STATEMENT OF WORK
BUILDING 1075 MOLD REMEDATION
MALMSTROM AIR FORCE BASE, MT
WT 14660841
10 JULY 2025
SECTION 1
DESCRIPTION OF SERVICE
1.1 GENERAL REQUIREMENTS
Provide all items, articles, materials, operations, or methods listed, mentioned or scheduled on the drawings and/or as specified herein, including all labor, materials, equipment, and incidentals necessary and required for completion of the scope of this project.
1.2 SUMMARY OF WORK
1.2.1 MOLD REMEDIATION
1.2.1.1 The Contractor shall provide all labor, equipment, and materials to perform mold remediation in the approximately 7,000 SF crawlspace of Building 1075. Mold removal shall be conducted in containment. After mold remediation is complete, a clearance will be performed by an independent industrial hygiene technician.
1.2.1.2 The Contractor shall dispose of all materials removed during remediation to include water, mold-contaminated products, organic debris, all pipe insulation in the crawlspace (see Table 1), and general waste.
1.2.1.3 The Contractor shall submit a detailed report of work accomplished. The Contractor shall provide all documentation and inspections in accordance with State and Federal requirements.
1.2.1.4 Work shall be conducted in accordance with all manufacturer’s specifications and recommendations.
1.2.1.5 A Limited Environmental Assessment of Crawlspace was conducted for this building in
2023. The report is provided in Appendix A.

1.2.1.6 Photographs of mold contamination in the crawlspace are included in Appendix B.
1.3 NOT USED
1.4 EXECUTION
1.4.1 PROJECT SEQUENCE
1.4.1.1 The Contractor shall prepare a Health and Safety Plan (HASP) and submit a copy of the HASP to the Government prior to the start of work.
1.4.1.2 For the mold remediation work to be considered complete, the independent industrial hygiene technician must determine that the mold remediation passed clearance based on the results of the visual inspection and laboratory results of the pre-work and clearance air samples.
1.4.1.3 A floor plan of the crawlspace is provided in Appendix C.
1.4.2 MOLD REMEDIATION
1.4.2.1 The Contractor shall submit a work plan to the Government for approval prior to starting work. The work plan shall include the procedures to enter and exit the work area, procedures for mold removal and bacterial disinfection, method of active ventilation to be used during remediation, decontamination procedures, method to protect upstairs and other areas not included in the project during the work, clean-up procedures, waste management, and a work schedule. The work plan shall specify which chemicals will be used during the work and shall provide Safety Data Sheets for each chemical.
1.4.2.2 Set up containment. The Contractor shall ensure that main floor, mechanical room, and areas not included in the project are protected from the work, and decontamination procedures shall be followed prior to workers exiting containment. Seal the interior floor hatches and any other fenestrations. The crawlspace access in the kitchen area and janitor’s room of Building 1075 shall be sealed during remediation work such that it will not interrupt food preparation or impede kitchen staff activities. The Contractor shall coordinate with the independent industrial hygiene technician to collect pre-work air samples from inside the containment, outside the containment entrance, and the building exterior (control). Record humidity inside and outside the containment. The Contractor shall set up air scrubbers in the containment to pull spores from the air.
1.4.2.3 The Contractor shall remove all trash, debris, and refuse from the crawlspace. All pipe insulation shall be removed from the crawlspace. Appendix D shows how far to extend pipe insulation removal into the boiler room. Perform gross removal of mold and a deeper cleaning. For the deeper cleaning, the crawlspace shall be cleaned down to the concrete, standing water shall be removed from the crawlspace, and all other foreign and organic materials shall be removed from the crawlspace as part of remediation. For exposed areas of soil below the concrete, the Contractor shall remove water to the soil, and surface soil shall be removed until visible mold and bacteria are no longer observed.
1.4.2.4 When mold remediation activities are complete, inform the Government. It is recommended to run the air scrubbers for an additional 24 hours prior to the clearance.
An independent industrial hygiene technician will perform the clearance. The Contractor shall be present during the visual portion of the clearance. The clearance will consist of a visual inspection, tape samples if needed, a humidity recording, and air samples. The clearance will fail if there is any visible mold or if the industrial hygiene technician determines that the air samples failed based on the results of the pre-work and clearance air samples.
