Along with the information required in this paragraph, the offeror shall submit a consent letter executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For a description of the characteristics or aspects the Government will consider in determining recency and relevance, see Section M, Performance Confidence Assessment.
5.2.1 Specific Content
Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate. This may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to a limited or no confidence rating, since the problems encountered may have been on a more complex program, or an offeror may have subsequently demonstrated the ability to overcome the problems encountered. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. This may allow the offeror to be considered a higher confidence candidate. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems is required.
The Offeror shall forward a copy of the past performance questionnaire to their most recent references, for whom the Offeror has contracted similar services with Federal, State, and Local Governments. If the Offeror does not have Government references, the Offeror shall provide references from private industry sources that are not prohibited from furnishing information to the Government. Prior to forwarding the questionnaire to each reference, the Offeror shall complete the Section entitled “Contractor Information”.
Offerors may include with their proposal information on problems encountered on the identified contracts and the Offeror’s corrective actions. The Government will consider this information as well as information obtained from any other sources when evaluating the Offeror’s past performance information. The Past Performance Questionnaires completed by the Offerors reference sources and sent directly to the 7th Contracting Squadron in response to this RFP are not considered part of the Past Performance Volume 35 page limitation.
A cover letter sent by the Offeror with the Past Performance Questionnaire should request the reference to complete the questionnaire and mail it to 7 CONS, Attn: Capt Hilary Ince, 381 Third Street Building 7238, Dyess AFB, TX 79607, Fax to (318) 456-4007, Attn: Capt Hilary Ince or email to 7conslgca@gmail.com, Hilary.Ince@us.af.mil, or lydia.simpkins@us.af.mil.

5.2.2 Organizational Structure Change History
Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
6.0 Volume IV - Contract Documentation and Pricing
6.1 Sample Project/Representations and Certifications
The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The offeror's proposal shall include a signed copy of the solicitation, and completed copies of Sections A, B & K. Offerors are required to complete all blocks requiring offeror information. Signature by the offeror constitutes an offer, which the Government may accept. The "original" copy should be clearly marked. In lieu of filling out Section B, offerors are required to submit pricing on the attached pricing schedule in order to meet the requirements outlined in Section M. Offerors will round to the nearest hundredth (i.e.
0.91) on the pricing schedule. Offerors should then place that document in at the end of Volume IV where Section B would normally be placed. Offerors must complete ALL required representations and certifications in Section K (Note: Be sure to sign and date where required).
6.1.1 Section A - Solicitation/Contract Form SF1442
Completion of blocks 14, 15, & 17 and signature and date for blocks 20b & 20c and submission of the proposal. Signature by the offeror on the proposal constitutes an offer, which the Government may accept. The "original" copy should be clearly marked under separate cover and should be provided along with any issued amendments.
6.1.2 Section B - Supplies or Services and Prices

6.1.3 Section K - Representations, Certifications, and other Statements of Offerors
Completed representations, certifications, acknowledgments and statements of offerors.
6.2 Exceptions to Solicitation Requirements
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, price and specific requirements of the solicitation. This information shall be provided in the format and content of Table 6.1.

