STATEMENT OF WORK
Project No: LXEZ 18-4647 Project Title: Radon Mitigation System, MFH (144UN) Location: Kadena Air Base, Okinawa, Japan
1.0 GENERAL SCOPE:
1.1 The Contractor shall furnish all tools, equipment, materials, testing, labor and personnel necessary to plan, design, manage, supervise and construct the project to provide reliable and functional radon mitigation systems for 106 housing units. The Contractor shall also assess and repair, upgrade, or replace existing radon mitigation systems in 38 housing units in order to bring them into compliance with exposure limits. Work includes but is not limited to pre-installation inspection, gathering of relevant site data, and review of existing drawings and radon sampling data. Determine size, location, and configuration of radon mitigation systems to continuously maintain radon levels under 4 picocuries per liter (pCi/L) for each housing unit.
1.2 Installation: Install new radon mitigation systems in listed homes where radon readings exceeded 4.0 pCi/L. Where possible, mitigation systems shall be completed using active soil depressurization (ASD) techniques and employ sub-slab depressurization (SSD) methods. All installed exterior hardware and components shall be corrosion resistant suitable for coastal marine environments. Provide electrical wire from new suction fan to existing circuit breaker panelboard. New interior conduit shall run above ceiling as much as possible.
1.3 Repair/Upgrade: Assess design and installations of existing radon mitigation units identified as failing to meet radon exposure limitations. Propose upgrade or new design which will successfully reduce radon readings to less than 4.0 pCi/L.
1.4 Preparation of Final Report: Upon completion of the installation work, the Contractor shall provide a final report summarizing all field activities including conclusions and recommendations. In addition, the Contractor shall provide (either as an appendix to the final report or a separate report) operations, maintenance and warranty information on installed systems. Include detailed photos of pre-existing conditions, work performed, and final conditions. Photos shall include unit numbers and written descriptions as necessary.
1.5 Post Testing: Following the installation of radon mitigation systems, perform radon tests in each unit in accordance with EPA Guidance (EPA 402-R-93-003). The radon test shall be a continuous monitoring system which will measure the radon levels over a minimum period of 91 days. If the results of the test exceed 4.0 pCi/L, the Contractor shall take immediate corrective action in order to reduce radon levels within the required threshold.

1.6 This project covers the following Units and floorplan types.

1.7 The project will require sampling and testing of asbestos containing material and lead based paint. The most recent Japan Environmental Governing Standards (JEGS) will be followed during the sampling process and testing.
1.8 The Contractor shall determine the best technical design solution in conformance with current codes and regulations, not specifically stated elsewhere in this project statement of work to provide a complete and useable facility upon completion. The scope of work is further defined in the Description of Work section.
2.0 PROJECT SCHEDULE AND TIME CONSTRAINTS:
2.1 Completion of Work: The period of performance (POP) for this contract where the Contractor is required to complete the entire work is 340 CALENDAR DAYS after the date of Notice to Proceed (NTP). Contractor shall provide overall performance schedule within 7 CALENDAR DAYS after NTP and keep the Government informed for any updates on the schedule.
2.2 Design Schedule: The POP includes 60 CALENDAR DAYS of design which must be accomplished prior to start of construction. After receiving the notice of award, a pre-design conference with the Contractor, CE, Contracting and all other required agencies will be held within twenty (20) days. This project shall, thereafter, be completed in accordance with the following milestones; 95%, 95% Revised. The Contractor shall organize review comments and facilitate Design Review meetings with the Government to discuss the comments and Contractor’s response, and prepare meeting minutes within three business days for each meeting. The written report shall include the project name, contract number, subject, participants, outlines of discussions, recommendations, and conclusions. Number each written record for the particular project under design in consecutive order.
SCHEDULE SHALL BE COMPLETED WITHIN

2.3 Construction Schedule: The POP includes 280 CALENDAR DAYS of construction.
Contractor shall provide construction schedule 7 CALENDAR DAYS after NTP for construction is issued and keep the Government informed for any updates on the schedule.