1.4.2.5 If the clearance fails, the Contractor shall reclean at no additional cost to the Government.
When finished with the reclean, inform the Government so that another clearance may be performed.
1.4.2.6 When the clearance passes, the Contractor shall apply a fungicide to the concrete in the crawlspace. The Contractor may apply the fungicide prior to the clearance; however, it will need to be reapplied if the clearance fails.
1.4.2.7 The Contractor shall turn off the air scrubbers, perform site cleanup, and remove the containment.
1.4.2.8 Contractor shall deliver a clean crawlspace with all Contractor remediation products and waste removed.
1.4.2.9 Appendix D contains helpful figures and photographs which further describe the mold remediation work.
1.5 GOVERNMENT SUPPORT
1.5.1 The Government will be responsible for removal of all auxiliary equipment that would impede project execution by the Contractor.
1.5.2 A Limited Environmental Assessment of Crawlspace was completed on August 4, 2023.
This included an asbestos inspection, chemical monitoring, and a mold and bacteria inspection. Asbestos was NOT found in the crawlspace. The Limited Environmental
Assessment of Crawlspace is provided in Appendix A.
1.6 UTILITIES
1.6.1 The Contractor shall be held responsible for any damage to a utility resulting from their activities.
1.6.2 If a pipe leaks or breaks during mold remediation activities, stop work and contact Mr.
Jon Wood at (406) 731-6127 and Ms. Laura Hart at (406) 731-7703. Remediation work stoppage due to pipe leaks or breaks will not count against the period of performance.
The Contractor shall not be compensated for labor during work stoppage.
1.6.3 The Contractor shall provide any necessary portable power.
1.7 CONTINUED USE OF FACILITIES
1.7.1 Buildings, structures, facilities, and utilities will continue in use by the Government during this contract.
1.7.2 The Contractor shall notify the Contracting Officer at least three (3) days prior when a no-work condition will occur for more than one day by the Contractor or sub-contractors.
1.8 WORK SCHEDULE
1.8.1 Working hours for the Contractor shall normally be between the hours of 7:30 a.m. and
4:30 p.m., excluding Saturdays, Sundays, Federal Holidays, and when the base is officially closed by the base commander. Typical DFAC work hours are between 5:00 a.m. and 8:00 p.m., 7 days a week.
1.8.3 The Contractor is not required to provide routine service on the following Recognized
Holidays: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. NOTE: Normally, if the holiday falls on Saturday, it is observed on the Friday prior. If the holiday falls on Sunday, it is observed on the Monday after.
1.8.4 If the Contractor desires to work during periods other than described above, additional
Government inspection forces may be required. The Contractor must notify the Contracting Officer three (3) days in advance of his intention to work during other periods to allow assignment of additional inspection forces when the Contracting Officer determines that they are reasonably available. If such inspection forces are reasonably available, the Contracting Officer may authorize the Contractor to perform work during periods other than normal duty hours/days. However, if inspectors are required to perform in excess of their normal duty hours/days solely for the benefit of the Contractor, adjustments to the contract price may be made periodically as directed by the Contracting Officer.
1.9 PERIOD OF PERFORMANCE
The Contractor shall have all required work, and tests completed no later than 45 calendar days from the issuance of their Notice to Proceed. This period of performance shall include any additional recleans or clearances if required.
SECTION 2
CONTRACTOR REQUIREMENTS
2.1 CONTRACTOR CERTIFICATIONS
The Contractor shall have a valid Contractor’s License or hire sub-contractors with a valid Contractor’s License with the State of Montana Contractors Licensing Board.
2.2 CONTRACTOR VEHICLES AND EQUIPMENT
2.2.1 The Contractor shall provide all vehicles and equipment necessary to accomplish the contract work. Power equipment shall be equipped with safety and noise limiting devices.
The equipment shall be in a safe and efficient operating condition.
2.2.2 The Contractor shall not park vehicles in the project area during non-working hours. A designated parking area for vehicles and equipment will be provided. Vehicles will not be allowed on lawns or sidewalks without prior written approval.
