STATEMENT OF WORK

Building 338 Asbestos Abatement with Final Clearance

Luke Air Force Base, Arizona 24 August 2020

1 OVERVIEW

1.1 The purpose of this project is to remove approximately 1,300 square feet of asbestos containing material (ACM) including tile flooring, carpet, baseboards and glue in the areas marked on the provided floorplan diagram. Note that ceramic tile in the restrooms is not included with this project.

1.2 The contractor will remove all existing floor tile, carpet and adhesives; abate asbestos in the area;

grind and polish concrete surface to a smooth, polished finish; and apply sealant to the finished polished concrete floor. No asbestos under the walls will be abated. This work will be performed during normal business hours at a date TBD upon award of contract.

1.3 No updated asbestos survey will be provided, nor is one requested under this statement of work (SOW). The Government has confidence in the 2002 Asbestos Survey Report.

2 DESCRIPTION OF WORK

2.1 This project includes all necessary demolition, construction and testing to perform work as listed in description of work below.

2.2 Performance Period: This work should be completed within 15 calendar days after the Notice to

Proceed is issued.

2.3 Hazardous Material Abatement: Asbestos is present. An Asbestos Report is available for review.

Although testing was accomplished, the Contractor shall report any additional suspicious material immediately to the Contracting Officer.

2.4 Design Drawings: None required.

2.5 Calculations: None required.

2.6 Survey: None required.

2.7 Asbestos Abatement:

2.7.1 The contractor shall safely remove old carpet, tile, baseboards and adhesives and pile under the mil work. Removal shall be done by qualified asbestos abatement contractor in proper containment.

2.7.2 The contractor will remove carpet, tile and glue, repair cracks and divots in the floor as necessary, grind and polish the bare concrete floor to a “salt and pepper” finish, and apply appropriate concrete floor sealant.

2.7.3 All asbestos encountered shall be properly abated. The Government anticipates one containment will need to be constructed for this building.

2.7.4 Abatement shall be performed utilizing Environmental Protection Agency’s National

Emissions Standards for Hazardous Air Pollutants (NESHAP). Contractor's work must pass the AHERA TEM clearance test upon abatement completion.

2.7.5 Contractor will employ a third-party to conduct final air sampling and clearance test after abatement.

3 GOVERNMENT FURNISHED INFORMATION, PROPERTY, OR SERVICES

3.1 Upon the Contractor’s request, the Government will provide electronic and/or hard copies of available documents and correspondences pertinent to the work.

3.2 Adequate power to support contractor tools shall be provided by the government in the form of commercial power from the facility or government supplied generator in the event facility power is inadequate.

3.3 All furnishings and non-permanent fixtures shall be removed by government personnel prior to contractor’s commencement of work.

4 GENERAL INFORMATION

4.1 The Contractor shall furnish all materials, equipment, and personnel necessary to design, manage, and accomplish the project. The Contractor shall thoroughly review the request for proposals for constructability and advise the Contracting Officer of potential problem areas prior to submitting a cost proposal. No alterations, additions or deletions shall be made unless prior written approval is given by the Contracting Officer. Under no circumstances is the Contractor to begin any work on the project until a Notice to Proceed is issued by the Contracting Officer.

4.2 Site Visit

4.2.1 The Contractor shall inspect the site where services are to be performed. This site review shall satisfy the Contractor’s need to identify all general and local conditions that may affect the cost of the contract.
