About this file

This Performance Work Statement (PWS) details medical waste and Personally Identifiable Information (PII) disposal services for the Air Force Drug Testing Laboratory (AFDTL), located at Joint Base San Antonio Lackland, TX. The contract requires a contractor to collect, transport, and securely destroy approximately 1,000,000 urinalysis sample containers annually, which represent about 160,000 pounds of medical waste. Each sample container consists of a 105 mL bottle containing 15-90 mL of urine and labels with social security numbers and names that constitute PII.

Key performance requirements include providing 20 clean 96-gallon containers on-site, removing containers three times per week (Monday/Wednesday/Friday) during 6:00-10:00 AM hours, destroying all PII data within 72 hours of collection, and maintaining strict privacy and security protocols. The contractor must provide monthly status reports, maintain employee privacy training documentation, designate a single point of contact, and comply with all federal, state, and local regulations regarding medical waste and PII disposal. Performance will be measured against specific thresholds, such as no more than two late/missed removals in a six-month period and 100% evidence of proper disposal.

PERFORMANCE WORK STATEMENT (PWS)

AFDTL MEDICAL WASTE AND PII DISPOSAL SERVICES

1. DESCRIPTION OF SERVICES

1.1. Background. The Air Force Drug Testing Laboratory (AFDTL) has a threefold mission to detect and deter drug use by military personnel through drug testing, to report test results and support administrative and judicial proceedings as the result of those test results and to develop new drug detection methodologies as needed. The AFDTL is one (1) of the six (6) Department of Defense (DoD) Laboratories supporting the DoD Drug Demand Reduction Program. Referenced in Department of Defense Instruction (DoDI) 1010.16, Technical Procedures for the Military Personnel Drug Abuse Testing Program, and Department of the Air Force Manual (DAFM) 44-

197. Contracted service is required to properly remove and destroy medical waste accrued to the normal course of drug testing operations.

1.2. Scope. This is a performance based, non-personal services requirement to provide medical waste and Personally Identifiable Information (PII) disposal services. The AFDTL disposes of approximately 1,000,000 urinalysis samples annually. A sample consists of a 105 mL round capped bottle, 15 to 90 mL of urine, and less than 5 grams of labels and seals. The labels contain social security numbers and names, which constitutes PII. This may add up to 160,000 total pounds of medical waste annually. The AFDTL can handle medical waste containers of 96-gallon capacity. These containers can hold a maximum of 500 lbs. of waste, but on average contain only 320 lbs. of waste upon collection. On site disposal and destruction of excess sample collection bottles is not feasible due to manpower requirements and facility limitations. The AFDTL requires services for this medical waste and PII disposal.

2. GENERAL INFORMATION

2.1. Contractor Identification. All contractor personnel (including subcontractors) shall identify themselves as government contractor personnel during all forms of communications such as business meetings, telephone conversations, electronic mail, attendance sheets, coordination documentations, reports, and the signature blocks utilized in all correspondence. If a contract requires government workspace, the contractor personnel shall wear a picture identification badge and identify their workspace area with their name and company affiliation.

2.1.1. The contractor shall provide a list of associated employees to the Contracting Officer’s Representative (COR), no later than three (3) days after contract award, of contract employees having access to, and responsible for collection and disposal of PII. The list of contract employees will be utilized by the government to validate that only appropriate personnel have access to the collection and disposal of PII in accordance with (IAW) Contractor Roster Contract Data Requirements List (CDRL) A001 and corresponding Data Item Description (DID).

2.2. Contractor Training.

2.2.1. IAW Federal Acquisition Regulation (FAR) 24.301, contractors are responsible for ensuring that initial privacy training, and annual privacy training, thereafter, is completed by contractor employees. Privacy training shall address the key elements necessary for ensuring the safeguarding of PII or a system of records.

2.2.2. The training shall be role-based, provide foundational as well as more advanced levels of training, and have measures in place to test the knowledge level of users.

2.2.3. The contractor may provide its own training or use the training of another agency unless the contracting agency specifies that only its agency-provided training is acceptable.

2.2.4. The contractor is required to maintain and, upon request (within one (1) day), to provide documentation of completion of privacy training for all applicable employees.

2.2.5. No contractor employee shall be permitted to have or retain access to a system of records, create, collect, use, process, store, maintain, disseminate, disclose, or dispose, or otherwise handle personally identifiable information, or design, develop, maintain, or operate a system of records, unless the employee has completed privacy training that, at a minimum, addresses the elements in FAR 24.301(b).

2.3. Contractor/Government Communication.

2.3.1. The contractor shall designate a Contractor’s Focal Point (CFP) to be the single point of contact for all contractor and government correspondence. The CFP shall provide clear and consistent written and verbal response to government within 12 hours of government-initiated communication (e.g., return phone calls, emails, or other communication).

2.3.2. The CFP shall meet with the government team at least quarterly. Additional meetings may be requested by the government or the contractor as necessary. The Contracting Officer (CO), CFP, COR, and/or other designated representative will provide monthly performance feedback to the contractor.

2.4. Monthly Status/Financial Report. The contractor shall prepare and submit a monthly status/financial report identifying all tasks performed, status, issues, and anticipated actions consistent with this PWS each month. The contractor shall provide the Monthly Status/Financial Report IAW Status Report CDRL A002 and corresponding DID.
