1. DESCRIPTION OF PROPOSED WORK: The scope of work covered by the specifications and drawings consists of providing all engineering, labor, materials, equipment, supervision and effort necessary to replace the fire access road between dorms 1159 and 1161 at Cannon Air Force Base, New Mexico, in accordance with the current Unified Facilities Criteria (UFC). The project, Repair Fire Access Road, Dorms, will provide a full depth replacement of the fire access road with a layout that is compatible with current firefighting vehicles. Refer to Specification Section 01 11 00 Summary of Work for additional Information.
1.1 Period of Performance: The Period of Performance (POP) duration is 120 calendar days.
2. GENERAL REQUIREMENTS: The Contractor shall perform all the tasks necessary to deliver the requirement in compliance with the Specifications and Drawings attached to the contract. The Contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting the specifications provided.
2.1 Qualified Personnel: The Contractor shall ensure that only qualified, competent personnel carry out the tasks outlined in Specifications. Competent is defined as registered professional or, where registration is not applicable, trained and/or certified in their respective field. The Contractor shall have 48 hours to replace key personnel on the project: Project Manager, Site Superintendent, Site Safety and Health Officer, and the Quality Control Manager.
2.2 Construction Requirement: The project shall conform to all applicable Federal, State and local laws, regulations and base-specific standards and specifications. Such decisions will be coordinated with 27th Special Operations Contracting Squadron (27 SOCONS). All work accomplished by the Contractor shall comply with these criteria unless a waiver is obtained from the United States Air Force (USAF) when conditions warrant such consideration.
2.3 Security: Refer to Specification Section 01 14 00 Work Restrictions for this project. The Contractor shall provide personnel who meet security requirements and can gain and maintain access to the base for the duration of the project.
2.4 Hazardous Materials: The Contractor shall comply with Specification Section 01 74 19 for requirements.
2.5 Environmental Requirements: The Contractor shall comply with all relevant and applicable Federal, State, and local environmental laws, regulations, and executive orders, and base regulations as documented in 27 SOCES Environmental Specification section 01 57 19 which outlines provisions to help ensure Contractor compliance with environmental law.
2.6 Proposal: Requirements for the Contractor’s proposal can be found in the Request for Proposal (RFP). At a minimum the Contractor’s proposal shall include:
a) A preliminary project schedule (Gantt chart) showing major project milestones and the critical path

b) A project narrative supporting the preliminary project schedule
c) Past performance data (3-5 recent/relevant projects of similar scope and magnitude)
d) Additional guidance and requirements may be listed in the RFP
3. ADMINISTRATIVE REQUIREMENTS: The Contractor shall identify a lead Program Manager and perform all program management functions and tasks necessary to satisfy the requirements consistent with operational quality, safety, security, and environmental standards as required in Specification Sections 01 30 00 Administrative Requirements, 01 35 26 Government Safety Requirements and 01 45 00.10 Quality Control for Minor Construction. The program management function of this contract shall provide insight to the Government for all technical and programmatic activities performed under this contract.
3.1 On-Site Supervision: The Contractor shall provide the manpower, equipment, materials, services, and transportation necessary to ensure oversight services throughout accomplishment of this contract. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract and as required in Specification Sections 01 30 00 Administrative Requirements, 01 35 26 Government Safety Requirements and 01 45 00.10 Quality Control for Minor Construction.
3.2 Staffing: The Contractor shall provide the manpower, equipment, materials, services, and transportation necessary to execute the project requirements in accordance with the period of performance. Cannon AFB is located in a sparse location where the labor force is limited.
4. WORK PLANNING/COORDINATION
4.1 Project Schedule: Within fourteen (14) calendar days of contract award the Contractor shall submit a project schedule showing all long lead material procurement timelines, major project milestones, definable features of work and shall clearly identify the critical path. The contractor shall submit a gantt chart along with an AF3064.
4.2 Contractor Operational Hours: The Contractor shall establish a work schedule, which requires work to be performed from 7:00AM to 4:00PM, Monday through Friday. The Contractor shall provide to the CO or COR in writing any request to work additional hours (overtime), a minimum of 48 hours prior to the requested time interval. Additional hours must be approved prior to work commencement. Refer to Specification Section 01 01 00 General Requirements for additional information.
