SCOPE OF WORK

FOR THE FOLLOWING PROJECTS

Northwest Field Hangars 1&2 Demolition

Andersen Air Force, Guam

18 JUNE 2024

I. SCOPE:

The contractor shall provide all labor, materials, equipment, permit and clearances, architectural and engineering services, transportation, supervision, and other incidental work required for the project. Other incidental work required includes all items and considerations necessary to ensure a complete and usable final product / site condition, including, but not limited to, the necessary structure demolition considerations not specifically stated elsewhere in this statement of work. Complete and usable final product means that the completed final product is finished throughout and can be used to fully satisfy the requirements and the intended purpose of the project.

The Contractor shall provide all resources (except as may be expressly stated in the contract as furnished by the Government) necessary to deliver and perform the work in accordance with the minimum requirements identified below.

This includes but is not limited to financial resources, overall oversight and management or other resources including subcontractors that may affect performance of the instant acquisition.

II. GENERAL REQUIREMENTS

The Contractor shall perform a site visit to field verify dimensions and scope of work for the demolition of the following projects:

Northwest Field (NWF) Hangars 1 & 2 and associated support structures

1. The Contractor shall furnish all labor, supervision, transportation, materials, testing, analysis, reports, permits, hauling, disposal, tools, equipment, and professional services to accomplish all work involved as necessary to demolish the two fabric tension hangars located on Northwest Field (NWF) in Yigo, Guam (see site map (Attachment A)). Specific work elements are detailed below:

2. The Contractor shall submit a work plan for approval, no later than 30 days after contract award but prior to mobilization. The work plan shall address site environmental concerns, hazardous material sampling and abatement procedures (see section H and I below) and detail the demolition work to be performed including utility disconnections and associated site work. The plan should include a schedule/timeline for required work. All work will be performed in accordance with all applicable local, state, and federal codes, standards, and requirements.

Contractor shall observe all environmental requirements listed in the attached AF813 (Attachment B). All submittals shall utilize a standard Air Force (AF) form 3000 cover sheet.

3. The contractor is responsible for providing all notifications, procedures, safety equipment, labor, packaging, and disposal services for abatement (performed by qualified individuals) needed throughout the two hangars and associated structures as identified in the report of findings.

4. Contractor is to disconnect supporting utilities for water, sewer, electrical, and data/communications as detailed in the work plan. Electrical service demolition is to include all overhead wiring/cabling, poles, pole mounted transformers, and street lighting back to the site disconnection point defined by Anderson AFB Civil Engineering.

Remaining work to be performed by contractor. Site utility map is being provided for reference only (Attachment A).

5. Contractor is to perform full demolition of two tension fabric hangars on NWF along with all attached/detached utility/storage rooms and fencing. No vegetation shall be disturbed during the execution of project activities. All driveways, parking, taxiways and apron areas are to remain. The contractor is to remove and properly dispose of all debris and hazmat to the appropriate landfill. Debris shall be cleared from all paved surfaces and ten (10) feet beyond paved surfaces in all directions of the work site. No debris is to be left on site – the government does not have any on-base disposal areas for contractor use. Additionally, the contractor shall recycle/dispose of all electrical transformers and HVAC units according to Federal and local EPA regulations.

6. Site restoration. Following the removal of all structures from the project site, all bolts and anchors protruding through the existing concrete shall be cut-off and sealed to prevent further erosion of steel and or concrete. The end state condition of the site will be a concrete surface that is level with existing concrete and that will not retain water. All disturbed areas, outside the existing concrete surfaces, are to be seeded and suitable growth established.

No spall areas are allowed. No holes or depressions are allowed. Contractor shall import suitable fill as needed to ensure adequate grade in demolished areas. Contractor is to abide by other requirements as listed in the project AF813 (Attachment B). If demolition will occur near wooded areas, impacts may be incurred due to nesting birds.

7. Debris control. In addition to instructions included in Section III.R., during demolition activities all material will be positively controlled at all times. Demolition activities are to include controls that ensure no loose material is permitted to depart the project site and into/onto active air spaces or surface areas. Special demolition methods will be discussed in the contractor’s work plan. Additionally, at the end of each workday all project site materials and debris will be placed in appropriate collection containers with lids that are latched closed. The project site will be inspected at the end of each workday to confirm there are no loose materials on the project site.

