About this file
This is an asbestos chain of custody and laboratory analytical report documenting bulk material sampling and testing conducted at Tinker Air Force Base.
The documentation includes chain of custody procedures initiated by QuantEM Laboratories (Lab No. 213245) for samples collected from Building 1055 at Tinker AFB on February 23, 2006, by Mark Patterson. The chain of custody form indicates samples were collected from a 12x12 floor tile in black mastic. The testing methodology employed was EPA 600 (Polarized Light Microscopy). Laboratory results from both QuantEM Laboratories and Tec-An, Inc. (NVLAP Lab Code 200325-0) document analysis of floor tiles, mastics, and leveling compound samples. QuantEM's analysis (dated 02/26/2006) identified asbestos present in samples 005 and 005a (chrysotile at 2%), with asbestos not present in samples 001-004. Tec-An's analytical report (dated 03/02/2006) for the same project (Building 1055, Project 06-0202) identified chrysotile components ranging from 3-4% in floor tile and mastic samples, with no asbestos detected in the leveling compound. Non-asbestos components including cellulose, synthetic materials, and other fibers were documented across all samples. The analysis confirms the presence of friable asbestos materials requiring appropriate handling and remediation protocols at the Air Force facility.
