SECTION 4: EVALUATION

Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

PERFORMANCE RATING

EXCEPTIONAL (E) - During the entire period, the contractor met and exceeded many of the requirements of the contract and consistently performed at a superior level. Performance was accomplished with very few minor problems, and the contractor took immediate and effective corrective actions for those problems that did occur.

VERY GOOD (VG) - During the entire period, the contractor met and exceeded some of the requirements of the contract and consistently performed very well. Performance was accomplished with some minor problems, and the contractor took timely and effective corrective action for those problems that did occur.

SATISFACTORY (S) - During the entire period, the contractor met the requirements of the contract and consistently performed at an acceptable level. Performance was accomplished with some problems, and the contractor took effective corrective action for those problems that did occur.

MARGINAL (M) – During the entire period, the contractor did not always meet some of the requirements of the contract and intervention was required to continue performance. There were instances where performance was at a less than acceptable level; performance was accomplished with some problems and some corrective actions appear only marginally effective or were not fully implemented.

UNSATISFACTORY(US) – During the entire period, the contractor did not meet the requirements of the contract and performance was at an unacceptable level. There were a number of serious problems that required extensive oversight and involvement, and corrective actions were either ineffective or non-existent.

T1 How well did the contractor meet on-site technical support, database administration, equipment and software maintenance and sustainment of stand-alone and web-based capabilities to integrate into civil engineering and/or other functional community business rules and practices such as

Linear Segmentation of Horizontal Infrastructure mapping data to identify and quantify linear segments and nodes to ensure: asset visibility;

accurate inventory; characterization of age, type and condition; and integrated engineering performance analysis for visualization of a systems criticalities and the associated costs of operation and sustainment?

T2 How well did the contractor acquire, maintain and consolidate geospatial data holdings within an organization’s-specific Spatial Data

Standards for Facilities, Infrastructure, and Environment model meeting timeframe and quality specifications; encompassing data from geographically separated units that collect, edit and store data independently and for replication organization-wide through global positioning system surveys; imagery rectification, feature extraction and digitizing; topology and data reviews to provide authoritative imagery, vector and attribute data?

T3 How well did the contractor meet Defense Information Systems

T4 How well did the contractor provide IT consultation; administration, maintenance and sustainment of hardware and software for a federated enterprise solution of servers at geographically separated organizations to include backing up of data, hosting and replicating of web-based services and authenticating users and various security groups to provide information to internal and external customers?

T5. How well did the contractor provide server and desktop development for Oracle databases and ArcSDE web-services (i.e., creating desktop/server geo-processing tasks and server object extensions) for geospatial applications?

T6. How well did the contractor geo-enable cross-functional communities and Automated Information Systems by obtaining, creating and integrating data and web-based viewers, applications, interfaces and tools to more efficiently and effectively perform tasks with real property, asset management and maintenance IAW Law, Regulatory tracking and reporting for Environmental, Safety, and Occupational Health

Management System’s; as well as, systems and processes required for

Airfield and/or Military Operations and Hazard Analysis and

Management?
