About this file

This document is a Past Performance Questionnaire issued by the 509th Contracting Squadron of the Air Force Global Strike Command for a Fitness Instructor services contract at Whiteman Air Force Base. The questionnaire is designed to collect detailed performance feedback from previous clients about a potential contractor, with the goal of evaluating the contractor's past performance as part of a competitive source selection process.

The document provides a comprehensive evaluation framework with 14 specific performance criteria, including cooperation, management capabilities, safety, regulatory compliance, and overall contract performance. Respondents are asked to rate the contractor on each criterion using a scale ranging from Exceptional (E) to Unsatisfactory (U), with space for supporting comments. The questionnaire also includes sections for contract identification, customer information, and final assessment questions such as whether a cure notice was issued and if the respondent would award another contract to the same contractor. The completed questionnaire must be returned within seven calendar days to A1C Brevin Morris at the provided contact information.
