ADDENDA TO 52.212-1 PROPOSAL PREPARATION INSTRUCTIONS

A. To ensure timely evaluation of the quotation, the offeror must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factor or sub factors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two separate parts; Part I – Price and Part II –Technical.

B. The Contracting Officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness.

C. Specific Instructions:

1. PART I – PRICE PROPOSAL

(a) Complete blocks 12, 17a, and 30a, b and c of the SF 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The SOLICITATION constitutes the model contract.

(b) Insert proposed unit and extended prices in the Pricing Schedule pages 3, 4 and 5 of the SF 1449. The proposal(s) must be submitted for all CLINs.

(c) Complete the necessary fill-ins and certifications in provisions. 52.212-3 has been incorporated by reference. If your certifications are different than your SAM registration or you are not currently registered in SAM please go to http://farsite.hill.af.mil/ and print out the provision and return with your proposal or request a copy from our office.

This in no way will be grounds for a proposal extension.

PART II – TECHNICAL PROPOSAL

This Technical Proposal shall be limited to no more than 10 pages. In the event that the Technical Proposal received in response to this solicitation exceeds 10 pages, only the first 10 pages will be evaluated for technical acceptability.

(a) The offeror must supply an original or a copy Bachelor’s degree(s) earned from a nationally or regionally accredited collegiate institution with the employee’s full legal name (First, Middle Initial, Last is acceptable).

(b) The offeror must submit in writing past experience(s) of working directly with youth, to include a point of contact (POC), duration of experience, employment title, and a brief synopsis of performance tasks. Past Experience shall be recent and relevant to the services within the PWS. To be considered recent, services must have been performed within the last seven (7) years from the issuance date of solicitation. To be considered relevant, past experience has to be in working with youth, e.g. after school youth programs, church youth ministries, or youth organizations for a minimum of six (6) months.

(c) Documents submitted in response to this RFP must be fully responsive to and consistent with the following:

1. Requirements of the RFP (CLINs & PWS) and Government standards and regulations pertaining to the PWS.

2. Evaluation Factors for Award in provision 52.212-2, Evaluation—Commercial Items (Addendum).

3. Any limitation on the number of pages. Pages exceeding the page limitations set forth in this solicitation provision will not be read or evaluated, and will be removed from the proposal.

(a) Format for proposal Parts II and III shall be as follows:

(i) The proposals will be 8 ½ inch x 11inch paper, or European equivalent, except for fold-outs used for charts, tables, or diagrams, which may not exceed 11inch x 17inch or European equivalent.

(ii) A page is defined as one face of a sheet of paper containing information.

(iii) Typing shall not be less than 10 pitch.

(iv) Elaborate formats, bindings or color presentations are not desired or required.

(v) The technical proposal is limited to no more than 50 pages.

(vi) Past Performance proposal is limited to no more than 10 pages.

4. Proposals are to be valid through to 1 September 2017.

(e). REQUESTS FOR INFORMATION All requests for information (RFI) shall be submitted to the 501st Regional Contracting Office no later than 1500hrs, 14 July 2017. RFIs shall be submitted to sarah.williams.17.gb@us.af.mil and 501csw.rco@us.af.mil

(f). GENERAL INFORMATION INFORMATION REGARDING SUBMISSION OF PROPOSAL: Mailed and hand carried proposals must be delivered to 501 CSW/RCO, Bldg 516, RAF Alconbury, Huntingdon, Cambridgeshire PE28 4DA, UK. The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the Solicitation Number, and the name and address of the offeror.

E-mailed proposals should be sent to sarah.williams.17.gb@us.af.mil and 501csw.rco@us.af.mil.

If submitting proposal by e-mail, only submit one copy. Maximum size of email shall not exceed 4MB. Proposal can be split over multiple e-mails. If sending multiple e-mails, ensure subject line contains how many emails have been sent. Facsimile proposals will not be accepted.