8. General. Government reserves the right to salvage any materials/equipment prior to building turnover to the contractor.

III. GENERAL INFORMATION

A. PERFORMANCE PERIOD

All demolition actions shall be completed as soon as possible, but no later than 90 calendar days after receipt of Notice to Proceed (NTP). The government has 7 days to review the demolition plan and the Contractor has 5 days to resubmit a disapproved plan.

B. GOVERNMENT FURNISHED UTILITIES

Due to the remote area of the worksite, government furnished utilities are not available for this contract.

C. WORKING HOURS

The hours of work for this project shall be from 7:00 A.M. to 4:00 P.M. Monday through Friday, excluding Federal holidays. No work shall commence without prior coordination with Contracting Officer’s Representative (COR). The Contractor must notify the Government in advance if work needs to be performed outside these working hours at least 21 days ahead of time and contingent upon approval by the CO.

D. VEHICLE, PASS AND IDENTIFICATION, SECURITY REQUIREMENTS

1. Search, Inspection, and Seizure: All vehicles, equipment, personnel, and hand-carried items, entering or exiting any base control point, or while on the base, are subject to search and inspection and seizure of any prohibited item, at any time. Random, unannounced, and regular searches and inspections can be conducted. These searches and inspections are intended to assure prohibited articles are not introduced to government property and to ensure prohibited items are not removed from the base or its facilities. These delays will not be charged to the government, as they are to be expected entry procedures for the installation. Delays entering the installation can be experienced, during entry or exit to secure areas, during military exercises, and increased threat postures; the contractor assumes all liabilities associated with such delays.

2. Parking: Parking of the contractor's vehicles shall be restricted to the area of work. The company name shall be prominently displayed on all construction vehicles parked when on federal installations.

3. Driver's Licenses: All persons entering or working on a government Installation that are required to operate a motor vehicle shall possess a valid driver's license. For personnel operating specialized equipment, a Commercial Driver's License (CDL) is required as appropriate.

4. The use of seat belts by all vehicle operators and passengers is mandatory. Vehicle operators caught not wearing seatbelts will lose their driving privileges, subject to laws governing military and civilian employees.

5. On-base contractor employees shall ensure the following pass and identification items required for contract performance, are obtained for employees and non-government owned vehicles.

6. Base Access: All contractor personnel are required to pass security checks prior to being issued a base pass allowing them access to AAFB and/or other remote sites. The CO will provide instructions and forms for the contractor to fill out and return. These will be reviewed and forwarded to the 36 SFS for approval. Once approval/disapproval has been received, they will be forwarded to the contractor. This will be notice that the employees may go to the visitor's center and get their access pass one (1) to three (3) day base access passes may be requested through authorized COR and or 36 CONS personnel for the purposes of deliveries and infrequent access needs. Requests may be submitted 72 hours in advance by email or phone call.

7. Retrieving Identification Media: The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires, e.g., terminated for cause, retirement, etc. It is the responsibility of the contractor to turn in all base pass to the visitor center.

8. Emergency Exercise Support: The 36 Wing Commander has the responsibility to ensure the safety of all base personnel to include active duty, civilian personnel, family members and contractors on AAFB. Training exercises are accomplished to ensure preparedness for major disasters and emergencies, including catastrophic incidents in support of operations at AAFB. To achieve this goal, all members (including contractors) operating on AAFB are required to be prepared in case a real-world event occurs. Contractors and subcontractors shall follow the directions of first responders (security forces, fire department and medical personnel) during an exercise or real-world event.

9. Controlled Unclassified Information (CUI). The contractor shall comply with AFI 37-131, Freedom of Information Act Program (FOIA) requirements. The regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding CUI material.

10. The contractor shall comply with security requirements imposed by the Installation Commander and/or the agency occupying the space where work is to be performed, including any necessary security clearances. Prior to mobilization the contractor shall provide the CO a list of personnel requiring authority to work on AAFB. The list shall be kept current, and the CO shall be notified as to any changes in the list.

11. Additional Security Requirements: In accordance with DOD 5200.1-R, and AFI 31-401, the contractor shall comply with requirements identified in AFSSI 9100, Communications Security (COMSEC), Command, Control, Communications and Computer (C4) Systems Security, Security Awareness Training Education (SATE), and Operations Security (OPSEC).

