About this file

This is an Asbestos Inspection Report prepared by Ethos Environmental LLC for Aleut Facilities Support Services, LLC at the United States Air Force Academy. The inspection was conducted on October 3, 2016, by Colorado Department of Public Health and Environment (CDPHE) licensed asbestos inspector Nicholas Daviscourt (License #18179) regarding PO 59195, WO 55331, Building 2410, Room A29. The inspection covered drain line pipe insulation in the ceiling plenum, sprayed-on fireproofing on the beam above the line, and sprayed-on fireproofing overspray on the line itself, all in fair condition at the time of inspection. Three bulk samples were collected and analyzed by EMSL Analytical, Inc., using Polarized Light Microscopy (PLM) and Point Counting Method in accordance with EPA 600/R-93/116 standards. All three homogeneous areas sampled—pipe insulation on drain line (Sample #A01), overspray fireproofing on drain line (Sample #A02), and fireproofing on beam (Sample #A03)—tested negative for asbestos.

The report recommends that sampled materials may be removed as desired since no asbestos was detected. However, if any additional materials are encountered during work activities that differ from those specified in the inspection or appear to deviate from materials already inspected, work should cease and Ethos Environmental should be contacted to return and inspect and sample the suspect materials. The report includes photographs of sampled areas and certification of the inspector's qualifications, valid from April 20, 2016, through April 22, 2017.

ASBESTOS INSPECTION REPORT

Dear Ms. Brainerd, Ethos Environmental performed a limited asbestos inspection of the affected drain line pipe insulation, sprayed-on fireproofing on the beam above the line, and sprayed-on fireproofing overspray on the line itself in Building 2410 in Room A29 for Asbestos Containing Building Materials on 10-3-16.

Colorado Department of Public Health and Environment (CDPHE) licensed asbestos inspector Nicholas Daviscourt (license #18179) conducted this inspection in agreement with Aleut Facilities Support Services, LLC.

The inspection was performed with the purpose of: 1) Inspecting for and identifying any asbestos in the affected drywall, cove base, and vinyl flooring. 2) Determining the condition of the materials, and conducting random bulk-sampling of the suspect materials in compliance with State regulations. 3) Analyzing bulk samples by an NIST accredited NVLAP laboratory using Polarized Light Microscopy (PLM), and Point Counting Method (if results are <1% asbestos), in order to determine the percentage of asbestos composition of the sampled materials.

Ethos Environmental inspected the accessible areas within the site of planned renovations. Sampled materials were touched by the inspector to determine friability, and three samples were taken. In accordance with State regulations, a minimum of one sample per homogeneous area of miscellaneous materials was taken…

Project Scope:

Ethos was called out to the site at Room A29 to inspect and sample the affected drain line pipe insulation in the ceiling plenum above the main entrance, sprayed-on fireproofing on the beam above the line, and sprayed-on fireproofing overspray on the line itself. This area was slated for renovation activities, and thus these materials were sampled to facilitate their removal. Sampled materials were in fair condition at the time of inspection, with some of the

Homogeneous Areas Sampled:

Homogeneous Area #1: 2410, A29, pipe insulation on drain line in plenum.

Sample #A01: No asbestos was found in the sample taken.

Homogeneous Area #2: A29, overspray fireproofing on drain line.

Sample #A02: No asbestos was found in the sample taken.

Homogeneous Area #3: A29, fireproofing on beam (within the scope due to

Sample #A03: No asbestos was found in the sample taken.

Recommendations:

None at this time, as no asbestos was found in the samples taken. The sampled materials can be removed as desired.

If any materials are encountered during work activities that are not specified above, or that appear to deviate from other materials present, work activities should cease and Ethos should be contacted to return to the site to inspect and sample the suspect materials.

Ethos Environmental is proud to serve you, and to make your business flow smoothly. If you have any questions regarding this inspection, or any other way that we can serve you, contact Nick Daviscourt at 303-995-3652. Your trust and confidence is our highest goal.