Contractor shall facilitate Quality Control meetings with the Government to present work progress, summaries, issues, and impacts to the construction schedule during the construction phase from the start of the construction NTP. The Quality Control/construction update meetings shall occur once a month. If deemed necessary, the Government may request bi-weekly meetings if the project scope, cost, and schedule are severely impacted.
Separate meetings to discuss specific work components and issues, as well as the typical Three Phase Control inspections, shall occur as needed and apart from the regularly scheduled construction update meetings, unless the Government directs the contractor to combine multiple requested meetings into one.
2.4 For Design-Build projects, the Contractor may start work such as demolition, existing survey conditions, and any other proposed work with approval from the Government upon satisfactory responses to review comments with the 95% Revised design submission.
2.5 Start Work Notification: The Contractor shall notify the Government thirty (30) calendar days prior to on-site work starting. Before starting on-site work, The Contractor shall have a fully coordinated and approved AF Form 103 prior to submission of the Start work notification (Note: AF Form 103 cannot be completed without an approved EPP). All required documents, including the Work Clearance Request (AF Form 103), Environmental Protection Plan (EPP), Construction Quality Control plan (CQC), and safety plan must be developed and approved prior to start of work. The Contractor shall also provide written notice of construction to housing residents no later than two weeks prior to on-site work.
2.6 Construction Access: Assume all housing units are OCCUPIED. Actual occupancy status will be provided by the Government two weeks prior to start of work. Project includes GOQ 4445 which will require coordination for access through GOQ Manager. Provide written notice to the Government 30 days prior to work in GOQ 4445 to allow adequate time for notification and coordination.
3.0 DESIGN AND CONSTRUCTION REQUIREMENTS:
3.1 The design for this project shall provide for the completion of the work identified on this Statement of Work within the contract price and schedule. The design shall fully accommodate usage requirements of the Government facility and minimize interference with existing work. All design work shall be in accordance with the most current applicable criteria. The specifications shall be prepared using the most current Unified Facilities Guide Specifications (UFGS). Any review by the Government of technical items shall not be construed to relieve the Contractor of responsibility for technically correct and complete documents in compliance with applicable codes, industry standards, and the intent of this SOW. The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s design.
3.2 The project design shall conform to the following criteria as applicable:

3.3 Detailed design analysis/calculations shall be included with all design submittals.
3.4 The Government bears no responsibility for the accuracy of any provided as-built drawings. Actual field investigation and survey of the project area must be conducted by the Contractor to ascertain existing conditions to ensure accurate design, and must be performed in the presence of the Contracting Officer. The Contractor’s activities on the installation shall only be directly related to the directed design and construction herein, and photograph/media recordings may only be utilized for the project area and systems to be renovated.
3.5 Architectural and Mechanical will be stamped, certified, dated and signed by appropriately licensed Architects and Engineers. Proper credentials shall be included prior, for approval. Construction documents shall be stamped, signed and dated (in ink) prior to construction and upon completion of the project. All documents and correspondence will be provided in the English language.
3.5.1 Certification: The work must be accomplished by a contractor with advanced experience in mitigating high levels of radon in Single and Multifamily homes with slab on grade and crawl space construction. The work is to be performed under the technical direction of an individual certified under the NEHA NRPP (National Environmental Health Association National Radon Proficiency Program) and the American Association of Radon Scientists and Technologists. Contractor shall provide proof of certification at the time of bidding.
3.7 Project documents and drawings to be submitted for each design stage are as follows:

Notes: Submittal shall be U.S. Windows compatible CD-ROM with drawings in US version of AutoCAD and PDF, the complete specifications in PDF, and AF Form 66 in Microsoft Excel. Contractor shall submit EPP for Government review at 95%.
Government will not accept final design with unapproved EPP.
3.8 Construction Project Signs: Typical construction project signs with project name shall not be posted at the sites.
3.9 Storage of Material, Latrines and Field Office: Storage of materials, latrines and field office or assembly area on base are subject to approval by the Government. Contractor should submit proposal 45 CALENDAR DAYS prior to start of construction. Equipment shall be stored in a designated staging area while work is not being performed. Latrines are not available in the work area. The Contractor shall be responsible for providing latrines for use for the duration of this project. Contractor furnished latrines shall be removed from work area prior to final acceptance of the project.
3.10 Project Area Maintenance: The Contractor shall adequately maintain the project area, including buildings and associated systems and their paths under contract, according to Air Force Military Family Housing Brochure standards from the construction NTP through acceptance of work and demobilization. Work areas shall be kept in a neat and orderly manner free from accumulation of waste materials or rubbish. Existing landscaping and Government real property shall be protected with clearly marked signs, flags, and/or fencing.
The Contracting Officer shall be the measurer and enforcer of every and all project area maintenance inquiry and deficiency. For this project, Contractor is only responsible within the project site as defined through by the contractor’s installed barricades. Project area must be in order and safe at all times, with adequate barricades and locks surrounding the project area during off hours.
3.11 DD Form 1354: Contractor shall complete the DD Form 1354 for government approval.
A draft version of the DD Form 1354 will be submitted with the 95% design stage. An interim copy of the DD Form 1354 will be submitted at the pre-final inspection and a final DD Form

