About this file

This document is a Sample Subcontractor/Teaming Partner Consent Letter for the Release of Past Performance Information for RFP FA5641-26-R-0001, a solicitation for the Germany Air Mail Terminal IV (GAMT IV) contract issued by the United States Air Force Europe (USAFE). The consent letter template allows subcontractors and teaming partners to provide permission for the government to discuss their past and present performance information with the prime contractor during the source selection process, recognizing that the government is placing increased emphasis on past performance to obtain best value in source selections.

The document includes two identical sample consent letter formats addressed to Ms. Noemi Toth-Phillips, Contracting Officer for the 764 Enterprise Sourcing Squadron/PKA, which potential subcontractors can use to grant consent for performance information sharing. The letters are specifically related to the USAFE mail handling and distribution contract for the Germany Air Mail Terminal IV, and require signature and title from an authorized company representative who can legally bind the company.

SAMPLE SUBCONTRACTOR/TEAMING PARTNER CONSENT LETTER

FOR THE RELEASE OF PAST PERFORMANCE INFORMATION

TO THE PRIME CONTRACTOR

Past Performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime contractor during exchanges. In an effort to assist the Government’s Past Performance evaluation team in assessing your recent past performance relevancy and confidence, we request that the following consent letter be completed by the major subcontractor/teaming partners identified in your proposal. The completed consent letters should be as part of your Past Performance Volume.
