ADDENDUM TO FAR 52.212-1, INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS (JAN 2005)

Instructions to Offerors: Lowest Price Technically Acceptable (LPTA) Procedures

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two separate parts: Part I - Price Proposal and Part II – Technical Proposal/Minimum Qualifications B. The Contracting Officer (CO) and the Contract Specialist (CS) are the sole points of contract for this acquisition. Address any questions or concerns to the CO/CS. Written requests for clarification may be sent to the CO/CS via email. Email is the preferred method of communication. The CO and CS for this acquisition are as follows:

Email address: verna.williams@us.af.mil Upon examination of the initial offers, the CO and/or the CS will review this determination and if, in the CS’s opinion, adequate price competition exists, no additional cost information will be requested and certification under FAR15.406-2 will not be required. However, if at any time during this competition the CS determines that adequate price competition no longer exists, offerors may be required to submit information to the extent necessary for the CS to determine the reasonableness and affordability of the price.

C. Specific Instructions:

1. PART I – PRICE PROPOSAL - Submit original and one (1) sanitizied copy

(a) Complete blocks 12, 17a, 30a, 30b and 30c of the RFP (SF 1449). In doing so, the offeror accedes to the contract terms and conditions as written in the RFP.

(b) Insert the proposed unit and extended prices in Section B for each Contract Line Item Number (CLIN), including all option periods. The extended amount must equal the whole dollar unit price multiplied by the number of units.

(c) A Total Evaluated Price (TEP) spreadsheet will be used for evaluation purposes only and will be calculated as follows: Insert the Unit Price based on the Number of Attendees Per Event (where applicable). The extended prices (unit price X quantity) for all of the firm-fixed price CLINs and the corresponding option CLINs will be totaled. Upon award, if the travel CLIN(s) location changes, travel will be negotiated prior to Task Orders being processed.

2. PART II – TECHNICAL PROPOSAL – 20 page total limit

(a) Technical Approach/Minimum Qualifications shall consist of the contractor’s certification to the following:

(i). Sub-Factor 1: Credit Reference Letter from Banking institution outlining credit history.

(ii). Sub-Factor 2: Technical Approach

Submit resumes showing individual Program Manager experience, skills and training for personnel that would be directly working on this project in accordance with the AFRC Event Planner Services (See attached PWS).

Submit knowledge and abilities of event planning, business understanding, technology planning, and Yellow Ribbon Program event planning (if any).

(iii). Sub-Factor 3: Management Approach

Certification of CMP (Commercial Meeting Planner) or GMP (Government Meeting Planner), child care subcontractor certification and abilities, A/V experience or plan for back up, reach back capability

Submit a plan for facilitating Government-Contractor communications.

Submit a Staffing Plan showing the proper training and experience of proposed personnel (Child Care Provider, A/V provider/plan, event management plan) in accordance with the PWS.

(iv). Sub-Factor 4: Past Performance

Submit past performance Information on four (4) recent contracts that you consider most relevant in demonstrating your ability to perform the proposed effort. Recent is ongoing contracts or those completed within three years of the date of issuance of the solicitation. Also, include information on three (3) recent contracts performed by each of your teaming partners and significant subcontractors that you consider most relevant in demonstrating their ability to perform the proposed effort. Include rationale supporting your assertion of relevance.

Submit a “roadmap,” if your company has been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.

D. Documents submitted in response to the RFP must be fully responsive to and consistent with the following:

1. Requirements of the RFP (CLINs & PWS) and government standards and regulations pertaining to the PWS.

2. Evaluation Factors for Award are identified in FAR 52.212-2 of this RFP.

3. Submit one (1) original copy (with prices) and one (1) sanitizied copy (without prices) of the complete proposal. Proposals must be sent to: HQ AFRC A7KA, Attention Verna Williams, 255 Richard Ray Blvd, Robins AFB, GA., 31098-1635. The envelope/package used in submitting replies must be plainly marked with the solicitation number, and the date and local time set forth for receipt of proposals in the solicitation document. Responses are due no later than the exact time and date set for receipt of proposals in block 8 of the solicitation
