SUBJECT: Past Performance Questionnaire; FA4690-24-R-0005

Questionnaire on Contractor’s Present and Past Performance

Request you provide the following information on the contractor’s present and past performance by placing an X in the space provided by the most appropriate response. If you wish to elaborate on your answer, please provide comments on the lines provided after each question. If more space is needed, continue your comments on a separate sheet of paper and attach it to this questionnaire prior to submittal to the AFFSC. You are asked to provide frank, concise comments regarding your overall assessment of the contractor’s performance on the contract identified. You are urged to supplement your own knowledge of the contractor’s performance with the judgment of others in your organization. The following definitions are applicable rating levels of the contractor’s performance on the identified program.

PAST PERFORMANCE INFORMATION FORM

Air Force Financial Service Center, Ellsworth AFB, South Dakota, is in the process of selecting a contractor for the financial technician support. One of the considerations in proposal evaluation is the verification of the Offeror’s past and present performance on contracts, which reflect the Offeror’s ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an Offeror, for the evaluation of the Offeror’s performance on those contracts.

Please complete this form and providing comments wherever applicable. Please sign the document in the space provided. Please note we may contact the Offeror on problem areas or concerns identified to see what corrective actions have been taken or will be taken. However, the individual’s name completing the questionnaire will not be disclosed to the Offeror.

Please email the completed form as soon as possible but no later than 1630 MDT 7 August 2024. DO

NOT RETURN OR EMAIL THE COMPLETED QUESTIONNAIRE BACK TO THE CONTRACTOR.