1354 will be submitted with final billing. DD Form 1354 should be prepared in accordance with the most current UFC 1-300-08.
4.0 SPECIAL REQUIREMENTS
4.1 Excavation Permits: The Contractor shall obtain an excavation permit (Work Clearance, AF Form 103) before any excavation is started, and secure a Confined Space Entry permit prior to crawlspace work. The form(s) and the offices to be contacted will be provided by the Government and the Contractor is responsible for obtaining the required coordination. Those areas which are known to contain underground utility lines will be designated as caution areas. The Contractor must exercise extreme caution when accomplishing the required excavation work. Hand excavation is required within 3 feet of existing utility lines. The Contractor shall repair all damaged utility lines without additional cost to the Government.
4.2 Restoration: The Contractor shall restore all excavated areas to their original condition, to include backfill, original compaction, paved surfaces/walkways, sodding and scrubbings.
Sod shall be placed edge to edge and staggered unless otherwise indicated.
4.3 Confined Spaces: The work area is classified as non-permit confined space. The Contractor shall comply with all requirements specified in OSHA Standards, 29CFR 1910.
146, EM 385-1-1 and AFI 91-203 (Oct 2016). All areas designated, as non-permit required confined space shall be properly vented of all potentially harmful vapors and dust. When using material that produce unpleasant vapors in occupied facilities, proper ventilation will be maintained at all times. All supply and return air systems will be sealed and tested to assure no vapors enter unaffected areas. Warning signs shall be posted indicating usage of potentially harmful vapors. All electrical receptacles will be taped over to reduce explosion hazards.
5.0 DESCRIPTION OF WORK: The work shall consist of, but is not limited to the following:
5.1 General:
1. Single story units: 33 units
2. Two story units: 72 units
3. General Officer’s Quarters 4445: 1 unit
4. Assess and repair existing systems: 38 units
5.2 Civil:
1. Excavation, removal, disposal and back fill of soil.
2. Provide aggregate.
3. Remove and replace sodding including topsoil for damaged surface.
4. Drilling the core at exterior concrete wall under floor slab to provide 4” Ø PVC radon exhaust pipe.

1. Repair and patchwork to match existing adjacent finishes.
2. Remove and replace floor tile and/or ceiling as necessary.
5.4 Electrical:
1. Provide electrical power for exhaust fan. The interior conduit shall run above ceiling as much as possible
2. Interior exposed conduit shall be metal wiremold and exterior exposed conduit shall be rigid steel galvanize (thick wall).
3. Provide a circuit breaker 1P, 125V, 20A with enclosure by the existing circuit breaker panel for power supply of new exhaust fan.
4. Assume approximate distance from new exhaust fan to existing circuit breaker panel is as follows:
a. 1 Story unit: 19m (Assume 16m of conduits run above ceiling.)
b. 2 Story unit: 32m (Assume 29m of conduits run above ceiling.)
c. GOQ 4445 unit: 19m (Assume 16m of conduits run above ceiling.)
5. Conduct insulation resistance test for all new electrical wires and submit test result.
6. Minimum resistance shall be 250,000 ohms.
5.5 Mechanical:
1. Provide 4” Ø PVC radon exhaust pipe, fittings, and pipe supports from the radon suction pit under the floor slab to above the roof.
2. Provide weather proof type radon exhaust fan with radon system monitor and all associated accessories. Provide corrosion-proof protective cover for fan with paint to match existing exterior wall.
3. Provide rain cap with bird screen for radon exhaust outlet pipe.
5.6 Asbestos / LBP Test:
1. Asbestos Test: One sampling and test with 39 different housing styles. (Total 39)
2. LBP Test: Two samplings and test with 39 different housing styles. (Total 78)
5.7 Post-Installation Radon Testing:
1. Test radon levels in each dwelling unit after each new installation or repair/upgrade of existing mitigation system.
2. Post-installation testing should begin no sooner than 24 hours nor later than 30 days after installation.
3. Radon tests shall follow EPA guidelines and shall be continuously monitoring utilizing Alpha Track devices (long term detectors) and are set out for a minimum of 91 days. One detector shall be installed per 186 square meters or 2,000 square feet.
4. Detectors should be placed in the lowest level of the facility which contains a room that is used regularly (used at least 4 hours or more) (e.g., living room).

5. Detectors should not be placed in a room or wall that has any type of exhaust or ventilation nearby (e.g., kitchen, bathroom).
6. Detectors should be placed where it will not be disturbed during the measurement period and where there is adequate room/space for the detector.
7. Detectors should not be placed near drafts caused by heating, ventilating, and air conditioning vents, doors, fans, and windows. Locations near heat such as appliances or direct sunlight, and high humidity areas should be avoided.
8. Because some detectors are sensitive to increased air motion, fans should not be operated in the test area.
9. Measurement location should not be within 3 ft (90 cm) of the doors and windows or other potential openings to the outdoors.
10. The detector should be placed at least 20 in (50 cm) from the floor, and at least 4 in (10 cm) from other objects. On walls, place at the breathing zone height which is approximately 5 to 6 feet (1.6 m to 1.8 m).
11. Ensure date and time of install and retrieval are recorded for each detector.
12. Ensure detectors are re-sealed using a ziplock bag inside the home.
13. Submit results of radon tests to the government.
14. If either the result of test exceeds 4.0 pCi/L, the Contractor shall take immediate corrective action, at no additional cost to the government, in order to reduce radon levels within the required threshold.
5.8 Warranty:
1. The Contractor shall repair any defects in materials, equipment, workmanship or design furnished under this Contract, for a period of one year after acceptance by the Government.
2. The Contractor shall remedy, at the Contractor’s expense, any failure to conform to the terms and conditions of the contract, or any defect.
