About this file
This is a Questions and Answers document for the UK Mortuary Blank Purchase Agreement (BPA) FA558726Q0014. The document does not address specific required products or services in detail, but rather provides clarification on key requirements and operational parameters for mortuary services at RAF Mildenhall and Lakenheath bases in the United Kingdom.
The anticipated annual volume for services is estimated at 1-5 cases requiring varying levels of service, including transportation from location of death or hospital to a funeral home for holding and subsequent drop-off to base or commercial flights. Minimum preparations are defined as properly placing human remains into a human remains pouch or outer container for international transportation, with holding refrigeration and maintenance at the contractor's facilities; embalming or additional preparation is not a requirement and will be conducted elsewhere. All procedures, pick-ups, drop-offs, and custody transfers must be documented in accordance with UK law. Collection scenarios include hospitals on base and county coroner facilities, though all types of locations (private residences, institutions, public locations, road traffic collisions) may be applicable. Members are typically transferred to the United States or Germany, requiring death certificates or equivalent documentation from coroners or medical authorities that must be provided to RAF Mildenhall and Lakenheath mortuary officers and technicians before transport off-island.

Questions and Answer
UK Mortuary BPA – FA558726Q0014
Q: Could you please provide the estimated annual volumes for this service?
A: Varies: I would say about 1-5 requiring different levels of service of transportation to and from location of death/hospital to funeral home for holding and drop off to base or commercial flights.
Q: How would you define the term “minimum preparations” within the context of this requirement?
A: Minimum preparations are placing members properly into a human remains pouch or outer container/vessel for international transportation. Embalming or preparation is not a requirement. Holding refrigeration and maintaining human remains within their facilities.
Q: Could you clarify what is meant by “all processed paperwork in accordance with UK law”?
A: All procedures, pick-ups, drop-offs and different custody transfers documented. If picking up from coroner or obtaining custody from other agency/location does that require a transfer of custody? Is that documented?
Q: Is embalming prior to transport required, or will this be undertaken at an RAF facility?
A: Embalming is not required and will be conducted elsewhere.
Q: Could you elaborate on the documentation required to transport remains off-island to the next destination or final Point of Embarkation (POE)?
A: Members will most likely/often need to be transferred to United States or Germany.
Meaning member needs a death certificate or equivalent indicating the members passing.
Likely conducted by a coroner or medical authority. If that documentation is provided to the funeral home, would need to be provided to RAF Mildenhall and Lakenheath Mortuary officers and technicians.
Q: To support accurate pricing, could you clarify the typical collection scenarios (e.g., private residences, hospitals, institutions, public locations, road traffic collisions, or other sudden or hazardous circumstances)?
A: All of the above. However, mostly hospitals on base and from county coroner.
