Below is a record of questions received in response to solicitation FA4890-16-R-0004 and respective answers provided by the government.
Question: In the Instructions to Offerors, Instructions for Volume 1, Past Performance Information Para 9.5, E.1. Offeror’s Past and Present Performance Contract References: It says;
“Submit one (1) sheet for each contract reference to include an overview of the work performed, what capacity they serve under the reference(i.e. prime contractor, subcontractor, partner, etc.), offeror’s assessment of how the contract reference is relevant to the Relevancy Criteria, and any additional significant details.” Please clarify that one (1) sheet is equivalent to one piece of paper as defined in 7. Proposal Preparation Instructions, 7.3 Proposal Format, d. Paper size: 8 1/2” x 11” with background color of white or ivory stock only?
Answer: YES
Question: Please clarify/confirm the government’s intent/expectation for offerors IAW 9.5 E.1 of
PPI Contents. Offeror is to submit only one (1) sheet/page for each contract reference in the form of an overview only of the work performed regarding the contract reference submitted as part of the bidder’s proposal package?
Answer: YES
Question: 1.4 – “Offerors shall acknowledge awareness of all amendments to the solicitation by signing and dating page 1 of each Standard Form 30, Amendment of Solicitation/Modification of Contract, and including it in Volume 3, Price of the Proposal.” There have been no Standard Form 30’s issued for Amendments to this Solicitation, how would you prefer acknowledgement be completed for the amendments issued?
Answer: A statement of acknowledgement with the list of Amendments issued in a separate sheet of paper will suffice.
Question: 3.1 - “The original proposal and copies as specified in this document shall be sent to the issuing office listed in Block 9 of Standard Form (SF) 1449; Solicitation, /Contract/Order for Commercial Items; on or before the date and time specified in Block 8 of SF 1449….”
There is no SF 1449 used for this solicitation since the format of Subpart 12.6 is being utilized. Will the government be requiring a signed cover page inserted in the price volume as a replacement to the SF 1449?
Answer: No. A signed page will not be required.
