About this file
This is a Past Performance Questionnaire template for the Blakeslee Housekeeping Services contract (FA480926R0006) issued by the Department of the Air Force Air Combat Command. The questionnaire is designed to be completed by contracting officers or government officials who have worked with contractors on past similar projects to evaluate their suitability for the current solicitation.
The questionnaire requires respondents to rate contractor performance across four primary categories: Quality Control, Timely Performance, Management Effectiveness, and Customer Satisfaction Overall. Each category contains multiple evaluation criteria using a six-point scale (6=Exceptional, 5=Very Good, 4=Satisfactory, 3=Neutral, 2=Marginal, 1=Unsatisfactory, N/A=Not Applicable), with space for detailed comments and narratives. The Quality Control section assesses workmanship, quality control plans, deficiency correction, and overall quality outcomes. Timely Performance evaluates problem resolution speed, process flexibility, contract requirement adherence, and documentation submission timeliness. Management Effectiveness examines manager/supervisor qualifications, problem-solving cooperation, proposal accuracy, cost reasonableness, compliance oversight, management authority and site presence, and staff cooperation. The Customer Satisfaction section inquires about service duration, overall satisfaction, requirement fulfillment, service adequacy, and whether the respondent would contract with the vendor again. This questionnaire serves as source selection sensitive material and is intended to support evaluation of past performance references submitted by bidders for the housekeeping services opportunity.
