About this file
This document is a Past Performance Questionnaire for RFP FA5641-26-R-0001, a solicitation for the Germany Air Mail Terminal (GAMT) IV requirement by the 764th Enterprise Sourcing Squadron. The questionnaire is designed to collect detailed performance information about a contractor's previous mail handling and distribution services for the U.S. Government. The form requires a comprehensive evaluation of the contractor across 24 performance metrics, including their ability to process military mail, handle approximately 8M kilograms of mail annually, comply with European Union aviation security regulations, and demonstrate key operational capabilities such as personnel management, quality control, problem-solving, and adherence to project schedules.
The questionnaire instructs the respondent to electronically complete the form and send it directly to the Procuring Contracting Officer, Ms. Noemi Toth-Phillips, with a courtesy copy to Contract Specialist Ms. Barbara Wafzig. The document emphasizes the source selection sensitivity of the information and instructs recipients not to return the completed questionnaire to the offeror. Key areas of evaluation include project management, personnel training, customs compliance, mail processing volume, responsiveness to contract changes, and overall contractor performance, with space provided for detailed remarks on each assessment criterion.