12. Traffic Laws. The contractor and its employees shall comply with all state and base traffic regulations.

E. CONTRACTOR ANTI-TERRORISM AWARENESS

F. BUILDING OCCUPANCY

The facilities and supporting structures to be demolished are NOT currently occupied.

G. HAZARDOUS NOISE

Contractor is advised that work is adjacent to aircraft movement areas and noise levels may exceed OSHA standards.

H. HAZARDOUS MATERIAL

The following buildings were built:

NWF Hangar 1 & 2 and support structures – 2022

Asbestos Containing Materials (ACM), lead (most likely from lead-based paint), other hazardous materials (e.g., radon, refrigerants, heavy metals, etc.) may be presumed to be present, along with mold contamination. The Contractor shall have the facility surveyed for any hazardous materials by a certified HAZMAT assessment contractor prior to beginning the project.

The following must be followed:

a. 40 CFR Part 61 Subpart M (up to date as of 11/07/2023) National Emission Standard for Asbestos § 61.145 Standard for demolition and renovation MUST BE FOLLOWED for any renovation and demolition work performed on AAFB.

b. After the COR accepts the abatement plan (after review by an asbestos certified CEN/CEI inspector), any abatement work, including abatement set up, air monitoring, etc., and any waste removal must follow federal and Air Force standards and regulations. The paperwork for all abatement work must be accepted by the COR and an asbestos certified Construction Inspector. All certifications, licenses, etc. for the assessment and abatement contractors must be accepted by the COR prior to work being performed. (The paperwork must be accepted by an asbestos certified person from CEN/CEI prior to the acceptance by the COR.)

1. HAZMAT Results: The Contractor MUST use a certified pre-approved contractor who will submit a combined HAZMAT (for any asbestos, mold, lead, heavy metals, refrigerants, etc.) survey.

2. HAZMAT Reports: The report shall include the following information: (1) scope of work; (2) a description of the facility inspected (include age and type of construction); (3) list of regulations and standards applicable to work performed; (4) certifications of inspectors/assessors; (5) analytical laboratory certification; (6) analytical data; (7) type, condition and percentages of HAZMAT; and (8) building layouts/plans identifying sample locations, which should also have accompanying photographs detailing the exact area of testing. This assessment report along with any ensuring abatement plan (written by a qualified/certified contractor) must be included in the Demolition Plan submittal.

Qualifications. The work described herein shall be performed by:

• ASBESTOS

o USEPA Accredited Asbestos Assessor Training Certificate o USEPA Certified Asbestos Assessor (for the individual and the firm) o National Voluntary Laboratory Accreditation Program (NVLAP) accredited laboratory which can perform Polarized Light Microscopy (PLM) and Transmission Electron Microscopy analyses.

• LEAD BASED PAINT

o USEPA Accredited Lead Risk Assessor Training Certificate o USEPA Lead Certification (for the individual and the firm)

• MOLD, RADON, HEAVY METALS, PCB and REFRIGERANT o Properly Trained and Certified Personnel

I. DISPOSAL OF WASTE/EXCESS MATERIAL

Any HAZWASTE or excess material resulting from any contract requirement shall be removed and disposed of by the Contractor in strict accordance with applicable Federal, or Territory laws. Disposal, discharge, deposit, dumping, spilling, leaking, or placing of any material, wastes, effluents, trash, garbage, oil, grease, paint, chemicals, etc. in areas other than proper disposal sites away from Andersen Air Force Base shall be subject to the approval of the CO. If any HAZWASTE is dumped in unauthorized areas, or contaminates other areas, the contractor shall remove the material and restore the area to the condition of the adjacent undisturbed area with no reimbursement from the government, or, at no additional cost to the government. Any contaminated ground/soil shall be excavated and disposed of, as directed by the CO, and replaced with suitable fill material, compacted, and finished to match adjoining surfaces. The Contractor shall comply with all GUAM EPA, Federal and Air Force regulations pertaining to disposal of debris.

The Contractor shall furnish the following information to the CO in the HAZMAT ABATEMENT PLAN:

• Name and address of the proposed landfill to be used for disposal of project related waste.

• Landfill class

• Proposed materials to be disposed of at this location

This information is to be submitted for all materials to be disposed of by the contractor, to include HAZWASTE, and will be submitted and approved before demolition work begins. Copies of manifests and inventories of HAZWASTE that were disposed of during the course of the contract must be provided to the CO at the conclusion of the contract. The closeout submittal must contain all information required by the initial submittal.