4.3 Verification of Existing Conditions and Site Survey: Use of existing condition data provided by the USAF conveys acceptance and as such does not relieve the Contractor of liability associated with performance as the Engineer of Record. The Contractor shall be solely responsible for verification and validation of existing conditions, coordination of existing conditions in parallel with proposed requirements, and above and below-grade condition assessments. The Contractor shall visit the premises and take into consideration all conditions which might affect the work. No consideration will be given to any claims based on lack of knowledge of existing conditions.

4.4 Pre-Construction Site Survey: Prior to the start of any on-site construction activities, the Contractor shall:
a). Conduct a project site condition survey with the 27 SOCES Construction Manager
(CM).
b). The Contractor shall note the condition of trees, shrubs, grassed areas, concrete, asphalt, and any structural damage that may exist immediately within or adjacent to the work site. Existing infrastructure that is not part of the project shall be maintained in working order. If damage occurs, it is the responsibility of the Contractor to restore the infrastructure to equal or better condition than it was prior to the damage. If the Contractor fails to note conditions, the government shall direct the contractor’s repairs and may require a full removal and replacement of the damaged infrastructure by qualified personnel.
c). The Contractor shall field verify all dimensions or examine covered connections before ordering materials or performing any work.
4.5 Project Meetings: Contractor shall at a minimum hold meetings as required in Specification Section 01 30 00 Administrative Requirements and as noted below.
Pre-Construction Meeting: A pre-construction meeting will be held at the Contracting conference rooms within two (2) weeks after the contract award.
Kick Off Meeting: See Federal Acquisition’s Regulations (FAR) 52.236-12
Red Zone Meeting: Red Zone meeting shall be held at 80% project completion or within 45 days of Beneficial Occupancy.
5. CONSTRUCTION:
Upon the approval of the Contracting Officer (CO) and written notice to proceed (NTP), the Contractor may commence on-site construction activities. The Contractor shall complete the work as described below:
5.1 Staging Area: The Government will identify space on the jobsite or in a base Contractor staging area where the Contractor may deliver and store non-hazardous materials within the installation.
5.2 Design and Specifications: The Government will provide the Contractor access to the 35% design drawings and specifications including any available existing engineering plans, as-built drawings, diagrams, aerial photographs, digitized map files, etc. to facilitate evaluation of the site/building, as needed. Use of any such available existing engineering plans, as-built drawings, diagrams and other data provided by the USAF does not relieve the Contractor of his/her responsibility for field measurement, verification and validation of existing conditions in the effort of developing their own necessary design/construction drawings for this project.
a) Basis of Design: Any brand name items listed in this SOW and design drawings are intended as a Basis of Design (BOD). The Contractor may substitute a product, or
Last updated 25Nov2020 products, for the brand name items as long as the substituted product(s) meet the minimum performance standards established by the listed salient characteristics. Refer to Specification Section 01 01 00 General Requirements. An ‘Or Equal’ product shall be submitted with the initial proposal and will be reviewed for technical acceptability.
5.3 Permitting/Utility Coordination: The Contractor shall provide utility coordination prior to starting excavation work or related utilities work associated with this project, per Specification Section 01 01 00 General Requirements.
a) Dig Permits: The Contractor shall submit the standard AF Form 103 Work Clearance Request (i.e. Dig Permit) required prior to starting any excavation work or related utilities work for the project. A signed AF Form 103 is required prior to any work performed on the site. The AF Form 103 will be furnished to the Contractor upon award.
b) Utility Outages: For the duration of the project, Contractor shall provide information to the 27 SOCES CM for coordination of outages thirty-five (35) calendar days in advance.
c) Coordinate work schedules with the 27 SOCES CM at least seven (7) calendar days in advance.
5.4 Environmental Requirements: The Contractor shall adhere to 01 57 19 Temporary Environmental Controls. Unless otherwise approved by the government, all project waste shall be disposed of off-site, at a location that meets federal, state, local and installation requirements. Any hazardous materials to be used shall be submitted and approved prior to transportation and use on the installation. If the contractor intends to discharge any waste (ex. wastewater) the contractor shall adhere to 01 57 19 and obtain government approval via a discharge permit prior to the discharge. All resultant waste shall be disposed of at the expense of the contractor.