2.2.3 All Contractor vehicles and equipment will be clearly marked with the company name.
2.2.4 Truckload Limits: The following load limits shall apply to all Contractor-operated equipment on this project:

2.3 HOUSEKEEPING AND CUSTODIAL SERVICES
2.3.1 The Contractor must maintain the Contractor assigned areas in a clean, safe, and sanitary condition. The Contractor shall remove trash from all the work areas, store all flammables in an approved flammable storage container, and ensure the area is left in an orderly manner at the end of the workday.
2.3.2 The Contractor’s employees may use the nearest sanitary facilities available to the project site. The Contractor shall not use facilities in controlled or restricted areas in accordance with security requirements.
2.3.3 Waste generated by the Contractor in performance of the services specified in this contract shall be disposed of by the Contractor off base in compliance with State and Local ordinances, including hazardous materials in which proper disposal instructions shall be complied with. All trash, debris, and refuse caused by the project shall be removed, disposed of, and deposited in an approved receptacle or container furnished by the Contractor. The project site shall be cleaned at the end of each workday.
2.4 ENVIRONMENTAL, SAFETY, AND HEALTH
2.4.1 The Contractor shall comply with all existing Occupational Safety and Health Act
(OSHA) standards of safety at all times in the performance of this contract. Hazards to the safe use of the premises due to the Contractor's work and/or equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed.
2.4.2 The Contractor shall immediately report any and all injuries and damage involving
Contractor, his employees, Air Force civilian, or military personnel to the Quality Assurance Evaluator (QAE) and the Wing Safety Office.
2.4.3 The Contractor shall cooperate fully and assist Government personnel in any Government investigation of the mishap, until released by the mishap investigative authority through the Contracting Officer. Safety personnel shall have the right, at any time, to investigate and report all mishaps involving Air Force personnel and property. The Contracting Officer and Contractor Safety Representative may accompany Air Force mishap investigation personnel, but may not release information regarding the mishap without written approval from the Air Force Safety investigating official or senior installation commander.
2.4.4 The Contractor shall maintain records of Contractor employee’s occupational injuries, illnesses, and other related activities as required.
2.4.5 The Contractor shall comply with the highest degree of environmental protection or safety practices where any disagreements between requirements may exist.
2.4.6 The Contractor shall be liable and responsible for determining and maintaining compliance with all applicable Federal, State, and local environmental laws or regulations.
2.4.7 The Contractor shall be liable and responsible for the reporting and clean-up of any hazardous chemical discharge or spills resulting from Contractor negligence.
2.4.8 The Contractor shall be liable and responsible for all costs resulting from non-compliance, whether accidental or intended. This includes any fines or corrective actions assessed by any regulatory or governing agency due to the actions or omissions of the Contractor in carrying out his duties.
2.5 FIRE PREVENTION
The Contractor shall establish and implement procedures to ensure careful handling and storage of flammable and combustible products in accordance with the National Fire Protection Association (NFPA) Standard 30 and National Fire Codes.
2.6 NOT USED
2.7 ANTI-TERRORISM (AT) AWARENESS TRAINING
In accordance with DoDI O-2000.16v1_AFI 31-145-O, Antiterrorism (AT) Program Implementation, ensure Level I - AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the government. The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures.
2.8 FORCE PROTECTION
Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed. Examples include mandatory badge or ID checks, facility single points-of-entry and searches of hand carried items.
SECTION 3
ADMINSTRATION AND INSPECTION
3.1 ADMINISTRATIVE RECORDS
All records, documents, and associated papers provided by the Government and generated during the period of this contract shall become Government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.
3.2 INSPECTION
3.2.1 The 341st Civil Engineer Squadron will provide a Quality Assurance Evaluator (QAE) to verify accomplishment of work. The QAE will be the spokesman for compliance with base standards. Controversies between the QAE and the Contractor will be resolved by the Contracting Officer. If, for some reason, a change in the primary or alternate QAE is required, the Contractor will be notified in writing by the Contracting Officer. The Contractor shall coordinate Contractor activities with the 341st Civil Engineer Squadron.
3.2.2 The Contractor shall notify the QAE of their estimated work completion date at least three
(3) days in advance, to verify accomplishment of work and receive Contractor work report. Any deficiencies found shall be rectified by the Contractor and the Contractor shall inform the QAE upon completion.