1. Disposal of Hazardous Material: The Contractor shall comply with all territory of Guam and Federal laws pertaining to the protection of the environment and emission of hazardous pollutants in the performance of this contract. The Contractor shall also provide a copy to the CO and the Base Environmental Protection Monitor of all correspondence with local, GUAM EPA, or Federal pollution control authorities. Any HAZWASTE generated by the Contractor shall be the Contractor's responsibility to dispose of in accordance with the HAZMAT Abatement Plan. The Contractor shall coordinate all movement of hazardous waste with 36 CES/CEIE Environmental Flight.

2. Spillage: Special measures shall be taken to prevent paints, chemicals, fuels, oils, grease, etc., from being spilled and particularly from entering surface or groundwater. In the event of paint, fuel, oils, or chemical spill the Contractor shall immediately take containment measures to prevent the spill from migrating and/or entering the base drainage system.

Hazardous spill residue shall be disposed of in accordance with all GUAM EPA and Federal laws pertaining to the protection of the environmental and emission of hazardous pollutants in the performance of this contract. All spills shall be reported as soon as possible to the 36 MDG Base Environmental Flight, 36 CES/CEIE Environmental Flight and 36 WG/SEG Base Safety at (671) 366-4385 and (671) 366-4222 respectively.

J. ENVIRONMENTAL PROTECTION

For the purpose of this specification, environmental pollution is defined as a presence of chemical, physical, or biological elements or agents which adversely affect human health or welfare; unfavorably alter ecological balances of importance to human life; affect other species of importance to man; or degrade the utility of the environment for aesthetic and recreational purposes. The control of environmental pollution requires consideration of air, water, and land, and involves noise, solid-waste management and management of radiant energy and radioactive materials, as well as pollutants.

1. Applicable Regulations: In order to prevent and to provide for abatement and control of any environmental pollution arising from the demolition activities of the Contractor and any subcontractor employed by the Contractor in the performance of this contract, they shall comply with the applicable Federal, State, and local laws and regulations concerning environmental; pollution control and lead/asbestos abatement in effect on the date of the solicitation.

2. Protection of Land Resources: It is intended the land resources within the project boundaries of work performed under this contract be preserved in their present condition or be restored to a condition after completion of demolition that will appear to be natural and not detract from the appearance of the project. Insofar as possible, the Contractor shall confine activities to areas defined by plans and specifications.

3. Training: Contractor shall implement an employee’s environmental awareness program which will be provided to demolition and operation employees with information to encourage awareness and preservation of the desert ecosystem and the resources found in the NEPA regulations This information shall be distributed to and discussed with all employees during employee orientation sessions. This information shall also be provided to all visitors and subcontractors that will be on site. The Contractor shall have all persons participating in the environmental awareness program to sign an affidavit declaring that the individual understands and shall adhere to the guidelines set forth in the program material.

4. Protection of Water Resources: Contractor shall not pollute streams, lakes, or reservoirs with fuels, oils, bitumen, calcium chloride, painting materials, acids, or other harmful materials. It is the responsibility of the Contractor to investigate and comply with all applicable Federal, State, County, and Municipal laws concerning pollution of rivers and streams. All work under this contract shall be performed in such a manner that objectionable conditions will not be created in streams through or adjacent to the project area.

5. Protection of Fish and Wildlife: Contractor shall always perform all work, taking all such steps required to prevent any interference or disturbance to fish and/or wildlife. Fouling or polluting of water will not be tolerated. Wash waters and waste shall be processed, filtered, ponded, or effectively treated prior to their release into any body of water.

6. Protection of Air Quality: Contractor shall comply with all air pollution restrictions applicable to this project. Waste materials shall not be burned on Government premises. If the contractor elects to dispose of waste materials, (other than hazardous waste) off Government premises, by burning, he shall make his own arrangements for such burning area and shall be responsible for all permits and conformance to all local, state, and federal regulations.

7. AF Form 813 (Request for Environmental Impact Analysis): The Contractor shall comply with AF Form 813 (Attachment B) requirements checklist as defined. The project will comply with National Environmental Policy Act (NEPA Requirements).