5.5 Scope Of Work
Work shall conform to the requirements outlined by the design drawings, specifications and this SOW. This project is a full depth replacement of the fire access road with a new layout.
a) The Contractor shall accomplish the following pre-construction actions:
5.5.a.1 Pre-construction site visit/survey 5.5.a.1.1 Note the condition of existing infrastructure (both part of the project and not part of the project) 5.5.a.1.2 Field verify existing conditions and dimensions 5.5.a.2 Utility detection 5.5.a.3 Permitting/work approval 5.5.a.4 Submit and receive approval of required submittals according to the submittal register, specifications, SOMS and this SOW.
b) The Contractor shall accomplish the following demolition as indicated by the
Government provided drawings:
5.5.b.1 Curbs and gutters 5.5.b.2 Sidewalks 5.5.b.3 Existing paved surface 5.5.b.4 Excavate as required 5.5.b.5 All waste shall be disposed of off site

c) The Contractor shall accomplish the following construction as indicated by the Government provided drawings:
5.5.c.1 Backfill, grade and compact sub base and base course to meet requirements of the specifications and drawings
5.5.c.1.1 Rigid pavements (concrete) 5.5.c.1.2 Curbs and gutters 5.5.c.1.3 Sidewalks 5.5.c.1.4 Dumpster container area (foundation and surface) – design load for dumpster container area shall be a garbage truck 5.5.c.2 Walled dumpster container area 5.5.c.2.1 Exterior shall conform to CAFB architectural standards 5.5.c.3 Flexible pavements 5.5.c.3.1 Surface the fire lane with hot mix asphalt (HMA) as indicated by the drawings 5.5.c.3.2 NMDOT asphalt mix preferred 5.5.c.4 Pavement marking 5.5.c.4.1 Mark the curbs, gutters and asphalt surfaces appropriately in accordance with project specifications and drawings
d) Prior to project closeout the Contractor shall accomplish the following clean up:
5.5.d.1.1 Remove and dispose of all resultant project waste off-site 5.5.d.1.2 Street sweeping (as required) 5.5.d.1.3 Removal of all materials/equipment from laydown yard 5.5.d.1.4 Return landscape to original condition matching surrounding area
6. DELIVERABLES/SUBMITTALS:
6.1 Submittal Register: A draft submittal register is provided for this project included within Specifications Section 01 33 00A Submittal Register. Provide submittals and maintain the Submittal Register throughout project construction POP, as required by the Specifications Section 01 33 00, Submittal Procedures. This list of submittals is not all-inclusive; other submittals and forms may be required by the Contract documents based upon the means and methods chosen by the construction Contractor. Contractor shall submit the Master Submittal Register to the Contracting Officer within fourteen (14) calendar days after the NTP.
6.2 Contract Progress Schedule (AF 3064): The Contractor shall submit a signed AF 3064, Contract Progress Schedule (Appendix C2), depicting an overall contract progress schedule for the main work elements for the period of performance. The progress schedule shall be signed by either the COR or the 27 SOCES CM recommending approval, and then approved by the Contracting Officer. The Contractor shall submit an updated AF 3064, when applicable (i.e., additional work, time extension) for approval. Refer to Specifications for additional submittal information.
6.3 Contract Progress Report (AF 3065): The Contractor shall submit AF 3065, Contract Progress Report (Appendix C3). This report shall be signed by the Contractor’s on-site representative. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065 via submission of a AF 3000 submittal. An appropriate percentage will be bilaterally approved, Last updated 25Nov2020 applied, and the item must be received and stored on site before it is paid for. The Contractor shall obtain either the COR or the 27 SOCES CM acceptance by signature of the actual reported progress prior to submission to the Contracting Officer. The report shall also include the following:
a). Current Project Status b). Expected work activities for the following month c). Problems and/or concerns d). Updated Submittal Register (Specification Section 01 33 00A)
Each invoice shall be accompanied by an AF 3065 signed by the COR or the 27 SOCES CM. The AF 3065 and invoice shall all cover the same time period. Refer to Specifications Section 01 33 00 Submittal Procedures for additional submittal information.
