The following have been modified:
ADDENDUM TO FAR 52.212-1
FAR 52.212-1 INSTRUCTIONS TO OFFERORS – COMMERICAL ITEMS (APR 2014)
1. NOTICE TO OFFERORS: Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
Addendum to FAR 52.212-1(b), Submission of offers. The following paragraph is added: “(12) Submit signed and dated offer to 10th Contracting Squadron, ATTN: Alex Kephart, Contracting Officer, 8110 Industrial Drive, Suite
200, USAF Academy, CO 80840-2315. Offers must be submitted on the SF 1449 and as specified in the solicitation. Offers must be received by the date and time specified in block 8 on the SF 1449. Facsimile and e-mail proposals will not be accepted, no exception.”
Addendum to FAR 52.212-1(c) Period for acceptance of offers. The paragraph is tailored as follows: “The 30 days referenced in the standard clause is revised as follows: the offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers.”
Addendum to FAR 52.212-1(h) Multiple awards is amended as follows: “For this solicitation, the Government will award a contract on an ‘all or none’ basis, multiple awards will not be contemplated.”
2. FORMAL COMMUNICATIONS. Questions and requests for clarification or information concerning the solicitation must be provided in writing NO LATER THAN 01 June 2016 AT 2:00 PM Mountain Time. Answers will be compiled and posted electronically to the Federal Business Opportunities website: https://www.fbo.gov.

3. Addenda to FAR 52.212-1 PROPOSAL PREPARATION INSTRUCTIONS
General Instructions:
To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein.
Offerors are required to meet all solicitation requirements, including terms and conditions, and representations and certifications, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. The government intends to award a contract without discussion with respective offerors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete rationale. Proposals must be complete, self-sufficient, and respond directly to the requirements of this solicitation; alternate proposals will not be accepted. The response shall consist of four (4) separate parts; Part I -
Price Proposal, Part II - Technical Proposal - Likely Awardable Task Order(s), Part III - Past Performance
Information, and Part IV - Contract Documentation.
The contracting officer has determined there is a high probability of adequate price competition for this acquisition.
Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the https://www.fbo.gov/ mailto:alex.kephart@us.af.mil mailto:matthew.mclean.10@us.af.mil contracting officer determines that adequate price competition no longer exists offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
Specific Instructions:
a) PART I – PRICE PROPOSAL – No page limit. Submit one (1) hard copy and one (1) compact disk
(CD) with electronic data files, in accordance with paragraph (e) below:
i) Complete blocks 12, 17a, 30(a) (b) and (c) of the SF 1449. In doing so, the offeror agrees to the contract terms and conditions as written. These sections constitute the model contract.
ii) Insert proposed unit and extended prices in Attachment 2 - A&AS Contractor Proposed Labor
Rate Pricing Schedule. Attachment 2 shall be returned in its entirety. All dollar amounts SHALL
BE ROUNDED TO THE NEAREST CENT. The proposal(s) must contain hourly labor category pricing for each year of the three-year ordering period.
b) PART II – TECHNICAL PROPOSAL – Limited to no more than 20 pages. Submit two (2) hard copies and one (1) compact disk (CD) with the electronic data files in accordance with paragraph (e) below:
i) Offers shall provide the following:
(1) Factor 1: A formal response to each likely awardable task order/performance work statement
(Attachment 5 and Attachment 6). In each formal response, the offeror is required to identify labor category(ies), labor hours by category(ies), and name-redacted resume(s) to fulfill the requirements of this task order. Additionally, a brief narrative explaining the rationale for the labor category proposed is required. Pricing is required, and should be IAW the offeror’s proposed labor rate pricing schedule (Attachment 2) submitted within Part I – Price Proposal.
c) PART III – PAST PERFORMANCE INFORMATION – PART III – PAST PERFORMANCE
INFORMATION – Limited to no more than 24 pages. Only references for recent and relevant past efforts/contracts are desired. Submit four (4) hard copies and one (1) compact disk (CD) with the electronic data files in accordance with paragraph (e) below:
i) Recent/Relevant Contract References - limited to eight (8) citations total (including all prime, subcontractor and/or joint venture citations). Provide details on eight (8) of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years from the issue date of this solicitation. Recent past performance is defined as past performance on contracts within the last three (3) years from the issue date of this solicitation. Past performance for contracts exceeding the timeframe for this definition of recency will be excluded from evaluation. Relevant contracts include performance of Government Advisory and Assistance
Services and/or private sector consulting services equal to or greater in scope, magnitude, and complexity of the effort described in this solicitation. The government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this solicitation. The government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance. Furnish the following information for each contract listed:
(1) Company/Division name
(2) Contracting Agency/Customer
(3) Contract Number
(4) Contract Dollar Value
(5) Period of Performance
(6) Verified, up-to-date name, email address, address, FAX & telephone number of the contracting officer or point of contact
(7) A detailed description of the services rendered and its relevancy to the USAFA A&AS requirement
(8) Comments regarding compliance with contract terms and conditions
(9) Examples of performance (letters, metrics, customer surveys, independent surveys, etc.)
(10) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions. Describe mitigating circumstances or actions taken to correct the unacceptable condition and preclude recurrence.
* Note: Do not leave any of the above information blank. This information may also be used in determining responsibility.
If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past recent and relevant contracts (as noted above, a maximum of eight citations total is allowed).
ii) Subcontractor/Joint Venture Teaming Partner Consent, no page limit. Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors (on the subcontractor’s letterhead) that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
iii) In addition to the above information, the offeror shall mail Attachment 7 - Past Performance
Questionnaire (PPQ), to the list of submitted contract references. Instruct these references to complete the Past Performance Questionnaire and return directly to:

