3. EVALUATION
Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided. This scale is defined as follows:
EXCEPTIONAL: Performance meets contractual requirements and EXCEEDS MANY to the customer’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
VERY GOOD: Performance meets contractual requirements and EXCEEDS SOME to the customer’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
SATISFACTORY: Performance MEETS contractual requirements. The contractual performance of the mailto:Ryan.Truong.1@us.af.mil mailto:James.watanabe@us.af.mil element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
MARGINAL: Performance DOES NOT MEET SOME contractual requirements. The contractual Performance of the element or sub-element being assessed reflects a serious problem for which the Contractor has not yet identified corrective actions. The contractor’s proposed actions appear only Marginally effective or were not fully implemented.
UNSATISFACTORY: Performance DOES NOT MEET MOST contractual requirements and recovery is Not in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
NOT APPLICABLE: Performance is this area is not applicable to effort being assessed or is unknown.

P2. Maintained workforce that possessed requisite technical knowledge education, skills, certifications and appropriate security clearance throughout life of the contract
P3. Performed assigned tasks and produced required products to an acceptable level in accordance with prescribed timelines

TECHNICAL PERFORMANCE
TP1. Ability to understand/comply with customer objectives and technical requirements
TP2. Ability to plan and stay within prescribed budget(s) for assigned tasks, and Identify and implement technical improvements or cost savings that benefit the government
TP3. Ability to carry out theater commander’s guidance IAW theater plans, directives and policies
TP4. Capable of providing clear and concise updates to staff and senior leaders on mission-related areas and make Course of Action (COA) recommendations on assigned areas of responsibilities
TRANSITION
T1. Successfully transitioned resources and personnel to assume work under new contract and maintain a seamless continuity of services
T2. Effectively and efficiently identified shortfalls/gaps in service and adjusted to ensure mission requirements were met under new contract
COST
C1. Ability to accurately identify costs for all work activities or projects
C2. Accuracy in forecasting contract costs
C3. Ability to meet forecasted costs and perform within budget
C4. Ability to alert Government of unforeseen costs before they occur
