PAST PERFORMANCE QUESTIONNAIRE

Provide a list of the least three (3) and no more than five (5) contracts in the last 3 years, in accordance with 52.212-1, Instructions to Offerors and 52.212-2 Evaluation—Commercial Items. Contracts may include those entered into by the Italian or U.S. Government, agencies of local Governments and commercial customers. List only those of similar magnitude and scope. Include the following information for each contract:

If you are contemplating a Joint Venture, list past performance references for each company involved.

Also, indicate if you performed as the Prime Contractor, Sub-Contractor or Key personnel.

1. In this section of the questionnaire, you are asked to rate the contractor. Please indicate the rating that best applies. If you wish to elaborate on any of your answers, please provide comments at the end of this section. If more space is needed, continue your comments on a separate sheet of paper and attach it to this questionnaire prior to submitting it to 31 CONS/LGCB. 2. You are urged to supplement your own knowledge of the contractor's performance with the judgment of others in your organization. Any marginal ratings should be supplemented with an explanatory narrative in the remarks block of this survey. If you circle a Yes/No type question No, please provide a corresponding explanation in the remarks block.

3. ONCE COMPLETED, THIS SURVEY WILL BE CONSIDERED SOURCE SELECTION SENSITIVE IN ACCORDANCE WITH FAR PART 2.101 and 3.104.

Definitions Matrix

(E) Exceptional Performance meets contractual requirements with many exceeded to the Government's benefit. During performance evaluation, no more than a few minor problems were encountered for which corrective actions, taken by the contractor, were highly effective.

(G) Good Performance meets contractual requirements with some exceeded to the Government's benefit. During performance evaluation, no more than some minor problems for which corrective actions, taken by the contractor, were effective.

(S) Satisfactory Performance meets contractual requirements with some exceeded to the Government's benefit. During performance evaluation, no more than some minor problems for which corrective actions, taken by the contractor, were satisfactory.

(M) Marginal Performance does not meet some contractual requirements. During performance evaluation, a serious problem was identified, for which the contractor has not yet provided corrective action or the contractor's proposed actions appear only marginally effective, or were not fully implemented.

(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. During performance evaluation, a serious problem was identified for which the contractor's corrective actions appeared to be, or were, ineffective.

(N) Neutral Performance was not observed or not applicable to the current effort being reported against.