K. CONTRACTOR QUALITY CONTROL (CQC) PLAN

Contractor shall establish and maintained an effective Quality Control (QC) system that complies with FAR 52.246-13 Inspection of Dismantling, Demolition, or Removal of Improvements. QC Plan shall include but not limited to the following:

1. Procedures for scheduling, reviewing, certifying, and managing submittals, including those of subcontractors, consultants, suppliers, and purchasing agents.

2. Procedures for tracking preparatory, initial, and follow-up control phases and control, verification, and acceptance tests including documentation.

3. Cover demolition operation of this project, both on-site and off-site, and be keyed to the proposed demolition sequence.

4. Organizational chart showing the lines of authority and acknowledgement that the QC staff will implement the three-phase control system for all aspect of the work specified.

Contractor shall include in the QC Plan key personnel’s name, qualification (in resume format), duties, responsibilities, authorities of each person assigned.

Project Superintendent

Project Superintendent shall have a minimum of five (5) years’ experience in structure demolition with at least 3 years as a superintendent from projects similar in size and complexity. The individual must be familiar with the requirements of EM 385-1-1and have experience in the areas of hazard identification and safety compliance. The individual must be capable of interpreting a critical path schedule and demolition activities milestones. The qualification requirements for the alternate superintendent are the same as for the project superintendent. The CO may request proof of the superintendent's qualifications at any point in the project if the performance of the superintendent is in question.

Project Superintendent shall be responsible for the quality of work and subject to removal by the CO for non-compliance with the quality requirements specified in this Contract. Project Superintendent shall be always present on site and shall be responsible for all demolition and related activities on site. Notify the CO in writing of any proposed changes and are subject to approval/acceptance by the CO.

QA/QC Manager

QA/QC Manager shall be an individual within the onsite work organization that is responsible for overall management of QA/QC and has the authority to act in all QA/QC matters for the Prime Contractor. The QA/QC Manager shall have a minimum of five (5) years’ experience in structure demolition with at least 3 years as a QA/QC Manager on projects similar in size and complexity. QA/QC Manager always maintains a presence at the site during progress of the work and have complete authority and responsibility to take any action necessary to ensure contract compliance. QA/QC Manager must be physically present at the work site to participate in the QC Meetings, perform the three phases of control, including participation in Preparatory and Initial Phase meetings, and to perform and document follow-up inspections for each definable feature of work in their area of responsibility. QA/QC Manager shall be assigned no other duties and will not under Project Superintendent’s chain of command.

Site Safety and Health Officer (SSHO)

SSHO shall meet the requirements of EM 385-1-1Section 1. The SSHO shall ensure that the requirements of 29 CFR

1926.16 are met for the project. The SSHO or an equally qualified Alternate SSHO must be always at the work site to implement and administer the Contractor's HASP program and Government-accepted Accident Prevention Plan. The SSHO and Alternate SSHO must have the required training, experience, and qualifications in accordance with EM 385- 1-1 Section 01.A.17, and all associated sub-paragraphs.

SSHO must have completed a 40-hour contract safety awareness course based on the content and principles of EM 385- 1-1 and instructed in accordance with the guidelines of ASSP Z490.1, by a trainer meeting the qualifications of QUALIFIED TRAINER REQUIREMENTS. If the SSHO does not have a current certification, certification must be obtained within 60 days maximum of Contract award.

L. AF FORM 103 BCE WORK CLEARANCE REQUEST

Contractor shall coordinate with appropriate utility locating agencies, including base communications cable maintenance (item 1 through 15) on an AF Form 103. AF Form 103 will be given to the Contractor during the pre-demolition meeting.

Contractor shall complete and follow all instructions on AF Form 103 before start of work on base. A copy of AF Form 103 instruction must be always on the demolition project premises while work is being performed.

M. AF 592 WELDING PERMIT

Contractor shall obtain an AF 592 welding permit for when welding, cutting, brazing, or using of any open flame device from the 36 CES Fire Prevention Section, Bldg. 17002, (671) 362-3113, before welding equipment or a cutting torch is used. Request permits 24 hours in advance of need date.

N. UTILITY OUTAGES

An AF 103 BCE Work Clearance Request must be coordinated with 36 CES shops and must be approved at least thirty

(30) calendar days in advance prior to any utility outages. As a minimum, the request should include the location of the outage, utilities being affected, duration of outage and any necessary sketches. Contractor shall notify in writing the CO, PM, PI, affected tenants, and customers fourteen (14) calendar days prior to the interruption. Contractor shall coordinate all power outages with the CO, DZSP21, GPA and other applicable agencies and pay all charges incurred at additional cost to the Government. If the discontinued service is due to an emergency breakdown the Contractor shall notify the CO, affected tenants and customers as soon as practicable.

