About this file

This is a Questions and Answers document for federal contract solicitation FA465426Q0001 0004, issued on 19 August 2026 for Employment Physicals services at Grissom Air Reserve Base.

The solicitation establishes a Blanket Purchase Agreement (BPA) under FAR Part 13 with an overall ceiling of $350,000 and no guaranteed minimum order. Historical volume is approximately 20 new-hire physicals annually, though this is an estimate. The contract requires a Board-Certified Occupational Health Physician who may oversee a qualified clinical team (Nurse Practitioners, Physician Assistants, registered nurses, and certified technicians) performing tasks within their authorized scopes of practice; however, the physician must provide direct clinical oversight, review all results, make final medical determinations, and personally sign required documentation. Vendors must have a facility within driving distance of Grissom Air Reserve Base and the cities of Lafayette, Indianapolis, South Bend, and Fort Wayne, Indiana. An incumbent contractor currently supports this requirement. Submission requires two volumes: Volume I (Contract Documents) and Volume II (Price); a technical-capability narrative may be submitted as optional Volume III. For pricing, vendors may submit uniform costs across base and option years by stating subsequent years match base-year pricing, but percentage-increase submissions are not acceptable. All prime contractors and teaming partners must comply with cybersecurity requirements, including maintaining a current (not older than 3 years) NIST SP 800-171 DoD Assessment posted in the Supplier Performance Risk System (SPRS) prior to award and must flow down these requirements to any subcontractors or teaming partners processing, storing, or transmitting Controlled Unclassified Information.

19 August 2026 Solicitation FA465426Q0001 0004 Employment Physicals – Grissom ARB Questions and Answers

Part 1: Previous Vendor Questions & Answers

1. NIST Assessment Requirements

• Question: What is a NIST Assessment and how do we complete it?

• Answer: The Department of Defense now requires compliance with DFARS clauses 252.204-7019 and 252.204-7020, including a valid NIST SP 800-171 self-assessment score submitted in the

Supplier Performance Risk System (SPRS). This requirement pertains to cybersecurity and applies even if you do not anticipate handling Controlled Unclassified Information (CUI). This is a necessary measure to ensure the security of the entire government supply chain.

While achieving a perfect score is not currently required, a valid self-assessment is mandatory. If your score is not perfect, a plan of action with deadlines for achieving full compliance (a "Plan of Actions &

Milestones" or POA&M) will be necessary. Existing assessments and POA&Ms within their remediation period will satisfy this requirement.

2. Wide Area Workflow (WAWF)

• Question: What is Wide Area Workflow (WAWF)?

• Answer: Wide Area Workflow (WAWF) is a secure, web-based system designed for electronic invoicing, receipt, and acceptance, primarily used by the Department of Defense (DoD) to streamline the payment process for government contracts.

3. Submission of Option Year Pricing Structure

• Question: In order to reduce administration time, would it be possible for us to submit the base year numbers and simply state what percentage the cost will increase for the following option years?

• Answer: No, this would not be accepted. Unfortunately, this may open up further questions when it comes to rounding, etc.

4. Uniform Pricing Across Base and Option Years

• Question: In order to reduce administration time, if we plan to maintain the same numbers for the base and option years, would it be possible for us to submit the base year numbers and simply state the following years would be the same cost?

• Answer: Yes, this would be accepted.

5. Out-of-Area Prime Contractors with Local Teaming Facilities

• Question: Could you please confirm whether the government would consider a contractor whose corporate office is not in the area, but performs the services through a locally based partner facility that meets the geographic requirements outlined in the RFQ?

• Answer: If you are considering a teaming agreement, please note that a Teaming/Partnering agreement is required to be submitted with your quotation. Please review the Section L document provided on SAM.gov for further details.

6. Incumbent Contractor Status

• Question: Could you please advise whether this requirement is currently supported by an incumbent contractor?

• Answer: This requirement is currently supported.

7. Geographic Driving Distance Range

• Question: We are interested in submitting a quote and would like to confirm that the facility must be in driving distance to Grissom AFB.

