This is a combined synopsis/solicitation for Oil Waste Seperation service (base year plus four option years) prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation # FA4625-17-R-0012 is being issued as a Request for Quote.

This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95, Defense Acquisition Circular 20161222, and Air Force Acquisition Circular 2017-0127. This procurement is a 100% Small Business Set-Aside. The applicable North American Industry Classification System (NAICS) Code is 562998 (All Other Miscellaneous Waste Management Services). The Small Business Size Standard is $7.5M. Paper copies of the RFQ will not be made available. Quotes can be e-mailed to the following e-mail address:

Quotes must be received by 7 July 2017, 3:00 PM CST in order to be considered for award; late quotes will not be considered for award. All quotes shall be valid for a period of 160 days. Prospective offerors must be registered in the System for Award Management (SAM) (www.sam.gov) in order to be eligible for award.

All responsible sources may submit a quote which, if timely received, will be considered by this agency.

* Notice to Offeror(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date for receipt of proposals.

In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.

** The Offeror’s proposal will be evaluated by price and technical capability. Price will be evaluated based upon the lowest price proposed. Technical capability will be evaluated based upon the Offeror’s submission of two (2) references occurring within the last two (2) years for whom the Offeror has contracted similar service with Federal, State, or Local Governments. If the Offeror does not have Government references, the Offeror shall provide references from private industry sources that are not prohibited from furnishing information to the Government.

NOTE: A site visit for this solicitation will be held on 21 June 2017 at 2:00 p.m. CST.

Attendance will be limited to two (2) persons per offeror. All attendees will meet at Whiteman AFB Visitor’s center by 1:30 p.m. CST. Quotes and site visit coordination shall be sent to 509cons.sollgca@us.af.mil to the attention of SSgt Weiqi Fu and Amn Yong Song. Site visit information shall be submitted no later than 19 June 2017 at 1:00 p.m. CST. Provide name, phone number and company name for attendance. A valid Government issued ID will be required for entry. For either the site visit or submission of proposals, all personnel requesting access to Whiteman AFB must possess a valid State or Government picture identification card. Furthermore, individuals presenting identification cards from a state that is non-compliant with the REAL ID Act will require additional documentation to gain unescorted base access. Additional documentation include a valid U.S. or foreign government issued passport, an employment authorization document that contains a photograph, or identification cards issued by federal state or local government agencies that include a photo and biographic information. LIFT STATION, GREASE TRAPS & GRIT

PERFORMANCE WORK STATEMENT FOR OWS

1. DESCRIPTION AND SCOPE OF WORK. The contractor shall provide all manage-ment, tools, supplies, equipment and labor necessary to inspect, pump, clean and remove the Oil/Water Separators (OWS), Lift Stations, Grease Traps and Grit Chamber contents located at Whiteman Air Force Base, Missouri as defined in this Performance Work Statement (PWS).

Conform to all federal, state, local laws and regulations, commercial standards, and Air Force Occupational Safety and Health (AFOSH 91-501) and Occupational Safety and Health Admin-istration (OSHA) requirements. Work shall be performed in accordance with (IAW) the terms and conditions of the Performance Work Statement (PWS). Estimated requirements and esti-mated workload data for the OWS, Grease Traps, Lift Stations and Grit Chamber are listed in Appendix A. See attached maps for location of OWS, Grease Traps, Lift Stations, and Grit Chamber.

1.1. SPECIFICATIONS AND DESCRIPTION OF WORK:

1.1.1. SAFETY AND HEALTH STANDARDS. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act (OSHA) concerning entry requirements in confined spaces and handling potential hazardous substances as it pertains to liquid wastes. The fol-lowing is a list of federal regulations pertaining to liquid wastes:

Oil Pollution Act of 1990, Section 100I The Clean Water Act, Title 3, "Standards & Enforcement," and Title 4, "Permits & Licenses" Title 29 CFR Part 1910, "Occupational Safety & Health Stand-ards/Administration-Labor" Title 40 CRF Parts I 04-149 "Water Programs" Title 40 CRF Part 110, ''Discharge of Oil" Title 40 CRF Part 1 12, "Oil Pollution Prevention" Title 40 CRF Parts 401-471, "Effluent Guidelines and Standards" Title 40 CRF Subpart F, "Releases from Solid Waste Management Units" U.S. DOT Federal Motor Carrier Safety Administration (FMCSA), Commercial Motor Vehicle Safety Act of 1986 6 CCR 1007-2, Section 18, WASTE GREASE TRANSPORTERS, FACILITIES, AND PERSONAL USERS OF WASTE GREASE

1.1.2. CONFINED SPACE ENTRY TAINING. All OWSs on Whiteman AFB are situated below the ground surface and considered a permit-required confined space. However, all OWSs are assessable from ground level, and it should not be necessary for the contractor to break the surface plane to clean and inspect the OWSs. In the event that the contractor deems it necessary to break the surface plane and enter the confined space, they shall notify the Contracting Officer’s Rep-resentative (COR) to coordinate with Whiteman Fire Department. Service must be coordinated to ensure fire department staff availability.

