STATEMENT OF WORK

FOR

ASBESTOS BULK SAMPLING AND INSPECTION

REPORT

BUILDING A(TBD)

MINIMUM ORDER

Hill AFB 22 January 2015

STATEMENT OF WORK

1.0. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment and labor necessary to ensure that asbestos services performed at Hill AFB, shall be in accordance with applicable federal, state, and local regulations. The contractor shall collect five (5) bulk asbestos samples, the exact building and suspect material to be sampled will be determined by 75 CES/CEOHA prior to issuance of this task order. Collection of bulk samples shall be performed by certified UDAQ asbestos inspector. The contractor shall document all samples taken on a chain of custody and submit them to a NVLAP certified laboratory for asbestos analysis. Once results are received the contractor shall provide a detailed written inspection report, which can be submitted electronically to the 75 CES/CEOHA office.

Per Hill AFB requirements any detectable amount of asbestos, including less than 1%, shall be identified in the inspection report as asbestos containing material. Sampling and inspection report shall meet all requirements of R307-801-10.

2. GENERAL INFORMATION.

2.1. QUALITY CONTROL. Contractor shall develop and maintain a work plan to ensure services are performed in accordance with Federal, State, and local requirements for asbestos inspections. The contractor shall develop and implement procedures to identify and prevent defective services from reoccurring.

2.2. QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan.

2.3. DOCUMENTATION. The contractor shall provide copies of all project documentation to 75 CES/CEOHA to include daily logs, sample results, and waste manifests upon completion of the project.

2.4. SECURITY REQUIREMENTS. Must have security clearance badges to enter onto Hill Air Force Base and a security line badge to enter onto the airfield/runway area.

2.5 SPECIAL QUALIFICATIONS. All employee’s performing asbestos sampling and inspection reports shall be accredited with the Utah Division of Air Quality (DAQ). State cards issued to the employee’s must be on site at all times.

3.0 GOVERNMENT FURNISHED PROPERTY/SERVICES/EQUIPMENT

Water and Electricity: In accordance with FAR 52.236-14, Availability and Use of Utility Services, all reasonable amounts of domestic water and electricity will be made available to the contractor by the Government from existing system outlets and supplies where possible

4. Only the contracting officer can change the scope of this task assignment. Any work performed by the contractor outside of this scope of work, without written authorization from the contracting officer shall be at the contractor’s expense with no recourse against the government or its agents. Requests for changes to this scope of work must be submitted in writing to the contracting officer. No work on the requested change will be allowed until the contractor receives a signed modification to the task assignment.
