SECTION 4: EVALUATION
Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:
PERFORMANCE RATING
EXCEPTIONAL (E) - During the entire period, the contractor met and exceeded many of the requirements of the contract and consistently performed at a superior level. Performance was accomplished with very few minor problems, and the contractor took immediate and effective corrective actions for those problems that did occur.
VERY GOOD (VG) - During the entire period, the contractor met and exceeded some of the requirements of the contract and consistently performed very well. Performance was accomplished with some minor problems, and the contractor took timely and effective corrective action for those problems that did occur.
SATISFACTORY (S) - During the entire period, the contractor met the requirements of the contract and consistently performed at an acceptable level. Performance was accomplished with some problems, and the contractor took effective corrective action for those problems that did occur.
MARGINAL (M) – During the entire period, the contractor did not always meet some of the requirements of the contract and intervention was required to continue performance. There were instances where performance was at a less than acceptable level; performance was accomplished with some problems and some corrective actions appear only marginally effective or were not fully implemented.
UNSATISFACTORY(US) – During the entire period, the contractor did not meet the requirements of the contract and performance was at an unacceptable level. There were a number of serious problems that required extensive oversight and involvement, and corrective actions were either ineffective or non-existent.
NOT APPLICABLE (N/A) - Unable to provide a rating. Contract did not include performance for this aspect, or information was not available. Do not know.

T1 How well did the contractor meet your technical support requirements
(system and equipment setup, equipment and software support, production, breakdown, and maintenance requirements) to support your educational and/or events? Did they identify, trouble-shoot and quantify the technical solutions needed to ensure your educational/event information was visibility, sustained and effectively communicated or produced?
T2 How well did the contractor setup, maintain and operate your visual information systems (such as projectors, video routers/switches, audio systems, etc.) to meet schedules with the presentation quality specified?
Did they work with your event leads/coordinators to ensure the event and its messaging was effectively produced and smoothly conducted? Did their operations meet allowed costs of operation?
T3 How well did the contractor manage your multimedia materials (such as video recordings, video capture and editing, multimedia publications, etc.) to meet schedules and the production quality specified? Did they partner effectively with your business units to ensure the multimedia materials were effectively and credibly developed and coordinated?
T4 How well did the contractor demonstrate artistic knowledge and skill to meet your requirements for graphical information for presenting your event in an effective and insightful way with the presentation quality specified (Graphical product used could be items such as original artwork, charts, graphics, posters, brochures, lettering, book covers, publications, etc.)? Did they work with your command to ensure your visual messaging was effectively communicated and met schedules?
T5. How well did the contractor provide the desired knowledge of software and hardware to meet your needs in the administration and support of your servers, applications, network infrastructure, and their associated configuration, maintenance, and monitoring; supporting and securing of Microsoft Windows server technology, Active Directory, group policies and virtual networks & servers; configuring, securing, and optimizing networking topology, DNS, and routing?
T6. How well did the contractor meet Defense Information Systems
Agency, DoD and your command/agency requirements for security and business requirements (such as the DISA VTC Authority To Connect process)?
T7. How well did the contractor provide computer user Help Desk support in support of your training curriculum? Did they set up appropriate classroom IT or telecommunications environments; meet organization’s training requirements and provide the proper assistance in a timely manner, meeting schedules and priorities required of your command/agency; deploy digital technologies and assist users in the proper use of these assets; patiently answer questions and give training;
keep up-to-date on the latest trends in learning technologies, operations and users requests to ensure proper trouble-shooting resolutions?