1.2. TESTING.

1.2.1. SEMI ANNUAL REQUIREMENT S. Before solid and liquid materials can be removed the contractor shall perform appropriate tests of the contents (floating materials, liquid and sludge) to determine the materials present in tanks. Infor-mation from the test results shall be used to determine appropriate handling, transporting and reclamation/disposal of oil/water separator material. Analyzed samples must be in the parameters listed in the 40 CFR Part 279. 1 I. A copy of the results shall be sent to Service Contracts Office and Environmental Office in 509 CES. The government shall be notified if test results exceed any parameters out-lined in the 40 CFR Part 279. I I prior to maintenance scheduling or servicing.

Contractor shall transport and dispose, reuse or reclaim waste in accordance with federal, state and local regulations.

1.3. MAINTENANCE.

1.3.1. SCHEDULING MAINTENANCE. The contractor shall initiate scheduling the annual routine maintenance IAW Appendix A "Estimated Workload Data" and with Contracting Officer's Representative (COR) in Service Contracts at (660) 687- 6275/3204 prior to servicing OWS, Grease Traps, Lift Stations and Grit Chamber listed on Appendix A. At least one week prior to the proposed scheduled service date falling in each month the contractor must contact CE Service Contracts to coordinate the date(s) and time(s) with the using agencies and provide confirmation of approval of schedule to the contractor.

1.3.2. ROUTINE MAINTENANCE. Normally, all work shall be accomplished dur-ing the contractor's predetermined work schedule. Upon arrival at the base to per-form scheduled service, the contractor shall contact the CE Service Contracts office for validation of arrival.

1.3.2.1. OIL/WATER SEPARATORS. Services shall be performed bi-annually in the month of April and October to OWS listed in Appendix A. In the month of April, the contractor shall pump all oil, oil residue, and other waste water from separators and catch tanks; draining separator and catch tanks completely. All filters shall be removed, cleaned of all sludge and debris, and reinstalled. All oil, oil residue, and other waste wa-ter from tanks shall be properly disposed of in accordance with federal state and local regulation. After cleaning, contractor shall inspect entire system for proper perfor-mance. Contractor shall wash and drain the separator and fill with clean water as re-quired for the unique configuration of the separator. Contractor shall transport all mate-rials from oil/water separators to an approved off-base reclamation/disposal facility. Par-tial servicing, to include only pumping, will be performed in the month of October, un-less another month is approved by 509 CES/CEOS.

1.3.2.2. GREASE TRAPS. Services shall be performed to all grease traps listed in Appendix A any time during each quarter of fiscal year. The contractor shall furnish one five (5) gallon pail of powered ENZYME DIGESTANT for each grease trap listed in Appendix A at the beginning of the contractual year and treat grease traps when service is performed.

1.3.2.3. LIFT STATIONS Services include pumping, scraping, and/or power washing lift station at facilities listed in Appendix A. The contractor shall insure that the bot-toms of the wells are thoroughly pumped and cleaned with no debris or sludge remain-ing. After servicing of lift station all foreign matter including, but not limited to, grease, scum and grit shall be removed from the entire unit and all parts associated with the unit being serviced. The waste water lift station shall be serviced a minimum of times listed in Appendix A; please refer to Appendix A for specific frequencies.

1.3.2.4. GRIT CHAMBER. Grit Chamber (2 ft. x 14 ft.): Remove grit from chamber at least twice a year. This chamber is located at the Waste Water Treat-ment Plant facility (bldg. 5034).

1.4. EMERGENCY SERVICE CALLS. In the event that emergency service is re-quired, the contractor shall begin service within twenty-four (24) hours after notifica-tion by the COR. Emergency service calls shall be priced as a separate contract line item number (CLIN).

1.5. WASTE HANDLING. All liquids and semi-solid waste removed from Whiteman AFB shall be disposed of off Air Force property in accordance with all ap-plicable Federal, State and local environmental regulations stated in this PWS, Para-graph 1.1 .1 .

1.6. LICENSES AND PERMITS. The contractor shall be responsible for obtain-ing all federal, state, and local required permits in order to perform the work and transport of hazardous and non-hazardous materials as described in this statement of work and in accordance with Commercial Vehicle Safety Act of 1986, WASTE

GREASE TRANSPORTERS, FACILITIES, AND PERSONAL USERS OF

WASTE GREASE, HB 10-1125 and for complying with any federal, state, and municipal laws, codes, and regulations applicable to the performance of work.

The contractor shall provide proof of such applicable licenses and permits upon request of the Air Force.

