About this file

This is a Past Performance Questionnaire template for solicitation FA7000-10-R-0036 issued by the 36th Contracting Squadron at Andersen Air Force Base, Guam, for 36 WG Vehicle Removal and Towing Services. Contractors submitting quotes must complete and return the questionnaire via email to elena.gifford.1@us.af.mil and louella.navarrete@us.af.mil no later than September 22, 2026, at 1:00 PM Chamorro Standard Time (ChST). The questionnaire requires contractors to provide general contract information including contractor name, address, point of contact details, project title, contract number, overall contract value, and period of performance, with indication of whether the contractor served as prime or subcontractor.

The evaluation component of the questionnaire assesses contractor performance across six criteria using a five-point adjectival rating scale: Quality of Service, Schedule, Cost Control, Management of Key Personnel, Small Business Subcontracting, and Regulatory Compliance. Each criterion is defined with performance standards ranging from Exceptional to Unsatisfactory, with an additional "Not Applicable" option available for Cost Control. Contractors receiving Marginal or Unsatisfactory ratings on any criterion must provide substantive comments to support the assessment. The subject line of the email submission must include "PP 'Contractor Name'" to facilitate tracking and evaluation of past performance information.
