Contract Type Enter “C” Type, IDIQ/BPA or Task Order, etc.

**If ID/IQ or BPA, offerors are permitted to submit the ID/IQ/BPA and ONE task/delivery order/call thereunder IAW L-6.1.1.2, which will be counted as one contract reference. Although considered one contract reference, the offeror must submit separate documents (PPI information sheet, consent letter, PPQ) for (a) the IDIQ and (b) the order thereunder.

As the single contract reference, the Government will evaluate only the ONE submitted order/call under the IDIQ/BPA and is under no obligation to conduct evaluation on other and all orders/calls under the IDIQ. PAST PERFORMANCE QUESTIONNAIRE

Offeror Instructions: Complete Transmittal Letter and Section 1 of the Past Performance Questionnaire (PPQ) and send to the assessor(s). Ensure assessor returns a copy of the completed questionnaire to the POCs identified below prior to the solicitation closing date and time.

Assessor Instructions: Confirm accuracy and completeness of Section 1 of the PPQ. Complete Section 2 of the PPQ and submit Sections 1 and 2 directly to the Contracting Officers. Do not forward a copy of the completed questionnaire to the vendor whose performance you are evaluating. To be considered, the information must be received by the Government prior to the solicitation closing date and time.
