GENERAL

1. PROJECT LOCATION

1.1 All work under this project will be performed on Joint Base Elmendorf-Richardson

(JBER). Work areas and limited access areas are shown on drawings.

2. NOTIFICATION OF WORK START

2.1 The Contractor shall give at least 10 working days advance notice in writing to the

Contracting Officer at 10480 Sijan Ave, Joint Base Elmendorf-Richardson (JBER) prior to beginning any work under the scope of this project.

2.2 Coordinate with appropriate utility locating agencies, including base communications cable maintenance (Item 1 through 19) of 673rd ABW Form 3 (Base Civil Engineer

Work Clearance Request). Form 3 will be given to the contractor during the pre-construction meeting. Contractor shall complete and follow all instructions on Form 3 before start of work in accordance with Wing Instructions (WI 32-1007) “Safeguarding

Utilities From Damage”. A copy of the Wing Instructions will be available to the contractor during the pre-construction meeting.

2.3 Contractor shall schedule a meeting time to meet with each utility agency at the work site to locate affected utilities.

2.4 Contractor shall maintain all marked utilities in the field once they are initially marked by each utility agency. If markings are damaged, contractor shall request new locates and shall give the locating agency 48 hours to perform the new locates. Expenses for time and materials to perform repeat locates shall be borne by the contractor, unless otherwise approved by the Contrcating Officer. Cost to do the repeat locates will be deducted from the final invoice. Cost will be calculated using the average shop rate of

$35.00 per man-hour. In cases where utilities were marked late in the year and operations had to be shut down for winter, marked utilities would still have to be maintained by the contractor by placing stakes or transcribing the marks on a drawing.

When construction resumes, contractor shall re-mark the site and may request verification of utility locations at no additional cost to him (Off Base utility locates agencies might request additional cost for repeat locates).

2.5 Clearance request is not valid until signed by the approving officer (Block 23 of Form

3). Approving officer will not sign until all locates are marked and all utility locating agencies signatures are on the form. Excavation work shall commence within 30 days after the approving officer’s signature, otherwise clearance request is invalid and a new

01010-2 request is required. Notify Contracting Officer immediately if new utilities were installed by others and not marked after all initial locates are made.

2.6 The contractor is considered a “construction agency” for compliance with the provisions of WI 32-1007”.

3. WORK SCHEDULE

3.1 Work Hours: Work hours on this project shall be limited to Monday through Friday inclusive, between 0700 hours through 1700 hours, excluding federal holidays, unless approved otherwise in writing by the Contracting Officer. Government daily no later than 1200 hours the first working day following the date which the report covers. Where his shop drawing review and approval is accomplished away from the job site, the Contractor may provide weekly summary reports of these activities in lieu of daily reports as required for all on-site quality control activities.

26.9 In addition to the qualifications contained in paragraph 26.3 above, the Contractor's quality control organization will have the capability of performing the following required functions.

26.9.1 Check materials at the site against Contract specifications and the "approved" shop drawings, which shall be maintained at the job site for the duration of the Contract.

26.9.2 Prepare and maintain current records of all quality control operations to include those of subcontractors.

26.9.3 Review and submit all required guarantees.

26.9.4 Monitor safety program for effectiveness and conformance with the Contract requirements and furnish documentation of deficiencies noted and corrected.

26.9.5 Report to the Government any discrepancies in plans and/or specifications.

26.10 The Contractor shall submit a listing of all current construction deficiencies to the

Contracting Officer at the time he makes a request for an acceptance inspection. The

Contracting Officer will determine whether to schedule an acceptance inspection based in part upon his review of this listing for accuracy and completeness.

26.11 In addition to the Contractor's quality control program, the Government may direct or perform such additional inspections and tests as considered necessary by the

Contracting Officer to ensure proper performance.

26.12 Should recurring deficiencies as determined by the Government indicate that the quality control system is not adequate or quality control reporting is sub-standard, corrective action shall be taken as directed by the Contracting Officer. This may require the assignment of additional full time quality control personnel or the removal and replacement of ineffective personnel. If deficiencies in the work performed indicate that the Contractor's quality control system does not produce the desired results, corrective actions in both the quality control system and the work will be taken by the

Contractor. In addition, the Contractor shall not build upon or conceal any defective work. If the Contractor does not promptly make the necessary corrections, the

Contracting Officer may issue a suspension of work, suspending all or part of the work

01010-15 until satisfactory corrective action has been taken.

