STATEMENT OF WORK
IDIQ CARPET
HILL AFB, UTAH
9 July 18
Location: Building 507 Room 252 Project Title: Bldg. 507 Room 252 Remove and Replace Carpet and Cove Capital Number: 1026842
1. General Requirements
1.1 The contractor shall provide all labor, equipment, testing, transportation, and supervision to accomplish the following task in accordance with requirements contained in the original contract specifications.
1.2 The contractor shall provide a TURN KEY proposal for all areas of the project, when completed and ready for final inspection, shall meet all pertaining codes and governments’ statement of work (SOW).
1.3 Only items incorporated into this document from the accepted proposal shall be considered part of this statement of work.
1.4 The contractor shall notify the Project Managers before beginning any work.
1.5 The contractor shall comply with UFC 1-200-01 (Edition in effect at award), IBC Codes, NFPA/International Fire Codes, NEC Codes, International Mechanical/Plumbing Codes, Hill AFB Section 1000 Standards, ASHRAE Standards, AWWA, State of Utah Administrative Code, Hill AFB Design Standards, the 75th CS TAB K, dated 1 April 2010 and all manufacturers recommend installation, start-up and operation instructions.
1.6 The contractor shall have three (3) days to complete any punch list items upon completion of the project and prior to 100% beneficial occupancy is granted, based upon final visual inspection/walk thru with the customer, PK contracting officer (CO), Project Manager, contractor/sub-contractor(s).
1.7 The contractor shall dispose of all trash, empty containers, and removed materials, etc., used or generated in the performance of their tasks. No waste materials shall be placed in any trash receptacle on Hill AFB. The Contractor must comply with all federal, state, and local regulations regarding the transport and disposal of any/all wastes generated as a result of floor covering activities under this contract.
1.8 The contractor shall use qualified sub-contractors with work experience in flooring trade.
1.9 The contractor and sub-contractors must visit and inspect jobsite prior to bidding project, to verify locations. After or upon contractor inspection of jobsite, contractor may notify the Project Manager with questions of any deficiencies that might require modifications to the government statement of work (SOW), prior to contract award.
1.10 The contractor shall conform to all safety requirements contained in all applicable standards, for activities related to the accomplishment of the work performed and provide dust protection as reasonable, to protect government assets.
1.11 The contractor shall be responsible for safeguarding all government property. At the close of each work period, government facilities, property, and materials shall be secured.
1.12 The contractor shall provide protection to government property, to prevent damage during the period of time the property is under the control or in possession of the contractor.
1.13 The contractor shall record and report promptly (within one hour) to the contracting officers or Project Manager, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.
1.14 In the event of an accident/mishap, the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.
1.15 If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.
1.16 The contractor shall notify the Project Manager of any environmental spills, prior to reporting the incident to the Hill AFB Fire Dept. at 911. If the contractor spills or releases any substance as contained in 40 CFR 302 into the environment, the liability for the spill or release of such substances rests solely with the contractor and its agent to remediate the area as required, at no additional cost to the government.
1.17 The Prime and sub-contractors shall warranty all materials and installation for a period of one year upon acceptance of Beneficial Occupancy.
1.18 Contractor shall perform work Monday thru Friday from 0700-1630, unless otherwise scheduled and approved.
1.19 Contractor shall coordinate all work prior to starting the contract and during the performance with the project manager.
1.20 The contractor shall clean-up work in the interior/exterior of building area at end of each day.
1.21 The contractor will coordinate with the contract officer and the project monitor to obtain base access badges for contractor personnel. The contractor is responsible to immediately return all access badges for personnel who no longer work for the contractor. The contractor is responsible to immediately return all access badges after completion of the contract.
1.22 Government Remedies: The contracting officer shall follow the requirements of FAR 52.212-4, contract terms for Commercial Items, for contractor’s failure to correct non-conforming services.
1.23 The government will provide electricity and water in the form that they currently exist in the facility.
2. Statement of Work:
2.1 The contractor shall prepare the surfaces to be carpeted according to the carpet manufacturer’s specifications and accepted industry practice with floor primer and glue as recommended by the carpet manufacturer. The contractor shall provide all materials necessary to professionally complete the job; this includes all protective equipment, and any material needed to repair the surface prior to installing floor covering and related materials.
2.2 The contractor shall remove furniture out of the rooms, and the contractor shall perform approximately 580 sy of furniture lifting/moving and 580 sy sub floor prep (existing floors).
2.3 The contractor shall furnish and install approximately 580 sy of carpet and 7 pails of adhesive. The project manager will provide samples for the user to choose the carpet pattern and color. For this project the contractor will be required to pick from one of the four options listed below:
1) Shaw geometric large scale pattern ID 6GL “Easy on the Eyes EcoWorx” color “00507” with low VOC releasable wet adhesive ID 2A “Shaw 5100”.
2) J&J Flooring Group complimentary random pattern ID 5GCR “Problem Solved modular” color “1210 Duct Tape” with low VOC releasable wet adhesive ID 2A “Nexus Modular Adhesive”.
3) Mohawk geometric large scale pattern ID 6GL “Chilled Check” color “989 Mystic” with low VOC releasable wet adhesive ID 2A “Enpress PSA”.
4) CMA-Milliken geometric large scale pattern ID 6GL “Southern Analog ‘Viewfinder’” color “VF217 Vignette” with low VOC releasable wet adhesive “Milliken Modular Carpet Adhesive - 100V”.
The contractor shall request pricing of the carpet and adhesive (ONLY) from each of the mandatory sources and choose one of the four options to include in their proposal.
2.4 The contractor shall furnish and install approximately 300 lf of 4” vinyl/rubber cove base, 12 lf transition strip, contractor shall furnish and install and contractor shall provide color samples and the user shall decide the color.
2.5 The contractor shall repair cement that is damaged before laying down carpet.
3. Demolition:
3.1 The contractor shall provide all equipment, materials, and labor necessary to remove the existing 580 sy of carpet, 300 lf of 4” vinyl/rubber cove base.
3.2 The contractor shall protect and use proper care not to damage the existing structure, landscaping, surroundings, personnel, and personal items of building occupants. Any items damaged will be the responsibility of the contractor to repair or replace. The contractor shall demo all carpet, bonding, including surface scraping off old glue, in preparation for install.
3.3 The contractor shall provide roll-off containers for construction debris and will not us any government furnished dumpsters/roll-off containers.

Any changes/addendums, modifications, etc…, to the original SOW must be approved by the Contract Officer; no other parties have approval authority.
