1. In Section 8 on Standard Form 1442, it is left blank. Who are we to address the offer to?
The offer may be addressed to the Contracting Officer:

2. In Section 11 on Standard Form 1442, it says “The contractor shall begin performance within 30 calendar days” and this statement conflicts with what is stated on Page 35, 52.211-10, “The Contractor shall be required to (a) commence work under this contract within 10[Contracting Officer insert number] calendar days”. Which statement is correct?
The Solicitation has been amended to reflect the correct timeline – The contractor shall begin performance within 10 calendar days of the issuance of the Notice to Proceed.
3. On Page 22, (h)(1), asks for a contract number to go with the NAICS Code. What is the contract number for us to use to fill this out?
FAR 52.219-28(h) is only applicable if the offeror does not have Representations and Certifications completed in SAM.gov for the NAICS Code Applicable to this contract: 237310
The Solicitation Number, FA485521R0005 may be used as the Contract Number.
4. On Page 77, (k)(1) and (k)(2) states the contracting officer is to indicate if these are applicable. I do not see where they are checked as applicable. Are items (k)(1) and (k)(2) applicable to this bid?
FAR 52.212-3(k)(1) and FAR 52.212-3(k)(2) are not checked – they are not applicable to this Solicitation.
5. I would like to have clarified: Amendment #1 seems to delete the requirement for two copies of our proposal. Is this correct or are we to turn in one original of our proposal and 2 copies of the proposal?
Amendment FA485521R00050001 removed the requirement to submit two sealed copies (as this requirement is being solicited as a Request for Bid, not a Sealed Bid).
