be provided in the format and content of Table 6 – Solicitation Exceptions below.

ATTACHMENT 2: SUBCONTRACTOR/TEAMING PARTNER CONSENT LETTER
FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO
THE PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime contractor during exchanges. In an effort to assist the Government’s Performance Confidence evaluation team in assessing the Offeror’s past performance relevancy and confidence, we request that the following consent letter be completed by the major subcontractors/teaming partners identified in the Offeror’s proposal. The completed consent letters shall be submitted as part of the Offeror’s Past Performance Volume.

ATTACHMENT 3: CLIENT AUTHORIZATION LETTER
Past performance information concerning private sector contractors, subcontractors and joint venture partners cannot be disclosed to the government without their consent. Client authorization letters are required for each identified effort for a commercial customer. This letter will authorize release to the Government of requested information on the Offeror's performance.
The government will need that consent before contacting commercial customers to assess the Offeror’s past performance. In an effort to assist the Government's Performance Confidence evaluation team in assessing the Offeror’s past performance relevancy and confidence, we request that the following client authorization letter be completed by any commercial customers identified in the Offeror’s proposal. The completed client authorization letters shall be submitted as part of the Offeror’s Past Performance Volume.
Offerors, including contractors, subcontractors and joint venture partners, should send with their list of references, a letter similar to the following authorizing the reference to provide past performance information to the Government.
