Instructions
| Instructions: Within this document there are 10 tabs, 9 of which are to be filled out minus the instructions. Within each of the 9 tabs, there are various requirements. As stated in the offeror requirements tab, do not duplicate the same product for multiple requirements. For each technical requirement, please confirm your company's ability to meet 100% of the stated requirement by selecting from the options shown in Column C (Meets or Does Not Meet) and provide a substantiating document reference within column D. Each technical requirement requires a substantiating document reference. If teaming will be used to fulfill requirement, notate that in the "Location of Substantiating Documentation/Discussion" column (Column D) of each tab. The cells in Column C are colored white until a selection is made, after which the cell will turn either green for Meets or red for Does Not Meet. Input the name of your offering in cell C3, and the SKU of your offering in cell C4 on each tab. Respond to each requirement as completely and accurately as possible. If your company cannot meet 100% of the requirement, please identify in the "Location of Substantiating Documentation/Discussion" column (Column D) what specifically your company is unable to meet. If your company cannot meet specific sizes or options, you technically Do Not Meet the requirement and will not be eligible for award. |

Attachment 4 - Requirements Matrix
Vendor Requirements Offeror Requirements Response Details / Comments / Location of Substantiating Documentation
| Blanket Purchase Agreement (BPA) Vendor Reporting Requirements (VR.R), the Vendor shall: | |
| VR.R.1 | Be responsible for delivery and terms & conditions of the BPA. |
| VR.R.2 | Provide an Air Force Office Seating Quarterly Spend Report. The report shall be submitted no later than the 15th day in the month following the end of each fiscal year (FY) quarter except for the 4th quarter which will be due by the 30th (January 15, April 15, July 15, and October 30) of each fiscal year throughout the life of the BPA. |
| BPA Vendor Testing Requirements (VR.T), the Vendor shall: | |
| VR.T.1 | Provide certification indicating all seating has been tested and meets or is certified in accordance with applicable American National Standards Institute (ANSI) / Business and Institutional Furniture Manufacturers Association (BIFMA) X5.1, BIFMA G1, and flammability Technical Bulletin (TB117) latest editions. |
| BPA Vendor Delivery Requirements (VR.D), the Vendor shall: | |
| VR.D.1 | Provide delivery in accordance with the terms of the Vendor’s Multiple Award Schedule (MAS) contract and resulting BPA call, unless the ordering activity designates an accelerated delivery schedule in the BPA call. |
| VR.D.2 | Provide notification to the government representative at least 5 days prior to delivery by the Vendor or member of the delivery crew. |
| VR.D.3 | Be responsible for damages incurred during shipping and installation. |
| VR.D.4 | Not use protective materials that will leave any type of residual print, outline or imaging on seating or seating fabric. |
| VR.D.5 | In cases of extreme necessity, submit a written request to the Contracting Officer (CO) for approval, to schedule delivery at other than the aforementioned times. Until CO approval is received, no delivery actions shall be taken. |
| VR.D.6 | Obtain a signed/approved “Inspection/Acceptance Checklist” from the accepting government representative validating acceptance prior to final departure/delivery of each BPA call. |
| VR.D.7 | Attach “Inspection/Acceptance Checklist” to the invoice that is submitted into Wide Area Work Flow (WAWF). |
| VR.D.8 | Unless otherwise specified by the call placed under this BPA call level, vendor shall deliver item within 45 days of order date. |
| BPA Vendor Assembly, Installation, and Removal Requirements (VR.A) the Vendor shall: | |
| VR.A.1 | Unload truck or delivery vehicles; remove office seating from packaging or boxes; perform all required assembly in accordance with the manufacturer’s instructions; place office seating in building(s) in accordance with the BPA call or as directed by the accepting government representative; and remove and dispose of all refuse, waste, and debris. Assembly, installation and removal shall be determined at the call level. |
| VR.A.2 | Provide the necessary equipment and appropriately experienced and trained personnel to complete seating assembly and delivery. |
| VR.A.3 | Be responsible for all personnel, equipment, tools, materials, supervision, storage, assembly, transportation, incidentals and all other items necessary to provide and deliver office seating at various locations throughout the USAF CONUS locations, including Alaska and Hawaii. |
| VR.A.4 | Ensure that all stickers and labels are removed (not to include warranty labels), and no adhesive residue remains on the surface. |
| VR.A.5 | Ensure that the seating is free of surface dirt and defects, and that the chair assembly is complete and the seating is ready to use. |
| BPA Vendor Product Documentation Requirement (VR.P) the Vendor shall: | |
| VR.P.1 | Properly mark the items sold to the Government with manufacturer’s name, part number, contract number, BPA call number and the month and year of delivery in a hidden, but readily available location or provide serial number that would provide this information. These markings shall remain readable throughout the warranty timeframe. |
| VR.P.2 | Ensure that all owners’ manuals, training material addressing the ergonomic features and adjustments of seating, warranty information, maintenance and care instructions, operating instructions, and any other information/documentation is provided to the accepting government representative upon completion of the installation. |
| VR.P.3 | Provide a PDF catalogue of the products that were selected for the AF |
Office Seating II program no later than 30 days of BPA Award. Include a picture of each chair, per the requirements matrix under each category (EX.UU, EX.LL, T.MU, T.UU, CR.MU, CR.UU, GS.MU, GS.UU), the finish options, upholstery options, patterned fabric options, leather options, and faux-leather/vinyl options. List Item Name and Model Number along with dimensions and brief description of features.