• Answer: You are correct. The solicitation specifies that any interested vendor must have a facility within the range of Grissom Air Reserve Base and the following cities:

o Lafayette, IN o Indianapolis, IN o South Bend, IN o Fort Wayne, IN

Employment Physicals – Grissom ARB Questions and Answers Part 2: New Vendor Questions & Answers (Amendment 0004)

8. Contract Physician Delegation of Exam Duties & Oversight

• Question: The PWS states that the Contract Physician must be Board Certified in Occupational

Health to provide physicals. Could you please clarify whether the Board-Certified Occupational

Health physician is required to personally perform each physical examination, or whether the physician may oversee a qualified clinical team, including Nurse Practitioners, Physician

Assistants, nurses, and certified technicians, who perform portions of the examinations and testing within their respective scopes of practice?

More specifically, would the requirement be satisfied if the Board-Certified Occupational Health physician provides oversight, reviews the examination and test results, makes the final medical qualification determination, and signs the required documentation? If this structure is permitted, are there any portions of the physical examination that must be personally performed by the Board-Certified

Occupational Health physician?

• Answer: Yes, this structure is permitted. The Board-Certified Occupational Health Physician is not required to personally perform every portion of the physical examination. They may oversee a qualified clinical team (including Nurse Practitioners, Physician Assistants, registered nurses, and certified technicians) performing tasks within their respective, legally authorized scopes of practice.

The Board-Certified Occupational Health Physician must provide direct clinical oversight, review all examination and testing results, make the final medical qualification determination, and personally sign all required medical documentation.

There are no specific portions of the physical examination that must be exclusively performed by the physician, provided the clinical team members are fully qualified under state licensing laws and the physician retains ultimate clinical responsibility for the final medical determination.

9. Controlling Cybersecurity Requirement (Amendment 0004 Status)

• Question: Amendment 0004 states that CMMC requirements were revised and deletes DFARS

252.240-7997. However, Section L, Section M, and the posted Questions and Answers still state that a current NIST SP 800-171 self-assessment score in SPRS is mandatory. Please identify the exact cybersecurity, CMMC, and SPRS status required of the prime offeror at quotation and at

BPA award.

• Answer: While the Department has suspended CMMC Phase II third-party certification requirements, the baseline requirement to protect Controlled Unclassified Information (CUI) remains active. In accordance with DFARS 252.204-7019, the Prime offeror must have a current

(not older than 3 years) NIST SP 800-171 DoD Assessment posted in the Supplier Performance

Risk System (SPRS) prior to contract award.

10. Teaming-Partner Cybersecurity Compliance

• Question: If the prime uses a local occupational-health clinic under a complete teaming agreement, must the prime itself hold the required SPRS/CMMC status, or may a qualified first-tier clinical teaming partner satisfy any applicable system requirement for the medical work and records it processes?

• Answer: Both must comply. The Prime contractor holds the privity of contract with the

Government and must have an active SPRS registration and assessment score.

Furthermore, under DFARS 252.204-7012 and 252.204-7020, the Prime is responsible for flowing down these cybersecurity requirements to any subcontractor or teaming partner that will process, store, or transmit CUI (such as medical records tied to mission readiness).

11. Proposal-Volume Structure

• Question: Section L refers to four separate volumes but expressly identifies only Volume I, Contract Documents, and Volume II, Price. Please confirm the complete required volume structure and whether a separate technical-capability narrative must be submitted.

• Answer: The reference to four volumes was an administrative error. The Government requires two (2) volumes: Volume I (Contract Documents) and Volume II (Price). A separate technical-capability narrative is not required but is permissible. If submitted, it should be submitted as

Volume III.

12. BPA Ordering Value and Volume Details

• Question: Amendment 0004 references revised maximum call-order and overall potential values, while the price schedule provides examination CLINs without estimated quantities.

Please confirm the maximum individual call amount, overall BPA ceiling, any guaranteed minimum, and any available historical or estimated annual examination volume.

• Answer: As this is a Blanket Purchase Agreement (BPA) established under FAR Part 13, there is no guaranteed minimum. The overall BPA ceiling is $350,000.

The maximum individual call limit is not specified, as the cost of each physical may depend on the services needed for the potential new employee. Historically, the Government requires approximately 20 new-hire physicals annually, though this is an estimate and not a guarantee of future volume.
