Amendment 1 Questions & Answers
RFQ FA520519QMG09
Deployment-related Health Assessment (DRHA)/Mental Health Assessment (MHA) Provider for 35th Medical Group, Misawa AB, Japan
1. In the solicitation’s Instructions to Offerors, Section C outlines the two Factors that must be addressed in the proposal response:
TECHNICAL CAPABILITY - Limited to no more than 15 pages to include but not limited to attachments, title pages, and table of contents. Submit one (1) original and one (1) copy or original only if submitted electronically. Any attachments provided by the Contracting Office does not count toward the limit.
PRICE - Limited to no more than 4 pages to include attachments. Submit one (1) original and one
(1) copy or original only if submitted electronically. Any attachments provided by the Contracting Office does not count toward the limit.
The wording of these statements and the specificity of the page counts with attachments, implies that Technical Capabilities and Price should be submitted as separate volumes, not as two distinct chapters of a single document. Is this correct?
A. Either way is acceptable as long as they are clearly labeled.
Also, are the completed Representations and Certifications to be included in the Technical Capabilities Section/Volume, as an Attachment that is outside of the page count?
A. As stated in the provision, if your company’s reps and certs are loaded in SAM, then only paragraph (b) needs to be complete. If they have to complete them in SAM, then the Provision will not count against the page count, as this is a part of the solicitation that the government has provided.
2. As a US citizen, will our candidate need a work visa and what is the process to obtain?
A. No, US Citizens will be eligible for SOFA status. The company will need to follow the instructions of DFARS 252.225-7976 in order to complete that process.
3. Is base housing available and if not, can you advise on housing process?
A. No, housing is not available. However, there are off base options available. Please see attached pamphlets with that information. Unfortunately, the housing office cannot provide assistance for contractor personnel. There are other housing option available as well.
4. We understand that contract employees are eligible for benefits under SOFA. Does that include gas coupons for travel off base?
A. Misawa AB does not give “gas coupons”. SOFA employees do have access to the base gas station. There is no rationing of gas on Misawa AB at this time.
5. Can our physician bring a spouse and is this considered a “command sponsored” billet.
A. Yes, however, they are not considered “command sponsored”. Their information needs to be submitted with the SOFA request when submitted by the company and the dependents will receive SOFA status as well as be included in the request for logistical support.
