PROPOSAL PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of Three (3) separate parts; Part I - Summary and Miscellaneous Data, Part II - Price Proposal and, Part III - Technical Proposal.

B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists within the parameters of FAR 15.402, no additional data will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists, offerors may be required to submit other than certified cost or pricing data IAW FAR 15.402(a)(2) to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.

C. Specific Instructions:

1. Only submit electronic copies of the proposal

2. Submit copies via email to the Government POC's in the posting.

1. PART I - SUMMARY AND MISCELLANEOUS DATA. Submit electronic copy only.

(a) The offeror shall provide a concise summary and master table of contents of the entire proposal.

(b) Complete blocks 14, 15, 16, 17 and 18 of the RFP Section A, SF1442 In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. These sections constitute the model contract.

(c) Complete block 19, 20a, 20b, and 20c of the RFP Section A, SF 1442, if any solicitation amendment(s) to the original solicitation documents was/were issued. Amendments will not be counted against the page limits.

(d) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections C through I, the offeror shall submit only those pages that require a fill-in.

(e) If applicable, provide a copy of the letter from the Small Business Administration (SBA) showing proof of Section 8(a) status.

2. PART II - PRICE PROPOSAL - Submit electronic copy only.

(a) This part shall contain a completed Section B (Supplies or Services and Prices/Costs).

(b) Insert proposed unit prices and extended amounts in Section B and attachment 8 - price schedule for each Contract Line Item Number (CLIN) or Sub CLIN (SCLIN), including all option periods. The extended amount

FA500021R00400003

must equal the unit price multiplied by the number of units. It is imperative that mathematical calculations are correct; particular care should be taken to ensure the amounts match when using a spreadsheet program, such as Microsoft EXCEL. The unit prices and extended amounts are limited to two decimal places.

(c) Extreme care should be exercised to ensure that no price data of any kind are included in any other Part of the proposal.

3. PART III - TECHNICAL PROPOSAL - Submit electronic copy only.

The factor is met when the offeror provided a proposal that identifies and meets all requirements stated in the attached SOW. Please note that this is NOT a request for a capabilities statement or business competencies. The objective of this part is to submit a technical proposal that details the contractor's plan to accomplish the work and addresses the specifics of this project's requirements. A word-for-word copy of the Statement of Work also does NOT meet this factor.

4. Format for proposal Parts I, II, and III, shall be as follows:

(a) Each written Part of the proposal shall contain a Table of Contents, Cross References, and the List of Tables and Drawings (if applicable).

(b) one electronic copy of each Part of the entire offer must be submitted. The electronic version of the proposal shall be submitted in a format readable by Adobe Acrobat portable document files (.pdf) format. The electronic version shall be submitted via email to enrique.gonzalez@us.af.mil and ryan.debonis.1@us.af.mil. USB flash drives, floppy disks, and zip disks are not acceptable.

1. Offerors are cautioned that JBER has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. Section M - Evaluation Factors for Award Miscellaneous text in this section has been modified to:

EVALUATION FACTORS FOR AWARD:

This solicitation is issued using FAR Part 15 Contracting By Negotiation. By submission of a proposal in accordance with the instructions provided in Section L, Instructions to Offerors, the Offeror accedes to the terms of this model contract. Failure to meet a requirement may result in a proposal being determined unacceptable.

Technical acceptability and past performance when combined are approximately equal to price. The contract award will be made to the Technically Acceptable, Lowest Priced with acceptable Past Performance Offeror.

The evaluation criteria are as follows:

1. Technical Acceptability Factor. The government will evaluate proposals on a pass/fail basis and assign an Acceptable or Unacceptable rating as described below:

Rating Description:

-Acceptable - Presents a thorough understanding of, and ability to meet objectives outlined in the Statement of Work. Technical narrative, drawings, and all required submittals shall be submitted with proposal. Proposal and all submittals, drawing, and technical narrative presents sufficient detail to allow the government to make an acceptability determination. Proposal meets all requirements of the solicitation.

-Unacceptable - This rating is met when the Offeror does not provide a proposal that presents a thorough understanding of, and ability to meet objectives outlined in the Statement of Work. Technical narrative, drawings and required submittals were not provided, or do not meet the specifications outlined in the solicitation. Or sufficient detail was not provided to allow the government to make an acceptability determination.

2. Price Evaluation Factor:

-The government will conduct proposal evaluations IAW the procedures at FAR 15.404-1(b)(2), however, upon examination of the initial offers and the determination that adequate price competition no longer exists, the Contracting Officer may use one or more, but not necessarily all of the evaluation techniques in FAR Part 15.404-1 to determine price reasonableness.

3. Past Performance: The government will evaluate past performance on a pass/fail basis. Quality will be evaluated based upon evaluation ratings in the Federal Past Performance Information Retrieval System (PPIRS), if available. The Government will use past performance information from Contractor Performance Assessment Reporting System (CPARS) IAW FAR Part 42.1503(g), which mandates construction contracts will evaluate CPARS information within six (6) years of the completion of performance of the evaluated contract or order and information contained in Federal Awardee Performance and Integrity Information System (FAPIIS), e.g., terminations for default or cause. Past Performance information may also be obtained from other established systems such as the Electronic Subcontract Reporting System (eSRS), or other databases; and may be obtained from other sources available to the Government, such as the Defense Contract Management Agency; and interviews with Program Managers, Contracting Officers or Fee Determining Officials.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be determined to have unknown (or "neutral") past performance. In the context of acceptable /unacceptability, a neutral rating shall be considered "acceptable".

Rating Description:

- Acceptable- Based on the offeror's performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown.

- Unacceptable- Based on the offeror's performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.

Offerors are cautioned to submit sufficient information and in the format specified in Section L of this solicitation. Offerors may be asked to clarify certain aspects of their proposals. Exchanges conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest.
