Instructions to Offerors 52.212-1 Addendum L-1 Addendum to FAR 52.212-1(c), Period for acceptance of offers.
The paragraph is tailored as follows: "The offeror agrees to hold prices in its offer firm for 90 calendar days from the date specified in Block 8 of the SF1449 solicitation."
L-2 General Proposal Instructions A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. A complete proposal consists of two (2) parts:

Failure to furnish a complete proposal may result in the proposal being determined unacceptable by the Government. Response to this solicitation must be submitted by the due date specified in Block 8 of the SF1449.
B. Response to this solicitation must be submitted by the due date specified in Block 8 of the SF1449. The SF1449 must bear the signature or electronic certification of an official in the company authorized to bind the firm. The signed SF1449 document must be submitted electronically to all individuals listed below. Receipt of your electronic submittal will be acknowledged via return email; if you do not receive an acknowledgement, you are advised to call for confirmation as firewall protections may block emails and/or strip attachments. D. System for Award Management (SAM) Registration. In order to eligible for award, offerors must be registered in SAM and shall complete Representations and Certifications in the system: https://www.sam.gov/. (Reference FAR Provision 52.204-7, System for Award Management). Content of the letter and mailing address may be found on the sam.gov website.
L-3 Specific Instructions Part 1, Price Proposal A. Price Proposal. Offeror is asked not to alter the electronic version of the Request for Quote (RFQ) other than to fill in the required information as outlined below. In particular, offeror is asked not to delete page breaks or to alter headers/footers.
(1) SF 1449, Page 1. Complete the items listed below. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation, to include attachments. The solicitation constitutes the model contract.

B. Price Schedule. The periods of performance and applicable contract line item numbers (CLINs) are reflected below.

For each contract line item number (CLIN), insert proposed unit price and extended amount. The extended amount must equal the quantity multiplied by the unit price. To prevent rounding issues, unit prices are limited to two (2) decimal places and no rounding of the extended amount is allowed.
Note: It is the responsibility of each offeror to verify their calculations.
C. Price Breakdown. In addition to completing the price schedule as described above, the offeror shall submit a cost breakdown reflecting labor categories, labor hours per category, labor rates per category, fringe benefits, other direct costs (e.g. materials, equipment, vehicles, etc.), and any other information required to fully explain how you arrived at your proposed prices. This price breakdown will assist the Government in determining price reasonableness. The offeror shall provide a separate breakdown for each period of performance (Basic period and each option years).
Part 2, Technical Acceptability & Past Performance
(a) Offerors shall prepare and submit technical information by the date/time stated in Block 8 of the SF1449. The basis of the offeror’s technical information is the Technical Response Questionnaire, attached to this Request for Quote (RFQ). Offerors shall provide sufficient information to allow the Government to judge the offeror’s technical capability to perform the requirements of this solicitation.
Technical Response Questionnaire. Provide one (1) completed questionnaire per offeror. Each completed questionnaire shall not exceed ten (10) pages in length (if printed double-sided, each side counts as one page), inclusive of questionnaire and any offeror-provided additional pages. Pages shall be numbered and shall have a header or footer that contains, at minimum, offeror’s name and the solicitation number applicable to this RFQ. If additional offeror-provided pages are attached to a completed questionnaire page, the attached pages shall be clearly marked to identify the questionnaire to which they are related.
(b) The offeror’s technical quote should address the following technical evaluation factors as outlined in the Technical Response Questionnaire.
Factor 1 At least three (3) years of experience within the five (5) year period immediately preceding release of this RFP providing commercial laundry and dry cleaning services similar in scope and magnitude to the Beale Air Force Base (BAFB) requirements (e.g. performance of commercial laundry and dry cleaning services for customers such as hospitals, hotels, restaurants, and/or other Government or business entities) and resulting in returned items that are properly finished and packaged; have a uniform appearance; and are free of visible dirt, debris, dampness, offensive odors, mildew; and cleansing chemical reside. Provide reference and contact information to support the experience claimed by offeror. References provided must support the experience claimed by offeror.
Factor 2 At least three (3) years of experience within the five (5) year period immediately preceding release of this RFP providing pick-up and delivery services similar in scope and magnitude to the Beale Air Force Base (BAFB) requirements. Experience to include pick-up of soiled items and delivery of clean items within required schedule timelines (e.g. 100% of original items picked up shall be returned no later than two [2] days after pick-up or within one [1] day for holiday / weekend service. Provide reference and contact information to support the experience claimed by offeror. References provided must support the experience claimed by offeror.
