Amendment 0001 to RFQ FA520526QB111 – Gym Equipment for B112
This is Amendment No. 0001 to a Request for Quotation (RFQ) issued by the 35th Contracting Squadron, USAF at Misawa Air Base, Japan for gym equipment. The amendment changes the quotation submission deadline to 20 August 2026 at 1200 hours Japan Standard Time and revises both the Statement of Work (Attachment No. 1) and the Statement of Work for CLIN 1001 (Attachment No. 2), both dated 4 August 2026, to delete paragraph 2.7.4. The RFQ solicits four line items: Gym Dividing Net System (CLIN 0001), Wall Padding (CLIN 0002), Gym Bleachers (CLIN 0003), and Gym Basketball Goals (CLIN 1001, an option item), all quoted on a lot basis. All proposed items must meet salient characteristics specified in the Statement of Work and must include manufacturer name, model number, and supporting catalog cuts or specifications; incomplete quotes will be deemed nonresponsive.
Offerors must be actively registered in the System Award Management (SAM) database and the Wide Area Work Flow (WAWF) system. Offerors are required to participate in a mandatory site visit at B112 on 22 July 2026 at 0900 hours; submission without site visit participation is not permitted. Quotations must include company name, CAGE/UEI, prompt payment discount term, quote expiration date, delivery time, shipping method, offeror name, email, phone, and signature. The Government intends to make a single award based on price and other factors, with offers held firm for 60 calendar days from the price list date. Japanese contractors are exempt from Japanese customs duties and taxes under the status of Forces Agreement. The primary point of contact is Katsuyoshi Kodera, Contract Specialist (011-81-176-3895; katsuyoshi.kodera.jp@us.af.mil), with Michiko Yoshizawa, Contracting Officer, as alternate POC (011-81-176-3895; michiko.yoshizawa.jp@us.af.mil).
