ADDENDUM TO FAR PROVISION 52.212-1---INSTRUCTIONS TO OFFERORS—
COMMERCIAL ITEMS (JUN 2008)
I. SPECIFIC INSTRUCTIONS:
A. Purpose: The purpose of these instructions is to prescribe the format of proposal and describe the approach for the development and presentation of proposal data. They are designed to ensure the submission of information essential to the understanding and comprehensive validation of proposals.
Offerors are cautioned to follow the detailed instructions fully and carefully, as the Government intends to make an award based on the initial offers received, without discussion of such offers.
B. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:
i. Requirements of the RFP Contract Line Items (CLINs) and Performance Work Statement (PWS), and
Government standards, instructions and regulations pertaining to the Performance Work Statement.
ii. The proposal shall consist of two separate and detachable submissions as listed below.
iii. Limitations on the number of proposal pages are as follows:
Proposal Page Limitations:
Past Performance (Part I): Maximum 25 pages.
Price (Part II): The Cost/Price proposal is not page limited; however, the Cost/Price proposal shall be used for only price information.
Pages exceeding the page limitations set forth in this Section will not be read nor evaluated, but will be removed from the proposal.
C. Format for proposal parts I and II shall be as follows:
i. Page is defined as one face of an 8 ½” x 11” sheet of paper containing information.
ii. Typing shall not be less than 10-pitch, Times New Roman font.
iii. Elaborate formats, bindings or color presentations are not desired or required.
iv. Submit original and one (1) paper copies, the 1 copy to be a redacted* copy, and 1 complete electronic copy in Microsoft Word format (PDF files are not acceptable).
*Note: Redacted copy shall be submitted in the same manner as paper copies. Information to be redacted should at a minimum include all references to the offeror’s company, i.e. company names and company logos/symbols. A redacted Part II (Price) is not required.
II. PROPOSAL PREPARATION INSTRUCTIONS:
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two (2) separate parts; Part I – Past Performance Information, Part II - Price
Proposal.
B. Part II – PAST PERFORMANCE INFORMATION- Submit as stated in Section I, Area C.
Only references for same or similar type contracts are desired.
i. Past Performance Subfactors. The following four past performance subfactors will be evaluated in descending order of importance. In the case of an offeror without a record of recent and relevant past performance or for whom past performance information is not available, the offeror will receive an
"unknown confidence" assessment rating.
a. Subfactor 1 – Schedule
b. Subfactor 2 – Quality of Service
c. Subfactor 3 – Management
d. Subfactor 4 – Business Relations
ii. Additional Data for Past Performance Evaluation. As part of past performance submission, offerors are encouraged to submit the following awards/recognition:
a. Professional/industry awards
b. Letters of appreciation, recognition, or commendation
c. Unique skills or accomplishments
iii. Performance Questionnaire: The government will evaluate the quality and extent of offerors performance deemed recent and relevant to the requirements of this Request for Proposal (RFP). The government will use information submitted by the offeror and other sources such as other federal government offices and commercial sources, to assess performance. Information should be both recent and relevant to be considered.
a. Questionnaires. The offeror shall send out the Past Performance Evaluation Questionnaire
(Attachment 7) to each of their past performance references. The offerors shall ensure that at least three (3) references complete the Past Performance Questionnaire. The responsibility to send out the Past Performance Questionnaire rests solely with the offeror. The questionnaire shall be used by the offeror in obtaining and providing past performance information. Once the questionnaires are completed by your references, the information contained therein shall be considered sensitive and not released outside of the government. Past Performance questionnaires shall be sent directly, from your reference, to this office to the attention of Margaret Merrell, Elizabeth Knight, and Linda Kyle via fax at 575-784-1147 or e-mail to margaret.merrell@cannon.af.mil , beth.knight@cannon.af.mil and linda.kyle@cannon.af.mil , please call before faxing questionnaires; 575-784-4305, 575-784-4312 and 575-784-2880 respectively.
b. Offerors must ensure that the questionnaires are sent to ALL references and completed within sufficient time. It is the sole responsibility of the offeror to track submittal of performance questionnaires and ensure that the government receives them no later than the proposal receipt date established in this solicitation.
iv. Additionally, offerors shall complete and provide with their proposal the Past and Present Performance
References information (Attachment 6). Submit information on no more than five (5) recent contracts performed for federal agencies and commercial customers. Include rationale supporting your assertion that the contract is recent and relevant and identify aspects of the contracts deemed recent and relevant and how they relate to the proposed effort. Past performance information must reflect recent and relevant experience.
v. Recent and Relevant. As noted in the PWS, this effort involves many facets of basewide laundry and dry cleaning services. For definitions on what constitutes recent and relevant, please refer to the tables mailto:margaret.merrell@cannon.af.mil mailto:beth.knight@cannon.af.mil mailto:linda.kyle@cannon.af.mil in the Addendum to FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), Attachment 5. Each prospective offeror has the responsibility to provide references that are recent and relevant to the new work and must explain the relevance of its past performance information submitted. Offerors are encouraged, for each reference, to identify how its work relates to each of the performance objectives identified in Section 2, “Services Summary”, of the PWS.
vi. Subcontract consent: Past performance information pertaining to a subcontract cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
C. PART II – PRICE PROPOSAL - Submit as stated in Section I, Area C.
i. Provide a firm fixed price (FFP) proposal in accordance with the Contract Line Items to include base year and all options. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness of the price.
III. PROPOSAL SUBMISSION:
Care should be exercised to ensure that past performance volume of the proposal does not include any estimated price data. The Price Data section of the proposal shall include only the following:
A. Standard Form 1449, PROPOSAL MUST BE SIGNED BY AN AUTHORIZED COMPANY OFFICIAL
AND BE RECEIVED AT THE FOLLOWING ADDRESS no later than 2:00 PM (Mountain Daylight
Time) on 09 April 2010 to the following address:

B. Price Schedule. Complete all blanks for unit & total prices in the Price List, Attachment 2. Submit pricing information for each CLIN item and RECAP TOTALS on a CD-ROM.
C. Representation and Certification. Complete all required fill-ins and certifications returning them in their entirety.
D. Provisions with Fill-ins. Complete all required provisions with fill-ins and return in their entirety.
E. Amendments. If issued, any solicitation amendments to the original solicitation documents, signed by the same official as noted above in III A.