Offerors are cautioned that RAF Alconbury has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at the 501st Regional Contracting Office PRIOR to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.212-1(f) ―Late submission, modifications, revisions, and withdrawals of offers.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(i) Technical capability

(ii) Price

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

ADDENDA TO 52.212-2 EVALUATION--COMMERCIAL ITEMS

BASIS FOR CONTRACT AWARD

In accordance with FAR 13.106-2(b)(1), the Government evaluation will be conducted on the offeror’s proposals considering price and technical capability. In using the Lowest Price Technically Acceptable (LPTA) decision approach, the Government intends to award to the responsive and responsible offeror whose technically acceptable proposal has the lowest evaluated price.

A. EVALUATION PROCESS: The Government’s evaluation of offeror proposals will be conducted as follows:

(i) Technical Capability- All proposals will be deemed technically acceptable if they meet requirements in 52.212-1 Part II (Technical Proposal).

(ii) Price - The Government will calculate the total evaluated price by adding the total price of the base year (CLIN 0001) plus the total price of the all option years (CLINs 1001-4001) plus the price for the six-month extension authorized by 52.217-8, Option to Extend Services. The price for the 52.217-8 six-month extension will be calculated using 50% of the total price for the last twelve-month period (CLIN 4001). Evaluation of options shall not obligate the Government to exercise the option(s). The proposal with the lowest total evaluated price will be chosen for award.

52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor no later than 30 days before contract expiration.

52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor no later than 30 days before the contract expires, provided that the Government gives the Contractor a preliminary written notice of its intent to extend no later than 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

OUTSIDE THE UNITED STATES (JUN 2015)

(a) Definition. United States, as used in this clause, means, the 50 States, the District of Columbia, and outlying areas.

(b) Except as provided in paragraph (c) of this clause, the Contractor and its subcontractors, if performing or traveling outside the United States under this contract, shall--

(1) Affiliate with the Overseas Security Advisory Council, if the Contractor or subcontractor is a U.S. entity;

(2) Ensure that Contractor and subcontractor personnel who are U.S. nationals and are in-country on a non-transitory basis, register with the U.S. Embassy, and that Contractor and subcontractor personnel who are third country nationals comply with any security related requirements of the Embassy of their nationality;

(3) Provide, to Contractor and subcontractor personnel, antiterrorism/force protection awareness information commensurate with that which the Department of Defense (DoD) provides to its military and civilian personnel and their families, to the extent such information can be made available prior to travel outside the United States; and

(a) Definitions. As used in this clause--

Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.

Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall--

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. (d) The ombudsman has no authority to render a decision that binds the agency.

(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer.

5352.223-9000 ELIMINATION OF USE OF CLASS I OZONE DEPLETING SUBSTANCES (ODS) (NOV 2012)

(a) Contractors shall not:

(1) Provide any service or product with any specification, standard, drawing, or other document that requires the use of a Class I ODS in the test, operation, or maintenance of any system, subsystem, item, component, or process; or

(2) Provide any specification, standard, drawing, or other document that establishes a test, operation, or maintenance requirement that can only be met by use of a Class I ODS as part of this contract/order.

[Note: This prohibition does not apply to manufacturing.]

(b) For the purposes of Air Force policy, the following products that are pure (i.e., they meet the relevant product specification identified in AFI 32-7086) are Class I ODSs:

(1) Halons: 1011, 1202, 1211, 1301, and 2402;

(2) Chlorofluorocarbons (CFCs): CFC-11, CFC-12, CFC-13, CFC-111, CFC-112, CFC-113, CFC-114, CFC-115, CFC-211, CFC-212, CFC-213, CFC-214, CFC-215, CFC-216, and CFC-217, and the blends R-500, R-501, R-502, and R-503; and

(3) Carbon Tetrachloride, Methyl Chloroform, and Methyl Bromide.

5352.223-9001 Health and Safety on Government Installations (NOV 2012)

(a) In performing work under this contract on a Government installation, the contractor shall:

(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and

(2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.

(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

5352.242-9000 Contractor Access to Air Force Installations (NOV 2012)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base.