SECTION M - EVALUATION FACTORS FOR AWARD
The following have been modified:
EVALUATION FACTORS FOR AWARD
Section M - Evaluation Factors for Award
EVALUATION FACTORS FOR AWARD
M-1 Source Selection
a. Basis for Contract Award
This is a competitive best value source selection in which competing offerors' past performance history will be evaluated on a basis significantly more important than cost or price considerations. The source selection shall be conducted in accordance with Federal Acquisition Regulation (FAR) 15.3, Source Selection, as supplemented by the Defense Federal Acquisition Regulation Supplement (DFARS), and the Air Force Federal Acquisition Regulation Supplement (AFFARS). These regulations are available electronically at the Air Force (AF) FARSite, http://farsite.hill.af.mil. By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. All technically acceptable offers will be considered equal. The tradeoff will occur only between the past performance factor and the price factor. Failure to meet a requirement may result in an offer being determined technically unacceptable. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. While the Government source selection evaluation team and the SSA will strive for maximum objectivity, the source selection process, by its nature, is subjective; and therefore, professional judgment is implicit throughout the entire process.
b. Number of Contracts to be Awarded
The Government intends to award a single contract for the Dyess Simplified Acquisition of Base Engineer Requirements (SABER) Indefinite-Delivery Indefinite-Quantity (ID/IQ)Program.
c. Competitive Advantage from Use of GFP
At this time, there is no intent of the government providing use of government-furnished property under this contract. Any use of government-furnished property would be based on a special situation and dealt with on a case by case basis.
d. Discussions
If, during the evaluation period, it is determined to be in the best interest of the Government to hold discussions, offeror responses to Evaluation Notices (ENs) and the Final Proposal Revision (FPR) will be considered in making the source selection decision. If the offeror’s proposal has been evaluated as acceptable at the time discussions are closed, any changes or exceptions in the Final Proposal Revision are subject to evaluation and may introduce risk that the offeror’s proposal be determined unacceptable and ineligible for award. Discussions will be conducted IAW AFFARS MP5315.3, Source Selection. However, the Government reserves the right to award without discussions.
M-2 Evaluation Factors
a. Evaluation Factors and Subfactors
(1) The following evaluation factors will be used to evaluate each proposal: Award will be made to the offeror proposing the combination most advantageous to the Government based upon an integrated assessment of the evaluation factors and subfactors described below.
Factor 1: Technical Subfactor 1: Management/Organizational Structure Subfactor 2: Bonding Capacity Subfactor 3: Quality Control Plan Subfactor 4: Sample Task Order
Factor 2: Past Performance Factor 3: Price
(2) The relative importance of each factor and subfactor is as follows: Factor 1 (Technical) and Factor 2 (Past Performance) are of equal importance and Factor 3 (Price) is last in importance. Within the Technical Factor, the subfactors are of equal importance.
NOTE: In accordance with FAR 15.304(e), all evaluation factors other than Price, when combined, are significantly more important than price.
b. Proposal Evaluation Approach
The proposal evaluation approach is:
(1) The total weighted coefficients shall be ranked in order from lowest to highest. The offerors coefficients shall be evaluated as fair and reasonable, based on competition or other technique deemed appropriate IAW FAR 15.404-1.
(2) Evaluate proposals for technical acceptability on an acceptable or unacceptable basis.
The government technical evaluation team shall evaluate the three lowest coefficient offers on a pass/fail basis, assigning a rating of Acceptable/Unacceptable. If any of these three offers are determined to be unacceptable, the technical evaluators will proceed to evaluate the next lowest responsive offer for technical acceptability. This process will continue until there are three Technically Acceptable offers or until all offers are evaluated.
(3) Evaluate past performance, starting with the offerors with the three lowest weighted coefficients and who are determined technically acceptable. If none of the three lowest weighted coefficients rated offerors have a substantial confidence rating then the evaluation will continue until a technically acceptable proposal is determined to have a substantial confidence rating.
c. Technical Factor
The Government’s technical evaluation team shall evaluate the technical proposals on an acceptable or unacceptable basis, assigning one of the ratings described below for each subfactor. Any subfactor evaluated as “Unacceptable” will render the entire proposal unacceptable and, therefore, unawardable. Only those proposals determined to be technically acceptable, either initially or as a result of discussions, will be considered for award. However, the offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussions.
Table 2-1. Technical Acceptable/Unacceptable Ratings
Rating Description
Acceptable Proposal clearly meets the minimum requirements of the solicitation.
Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation.
Evaluators shall assign a rating of ACCEPTABLE / UNACCEPTABLE based on the minimum requirements stated in section L and below.
To be determined technically acceptable at the factor level, the offeror must be rated acceptable in each subfactor. A subfactor will be rated acceptable when all the corresponding elements of the subfactor are met and the inherent risk is no more than moderate.
Risk will not receive a separate rating; rather, it will be inherent within the subfactor ratings.
Risk Levels will be defined using one of the ratings described in DoD Source Selection Procedures, Table 2-2 – Technical Risk Rating Method, excerpted below.
Table 2-2. Technical Risk Rating Method
Adjectival Rating Description
Low Proposal may contain weakness(es) which have little potential to cause disruption of schedule, increased cost or degradation of performance.
Normal contractor effort and normal Government monitoring will likely be able to overcome any difficulties.
Moderate Proposal contains a significant weakness or combination of weaknesses which may potentially cause disruption of schedule, increased cost or degradation of performance. Special contractor emphasis and close Government monitoring will likely be able to overcome difficulties.
High Proposal contains a significant weakness or combination of weaknesses which is likely to cause significant disruption of schedule, increased cost or degradation of performance. Is unlikely to overcome any difficulties, even with special contractor emphasis and close Government monitoring.
Unacceptable Proposal contains a material failure or a combination of significant weaknesses that increases the risk of unsuccessful performance to an unacceptable level.