27.0 DD FORM 1354: The contractor shall provide a completed Form 1354, Transfer and

SCOPE OF WORK

3/13/2020

Contractors are strongly encouraged to visit the site and get familiar with all the work related to the water line construction area prior to submitting a bid, to verify existing site condition and other information as noted in the scope of work and drawings, as well as to become familiar with the scope of work. Failure to visit the site does not relieve the Contractor from the responsibility of performing all work required to provide a complete and useable project in accordance with the Contract documents.

• Intent of this project is to construct a new water line by boring or excavating underneath Fighter Drive and connecting to 2 existing lines.

1. Excavate a pit where shown on the attached site plan, expose exiting 10” AC line. Cut and install new 10X10X8 Tee, one new 10 inch and one new 8 inch gate valve. Excavated pit shall be large enough to accommodate cutting the existing line and installing the new Tee/Valve. Maximum of 5 hours allowed to cut and install Tee/Valve (not including excavation time). 10 inch ductile iron pup will be used between existing 10 inch AC pipe, 10 inch gate valve and 10 inch tee.

2. Bore a new 8 inch HDPE SDR 11 water line underneath Fighter Drive to meet the existing 6 inch AC line across from the Chapel (Bldg. 7137). Horizontal ground distance is approximately 120 feet, but needs to be confirmed by contractor after reviewing the attached GeoBase Utility Map and on site conditions. New water line inside diameter must be a minimum of 6 inches.

3. Excavate and cut existing 6 inch AC pipe and connect new bored 8 inch HDPE SDR11 line to it.

4. Backfill pits and compact to prevent settlements in the spring. Backfill and compact at sidewalks and pavement sections to prevent settlements. Place 6 inch concrete sidewalks, concrete curb and gutters and 3 inch AC pavement.

5. At the contractor's option, the work may be accomplished by excavating the area required to perform the repair. Under this option all pipe parts will be ductile iron, and the contractor will be responsible for putting in place the necessary traffic control to direct traffic around the work site. Any dewatering required under this method will be the contractor's responsibility.

6. Water line isolation will be performed by the 773CES Water Shop.

7. For the water outage, Contractor shall give 5-days advance notice of intent to start work so occupants affected with water outage can be notified.

8. Traffic control plan with dates and alternate routing must be submitted 21 days prior to start of actual work to allow for proper processing.

9. JBER Work Clearance Permit (Dig permit) is required.

10. A Storm Water Pollution Prevention Plan (SWPPP) is not required for this project; however, best management practice must be followed to prevent sediments and other material from falling into the surrounding storm system inlets.

11. Water line work includes butt-fusing, fittings, flanges, reducers, tees, thrust blocks, restrains, dissimilar metals couplings, disinfecting, etc. to complete the installation of water lines, valves and to connect to existing lines.

12. If boring method is used, new water lines shall be High Density Polyethylene (HDPE) pipe, PE4710 polyethylene compounds that meet or exceed ASTM D3350 Cell Classification 445574; DR 11.0.

Connect HDPE to existing AC pipes with appropriate (HDPE manufacturer recommended) fittings.

13. All removed asphalt, curb/gutter and sidewalks in areas of excavation shall be replaced with new.

New pavement cross section shall be as follows: 3inches AC, 6 inches D-1 Base course in all areas where trench cuts into existing pavement. ADA compliant sidewalk ramps will be required where those existing sidewalk features are disturbed.

14. Contractor shall be aware that the existing pipe being replaced is Asbestos Cement (AC):

1. If the contractor cuts or damages the pipe while excavating and removal, he shall follow the asbestos abatement procedures. Where practicable the whole section of AC pipe should be removed without cutting or breaking at joints.

2. Work practices must be used that will minimize dust generation and/or dispersal. Any asbestos products to be cut must be thoroughly wetted down and cut with a low speed hand tool (e.g. hand saw). Upon completion, the entire work area of the task must be cleaned of all asbestos type waste. As many broken pieces as practicable shall be collected. Waste pieces are to be double bagged in heavy-duty plastic bags, which are to be labeled “ASBESTOS - FOR DISPOSAL”. All disposable clothing, respirators, respirator cartridges must also be included in these bags.