| BPA Vendor Warranty Requirements (VR.W) the Vendor shall: | |
| VR.W.1 | Specify the exact terms and conditions of the manufacturer warranty. |
| VR.W.2 | Be responsible for shipping, repairs and/or replacement of chair(s) if within warranty terms. |
| VR.W.3 | Meet minimum 10-Year warranty on all seating and related components. |
| BPA Vendor Market Changes Requirement (VR.MC) the Vendor shall: | |
| VR.MC.1 | Provide suitable substitutions to the CO, for approval/disapproval within 30 days prior to discontinuing. Suitable substitutions shall be of the same quality an be at the same price or lower than the discontinued chair. |
| VR.MC.2 | Add substitutions to the Pricing Matrix (which becomes Price List upon BPA award) once approved by the CO. |
| VR.MC.3 | Provide a 30 day advance notification to CO before updating the BPA Pricing Matrix (Price List upon BPA award) for any GSA schedule contract pricing changes that are approved by GSA modification and affect the pricing of the BPA GSA schedule. |
| BPA Vendor Environmental and Sustainability Requirements Requirement (VR.E) the Vendor shall provide: | |
| VR.E.1 | Office seating which is energy efficient, water efficient, bio-based environmentally preferable, non-ozone depleting, contain recycled content, or are non-toxic or less-toxic alternatives, consistent with the requirements of Executive Order 13693 and Unified Facilities Criteria (UFC) 1-200-02. |
| VR.E.2 | Certification indicating all seating meets or is certified in Greenguard Indoor Air Quality and meets or is certified ANSI/BIFMA M7.1, latest edition. |
| VR.E.3 | Products that meet the EPA recovered material percentage requirements in Table 1 below. |
| BPA Vendor General Requirements (VR.G) the Vendor shall provide: | |
| VR.G.1 | Work accomplished during a standard, 5-day week, Monday through Friday, between the hours of 8:00 AM and 4:00 PM local time, excluding federal legal holidays and non-work days resulting from holidays occurring on weekends. |
| VR.G.2 | A submitted request to the Inspection/Acceptance Official for delivery during nonstandard hours, as seen in VR.G.1 at least five work days prior to the scheduled delivery, and shall indicate the day/days, time/amount of work, and reason why permission should be granted to schedule. |
| VR.G.3 | A different chair style per category. (i.e. you cannot submit the same chair for two different categories, meaning your Task Chair cannot also be your Conference Room chair.) |
| VR.G.4 | 48-hour customer service response time (Sales, delivery, product questions, warranty, and product servicing) to all CONUS locations and regions, including Alaska and Hawaii during normal business hours (9am to 5pm). |
| VR.G.5 | The ability to support a production capacity, for this program, of 3,000 chairs in any given month. |
| VR.G.6 | Adjustment controls that are clearly labeled, easy to reach and operate from a seated position (if the chair has adjustment controls). |
| BPA Vendor Invoice Procedures Requirement (VR.I) the Vendor shall provide: | |
| VR.I.1 | Invoicing, accomplished via WAWF in accordance with DFARS clause 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports and specified in the BPA calls. |
| BPA Vendor Certification Requirement (VR.C) the Vendor shall provide: | |
| VR.C.1 | Certification in writing that all fabrics meet or exceed minimum requirements established by the Association for Contract Textiles (ACT) and ASTM for flame resistance, abrasion resistance, colorfastness, and physical properties. |
| VR.C.2 | Signed certification from company representative indicating all chairs offered are free from defects that would affect performance, safety, serviceability, and appearance. |