Once approved, and prior to beginning work on the utility system requiring shut down, the Contractor shall attend a pre-coordination meeting with the CO, Contracting Officer Representative (COR), PM, PI, DZSP21, and customers to review the scope of work and the lock-out/tag-out procedures for worker protection. No work will be performed on energized electrical circuits unless proof is provided that no other means exist. Fire system disconnection and reactivation requires 36 CES Fire Prevention Section, 7002, (671) 362-3113 coordination.

O. EXPLOSIVE SAFETY SUBMISSION (ESS) REQUIREMENTS

If any portion of the referenced scope of work requires ground intrusion activities or excavation work, the contractor will be required to adhere to Munitions Response Explosives Safety Submission (MRESS), Guam Construction Support, Amendment 7. Additionally, comply with the Latest edition specification Section 01 57 19.01 20 Supplemental Temporary Environmental Controls, Joint Region Marianas Explosives Safety Submission (JRM ESS).

Provide Munitions and Explosives of Concern (MEC) Clearance in accordance with the latest Explosives Safety Submission (ESS) Munitions Response Sites Guam Construction Support Implementation in accordance with the following items:

1. Munition and Explosives of Concerns (MEC) Services

a. Project Management and Planning

b. MEC Work Plans

c. Site Preparation, Mobilization/Demobilization

d. Digital Geophysical Survey (DGM) or Handheld Manual Magnetometer Detector and Marking

e. After Action Reporting (AAR)

2. Intrusive Investigation

a. MEC/MPPEH (Material Potentially Presenting an Explosive Hazard) removal, subsurface investigations, and other ground penetrating work

Provide details of ESS implementation and removal of anomalies upon demolition contract award and how this work is incorporated in the project schedule. Narrative solution shall demonstrate understanding of ESS implementation in accordance with the ESS requirements. Do not exceed three (3) single-sided, 8-1/2 x 11 pages.

DISCLAIMER: Historic anomaly averages contained within the E-16B memo incorporated into this contract do not reflect conditions at the project site. Contractor assumes the risk that the number of anomalies encountered at the project site may differ from historic averages reflected in the E-16B memo.

P. RADIOACTIVE MATERIAL (RAM) PERMIT

Use of equipment containing radioactive isotopes or any nuclear source (such as density testers, moisture detectors, etc.)

must be approved by the USAF Radioisotope Committee through the installation Radiological Protection Officer (IRPO).

If such equipment is to be used in the work, the Contractor must submit the following documents for issuance of RAM permit:

• Brief description of the proposed activities

• Copy of the NRC or Agreement State License with current NRC 241

• Name, local address, and phone number for the responsible local representative

• Name, address, and phone number of the RSO named on the license

• Copy of SOW or SOO describing the work to be done and inclusive dates

• Copy of plan ensuring compliance with NRC, AFI/AFMAN, or other safety requirements

• Location and duration of use and storage location

• Leak test (if applicable)

• Authorized user

The Contractor must submit the following documents through the CO’s designated representative, no less than 14 days before the anticipated use of such equipment to obtain the RAM permit. Additionally, during the period of use of such equipment, the IRPO is authorized to make periodic checks to insure that proper radiological health precautions are being followed. If the IRPO determines that these precautions are not being followed, the contractor will immediately notify the CO to initiate corrective action.

Q. NO SMOKING POLICY

In accordance with AFI 48-104 Tobacco Free Living (11 Jul 2019); there is NO SMOKING in any Government vehicle or buildings (this includes existing buildings, buildings under construction and building under renovation). Discarding tobacco materials other than into designated tobacco receptacles is considered littering and subject to fines. Smoking is only permitted in Designated Tobacco Use Areas (DTAs) Only.

R. TRASH AND LITTER CONTROL

The Contractor shall develop a strict trash and litter control program. The litter control program shall consist of supplying an adequate number of covered trash and litter receptacles in all appropriate locations. Good housekeeping shall be maintained to ensure that little fire ant (LFA) do not infest the project site. Any trash, litter, project site debris, or vehicles found to be infested by LFA are to be reported immediately, sanitized, treated, and re-tested until LFA are no longer detected. All equipment entering designated areas must be inventoried before and prior to leaving the designated areas. If necessary, green waste piles containing coconut rhinoceros beetle (CRB) host plant material such as coconut trees, pandanus, or other palms shall be covered with tekken netting and disposed off-site permitted facility as soon as possible.