(1) The questionnaire need not be sent to customers where past performance records exist in the
Government’s Past Performance Information Retrieval System (PPIRS).
(2) Any past performance questionnaires sent directly from a proposing offeror will not be accepted or evaluated. For the purpose of expediency of evaluation, please instruct references to respond to the past performance questionnaire as soon as possible. It is the sole responsibility of the offerors to ensure questionnaires are submitted by the solicitation closing date. The offerors are encouraged to follow-up with respondents for completed questionnaires, but are not required to do so.
d) PART IV – CONTRACT DOCUMENTATION – No page limitations. Submit one (1) hard copy and one (1) compact disk (CD) with the electronic data files in accordance with paragraph (e) below:
i) Complete the necessary Representations and Certifications in SAM.gov and supplemented in full text within this solicitation.
ii) Provide evidence as required by any amendment that the offeror’s proposal incorporates all amendments to the solicitation (if applicable)
iii) A letter of authorization for the Contracting Officer to verify offeror’s financial responsibility.
Include financial institution’s name, address, and primary point of contact and phone numbers for financial institutions and complete Attachment 8 - Offeror’s Financial Reference. The letter of authorization shall be separate from the Offeror’s Financial Reference.

iv) Joint Venture Agreements - Joint Ventures are allowable on competitive 8(a) set-asides; however, the joint venture agreement must be received by SBA prior to proposal due date and approved before award of any resulting contract, in accordance with the restrictions outlined in 13 CFR 124.513. If you are contemplating a joint venture on this project you must advise, in writing, your assigned SBA
Business Opportunity Specialist (BOS) and the USAFA Contracting Officer as soon as possible. It is also recommended that the agreement be submitted as soon as practicable to ensure compliance with established regulations. Any corrections and/or changes needed can be made only when your BOS has adequate time for a thorough review before the proposal due date.
v) Complete the following provision as required in the Solicitation: DFARS 252.209-7991. You may complete this provision in SAM or hard copy. If the certifications in SAM are completed, state accordingly in the proposal package.
e) Format for all Parts shall be as follows:
1. The proposal will be submitted on 8 1/2” x 11” paper except for fold-outs used for charts, tables, or diagrams, which may not exceed 11” x 17”. A page is defined as one face of an 8 ½” x 11” sheet of paper (white or ivory in color) containing information. Charts, tables, or diagrams, may not exceed
11” x 17” and each face will be counted as one page. Includes all appendices, charts, graphs, diagrams, tables, photographs, etc. Covers for volumes, table of contents, indices, title pages, cross reference indices, and section dividers/tables will not be included in the page count if they are inserted solely to provide ease to the reader in locating volumes/sections of the proposal. They will be counted if they contain any other information, (e.g. diagrams, extraneous data, etc.). Pages marked “This page intentionally left blank” will not be counted.
2. Typing shall not be less than 12 font, with 1” margins on each side of the page. Smaller, legible font size may be used for charts and tables.
3. Elaborate formats, bindings or color presentations are not desired or required. Company Name and solicitation number shall be included in the header of each page. Submit all Parts of the proposal using a three ring binder. All Parts shall be submitted in separate binders in order to clearly determine page counts.
4. Electronic copies of all data submitted with the proposal shall be submitted on one (1) compact disk. Compact disks will be CD ROM in MS Word 2010, MS Excel 2010, any Microsoft Office
2010 product unless otherwise specified, or Adobe Acrobat. The paper or hard copy of the offeror’s proposal shall control any conflict between the electronic version and the paper copy submitted in response to this solicitation.
5. Documents submitted in response to this solicitation must be fully responsive to and consistent with the following:
(1) Requirements of the solicitation, drawings, specifications, and government standards and regulations pertaining to the specifications.
(2) Evaluation Factors for Award in the supplemented clause FAR 52.212-2 Evaluation –
Commercial Items, included of this solicitation.
(3) Limitation on the number of proposal pages: Pages exceeding the page limitations set forth in this section will not be read or evaluated, and will be removed from the proposal.
6. It is incumbent upon the interested parties to review FBO.gov frequently for any updates or amendments. It should be noted that only mailed or hand-delivered copies of offerors’ proposals will be accepted and shall be sent to the following address:
