SUBJECT: Request for Past Performance Questionnaire
TO: Whom It May Concern
We are currently participating in a request for the Government’s Request for Quote No.
FA500024Q0014 for a firm fixed price contract for Boiler/Water Heater Maintenance at Joint Base Elmendorf-Richardson (JBER), Alaska.
To assist the Government in its assessment of our past performance, we are required to identify previous contracts and clients who are familiar with our past performance. For the purpose of this questionnaire, past performance is defined as contracts performed and/or being performed for any customer within the last five (5) years from the issuance date of the solicitation.
As one of our customers for past contracting work on , we are requesting that you complete the attached questionnaire regarding our past performance. This letter authorizes you to discuss our past performance with the Government and to release information regarding our past performance as directed.
Your prompt attention in completing the attached questionnaire will ensure we receive full consideration in this competition. Please return the completed form to the Government’s point of contacts identified below, to arrive no later than 2:00 P.M. local Alaska Time on Monday, 12 August 2024.