S. POST-DEMOLITION CLEANUP

The Contractor shall remove and clean up all signs of temporary demolition facilities such as haul roads, work areas, structures, foundations of temporary structures, or any vestiges of demolition activity resulting from this contract, upon completion of work as directed by the PI or PM.

T. PROHIBITED ITEMS

Alcohol, controlled substances including illegal drugs and associated paraphernalia, except for prescribed medicine, are not allowed on base. Prescription medicine must be in the original bottle with the prescribed owners name clearly printed on the bottle. Weapons, video and audio cassette players, recorders and tapes, binoculars, film, and computer media are prohibited from the work site.

U. SAFETY AND HEALTH REQUUIREMENTS

The Contractor shall ensure that job site cleanliness will be always maintained, and barricades/safety cones will be utilized to isolate the job site from normal personnel traffic. It is the Contractor's responsibility to ensure its employees and managers have a comprehensive understanding of and full compliance with OSHA requirements. Detailed information is available on the OSHA website at http://www.osha.gov. It is the Contractor's sole responsibility for compliance with OSHA, Public Law 91-596. The Contractor shall further comply with EM 385-1-1 Safety and Health Requirements Manual and OSHA Standards of Construction (Title 29, CFR Part 926); and all federal, state, and local laws, and Air Force Instructions for the protection of their employees while working on an Air Force Installation or property. The protection of contractor employees is exclusively the obligation of the Contractor. The Contractor shall manage all work areas to ensure the safety of building occupants, base personnel, or visitors in or near the areas.

The Contractor will be required to follow current Health Protection Condition (HPCON) restrictions and sanitization procedures to include social distancing and wearing of Center for Disease Control (CDC) - approved face masks while conducting business on the base or test locations to maintain personal and public health and safety.

1. MISHAP Reporting: The Contractor shall record and promptly report to the CO, PM, and PI pertinent facts regarding mishaps involving Government personnel or property coincident to work performed as part of the Statement of Work that occur on or off an Air Force installation IAW DAFI 91-204, Safety Investigation and Hazard Reporting. In the event of an accident or mishap, the Contractor shall take prudent and reasonable action to establish control of the http://www.osha.gov/ accident or mishap scene until relieved by the CO and PM. If the Government elects to investigate the accident or mishap, the Contractor shall cooperate fully and assist Government personnel in the investigation until such is complete.

2. Health and Safety Plan (HASP): In accordance with AFI 91-202, The US Air Force Mishap Prevention Program, the Contractor shall submit a HASP to the CO for review and approval by the 36 WG/SEG. The HASP will describe capabilities and processes to ensure compliance with safety and health requirements during the performance of the work described in the solicitation. The HASP must include site-specific mishap reporting procedures for incidents which result in damage to Government property or injury to Air Force personnel.

The Contractor’s plan shall include appropriate measures to ensure the Contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in Contractor’s work areas. The Contractor’s Plan must include appropriate points of contact (name, position, and contact numbers) for immediate notification should a safety issue be identified.

The Contractor is responsible for establishing these requirements for all subcontractors who qualify as subcontractors under the resulting contract.

3. Jet Aircraft Noise: Jet aircraft operating on the runway and/or taxiways produce hazardous noise levels. The Contractor is advised to take appropriate measures to protect themselves and their employees as well as notifying any subcontractors of the noise hazard.

4. Safety Equipment: Contractors will be responsible for the selection and acquisition of personal protective equipment (PPE) in performance of this contract. Safety equipment may include but not limited to shoes or boots, head, face and eye protection, respirators, and gloves.

5. Confined Space Requirements: When a Contractor will perform work that involves a permit-required confined space entry, the CO or PM will ensure compliance with the criteria in AFMAN 91-203, 23.8 and the Master Entry Plan of the unit controlling the space. While Contractors are responsible for rescue of their personnel, they must be advised that 911 calls will be routed back to 36 CES/CEF Emergency Operations Center. To prevent potential delays, notify the Fire Department at (671) 366-5284 prior to and after entry.