Note: If the COR does not agree that the progress is as represented by the Contractor they shall return the documents to the Contractor for correction.
6.4 Format for Deliverables: An electronic copy of all deliverables shall be provided, formatted for use on the latest version and compatible with the Government’s version of Microsoft Word, Excel, Adobe Acrobat and, if applicable, AutoCAD. All deliverables shall be dated.
Samples may be provided to the Contractor upon request. Refer to Specifications Section 01 33 00 Submittal Procedures for additional submittal information.
6.5 As-Built Drawings: The construction Contractor shall ensure their as-built CAD (computer-aided design) and GIS (geographic information systems) submittal is compliant with the latest CAFB CAD/GIS standards. CAFB CAD/GIS standard template files are available upon request. Contractors shall include in their proposal a line item that shows the cost for generating as-built drawings to meet CAFB standards.
6.6 Geographic Information Systems (GIS): All new facilities and utilities shall be located by the Contractor in an electronic format that is compatible with the current base GIS software and includes descriptive information required by the USAF. In such cases, the Contractor is required to assist USAF agents or personnel in the generation of GIS information if requested by the 27 SOCES PM. Subject to change, the following are the basic requirements for GIS information to be delivered to the Air Force on CD:
a) DATUM – WGS84
b) Coordinate System – UTM
c) Units – Meters
d) Data in attribute Table will be SDSFIE version 2.6 Compliant
e) Shape file Geodatabase format
6.7 Government Review: For all submittals and deliverables requiring Government review, the Contractor shall allow fourteen (14) calendar days for Government processing and review.
7. PROJECT CLOSEOUT: The project closeout documents required below shall be submitted to and accepted by the CO prior to final payment.

7.1 General Requirements: The Contractor shall perform all closeout requirements as shown in Specification Sections 01 78 00 Closeout Submittals and 01 78 23 Operations and Maintenance Data.
7.2 As-Built Drawings: The Contractor shall submit as-built drawings prior to final payment.
The as-built drawings shall be submitted to the Government within seven (7) calendar days of final inspection.
7.3 DD1354: The Contractor shall submit a final DD1354 within seven (7) calendar days of final inspection.
7.4 Warranty Information: The Contractor shall provide a one (1) year warranty on labor and materials in addition to manufacturer warranties. The warranty shall be issued in accordance with FAR 52.246-21. The warranty data shall be submitted to the Government within seven (7) calendar days of final inspection.
7.5 O&M Data: The Contractor shall submit one (1) hard copy and one (1) electronic copy containing operations and maintenance information prior to project closeout. If required by the Government, the Contractor shall provide O&M training on the real property/equipment installed under the project. The O&M data shall be submitted to the Government within seven (7) calendar days of final inspection.
7.6 Final Inspection: The Contractor shall conduct a Final Inspection with Base personnel and publish the findings in a Final Inspection Report. The contractor shall publish the Pre-Final Inspection findings in a Pre-Final Inspection (Punch List) Report. All punchlist items shall be resolved prior to acceptance. At the Final Inspection, the Contractor shall present a completed DD Form 1354 to the 27 SOCES CM for signature and acceptance by the Base Civil Engineer (BCE). Refer to Specifications Section 01 78 00 Closeout Submittals for additional closeout information.
8. CONTRACT ADMINISTRATION/POINTS OF CONTACT:
8.1 Contracting Officer: Notwithstanding any of the provisions of this contract, the CO shall be the only individual authorized to amend or modify the terms of this contract in any manner.
8.2 Contracting Officer’s Representative (COR): Upon contract award, the Government will assign a COR who will expeditiously review and certify the Contractor’s progress by signing AF Form(s) 3064 and 3065, submittals and monthly Status reports.
8.3 Construction Manager (CM): Upon contract award, the Government will assign a CM who will conduct routine site visits/site inspections and will review and certify submittals. In most cases the CM will also act as the COR.
