PRELIMINARY STATEMENT OF WORK

PORTABLE POP-UP TOWERS

MAINTENANCE/SERVICE CONTRACT

ANDERSEN AIR FORCE BASE (GUAM)

8 AUGUST 2016

1. GENERAL The work covered by this contract consists of, but is not limited to, the removal of the built in hydraulic system and replace with a pneumatic system, the maintenance, labor, services, tools, materials, equipment, transportation, supervision and all requirements to perform all operations in conjunction with maintenance and servicing of the Portable Pop-Up Towers once 100% safely operational, on Andersen Air Force Base (AAFB), Guam in support of the 736 SFS Commando

Warrior.

2. CONTRACTOR REQUIREMENTS

a. Remove built in hydraulic system (used to erect platform) and replace with a pneumatic system.

b. Ensure pop-up towers are 100% safe and operable prior to starting scheduled maintenance

(all towers must be able to raise and lower using the pneumatic system).

c. Remove existing locking mechanism which prevents the platform from dropping when the unit is erect and replace with a locking mechanism that will not require any personnel to be under the platform, when locking/unlocking.

d. The contractor shall provide quarterly maintenance service and respond within 48 hours of all unscheduled maintenance/repair.

e. Vendor must be experienced in servicing pneumatic system, hydraulic system, electrical system and metal structure framework.

f. Remove wooden platform (to include the wooden hatch) and replace with steel grate

(secure steel grate to the metal framework, covering the whole platform). Place at least 1/2” pressure treated plywood on top of the steel grate (covering the whole platform).

g. Use stainless steel hinge for hatch.

h. All work shall be in accordance with the manufacturer’s requirements.

i. Full Equipment List:

* Portable Pop-Up Tower (8 each)

j. Estimated Service List:

A. The list below is an estimate of the maintenance and service needs for the Pop-Up

Towers. If additional service or maintenance items are needed, approval must first be obtained through the Contracting Officer prior to proceeding with repairs.

B. These estimates for minor services are based on average maintenance per quarter.

CHECKLIST

-Check pneumatic system for 100% serviceability

-Check Structure for Integrity and Safety; Repair as Needed

-Check Platform; Repair as Needed

-Bolt, nut, fasteners tightness – inspect; Repair as Needed

-General lube – grease fittings, etc.

3. UNSCHEDULED MAINTENANCE

a. After Service Repair - If the tower has problems after repair, the contractor shall repair any discrepancy within 48 hours of notification. If the tower(s) are NOT repaired completely within the 48-hour timeframe, the contractor shall notify 736 SFS/CW-S4 with the rationale for the delay.

b. Unscheduled Repair - If the tower(s) malfunctions during the contract period (but out of cycle of the quarterly inspection and maintenance) the contractor shall provide needed repairs of the tower(s) within 48 hours of notification. If the tower(s) are NOT repaired completely within the 48-hour timeframe, the contractor shall notify 736 SFS/CW-S4 with the rationale for the delay.

4. BASE ACCESS The contractor must notify the 736 SFS/CW-S4, 72 hours prior to inspecting/repairing tower(s) and arrange a time to meet at AAFB visitors center Bldg. 2403, provide a valid photograph, vehicle registration form, and proof of insurance to obtain an AF Form 75, visitor and vehicle pass if needed.

5. LOCATION All maintenance work shall be performed at tower(s) site.

6. WORK INTERRUPTION All work must be accomplished in such a manner that minimum inconvenience will be caused to the using agency.

7. SCHEDULE All routine work shall be completed within one week. Notification is needed for any tower(s) needing more than one week maintenance/repair due to parts being ordered from off island vendors.

8. PERFORMANCE The performance period will begin within 10 working days after receipt of contract.

9. QUALITY ASSURANCE

a. Inspection at Tower Site Location - After completion of service the vendor must inspect the tower(s) to ensure proper function and provide a copy of the repair invoice to the

736 SFS/CW-S4. If there’s any additional services required, a written quote must be submitted to 736 SFS/CW-S4 for approval prior to providing additional service.

Inspection Checklist Items

1. Did the vendor provide a copy of the service ticket?

2. During on-site inspection, were all items service items completed?

3. Are ALL towers in proper and safe working condition?

4. Are there any additional services required to any tower(s)? If so, will it be rescheduled?

b. Reminder for inspection - Contractor is to provide a reminder for quarterly service at least

5 days prior to services being conducted.