1.7. RECORDS.

1.7.1. INVOICES. Invoices shall be submitted in Wide Area Work Flow (WAW F).

Invoices shall detail date of service, building number and name of location(s) serviced, cost of service at each building, and total cost. Specific directions on invoicing can be found at the WAWF help desk, at 866-618-5988.

1.7.2. SERVICE TICKETS. Service tickets shall be provided by the contractor for each of the areas listed in this agreement. As a minimum the tickets shall reflect the date, time, and type of service (approx. number of gallons removed) that was performed with a comments section that allows for description of services provided and notation of any repairs that the unit being serviced may require. A copy of the tickets shall be sent to Service Contracts Office in 509 CES.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.

3.1. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government will provide all water and electricity as may be required by the contractor to perform the service.

Contractor should be prepared to supply materials to access provided utilities. Contractor shall provide all extensions (hoses/electrical cords) to utilities and compatible hook ups to water source or hydrants where applicable. Contractor shall provide long distance hoses of at least two hundred (200) feet or more. Contractor shall protect water sources by use of back flow prevention supplied by the contractor. Available electrical connections can be utilized by con-tractor or contractor must provide a generator.

3.2. GOVERNMENT FURNISHED SUPPLIES AND EQUIPMENT. None.

3.3. SECURITY, FIRE AND MEDICAL SERVICES. The government will provide police and fire protection. In the event of a medical emergency, base ambulance service for trans-porting an injured employee to a local hospital is available on a cost reimbursement basis.

4. GENERAL INFORMATION.

4.1. HOURS OF OPERATION. The contractor shall perform the services required under this Contract during the following hours.

4.1.1. NORMAL OPERATIONS. Normal hour of operation at Whiteman AFB are Monday through Friday, 0730-1630, except for Federal holidays. Unless otherwise approved by the Contracting Officer, all work shall be accomplished during normal hours of operation.

4.1. 2. FEDERAL HOLIDAYS. The following is a list of Federal Holidays observed by this installation:

New Year's Day, I January or the Friday preceding or Monday follow-ing Martin Luther King's Birthday, 3rd Monday in January President's Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, 4 July or the Friday preceding or Monday follow-ing Labor Day, First Monday in September Columbus Day, 2nd Monday in October Veterans Day, 1 I November or the Friday preceding or Monday fol-lowing Thanksgiving Day, 4th Thursday in November Christmas Day, 25 December or the Friday preceding or Monday following.

4.2. CONTRACTING OFFICER (CO) AND CONTRACTING OFFICER REPRE-

SENTIVE (COR) (QUALITY ASSURANCE PERSONNEL. The CO will appoint a primary and alternate representative for management of the day to day activities of the contract. The identity, title, and authority of this representative will be provided in writ-ing to the contractor after contract award.

4.3. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government prop-erty or an accident resulting in death, injury, occupational disease, or adverse environ-mental impact. Provide a completed copy of required Accident Investigation Reports to the CO with five calendar days of each occurrence.

4.4. REAL ID ACT. All personnel requesting access to Whiteman Air Force Base must possess a valid State or Government picture identification card. Furthermore, individuals presenting identification cards from a state that is non-compliant with the REAL ID Act will require additional documentation to gain unescorted base access. Additional docu-mentation include a valid U.S. or foreign government issued passport, an employment au-thorization document that contains a photograph, or identification cards issued by federal state or local government agencies that include a photo and biographic information. MEMORANDUM FOR ALL PERSONNEL

SUBJECT: Interim Change #1 to WAFBI 31-116, Installation Traffic Code

1. The below changes to Whiteman AFB Instruction 31-116 are effective immediately and apply to all military personnel and civilians assigned, employed, or visiting on the installation with the exception of bargaining unit employees. With regard to bargaining unit employees, the below changes will be implemented in accordance with the procedures set forth in the Labor Management Agreement.

a. Delete paragraph 4.3.18.

b. Add paragraph: 2.5.3.9. Vehicle operators on Whiteman AFB and operators of government owned vehicles, both on and off the installation, will not use cell phones unless the vehicle is safely parked or unless they are using a hands-free device. In addition, the wearing and use of other portable headphones, earphones or other listening devices is prohibited while operating a motor vehicle. Exceptions: Hearing protection as required by AFI 91-203, (Air Force Consolidated Occupational Safety Instruction) standards for operating certain types of heavy equipment and hands-free devices for cell phones.

c. Add paragraph: 2.5.3.10. Using a cell phone in a manner prohibited by paragraph 2.5.3.9.

(e.g. talking, texting, etc.) shall result in a suspension of the operator’s driving privileges.

Suspension of driving privileges pursuant to this section shall be made as follows:

2.5.3.10.1. First Offense: 30-day suspension of driving privileges.

2.5.3.10.2. Second Offense: up to 6 month suspension of driving privileges.

2.5.3.10.3. Third or any subsequent offense: up to 1 year suspension of driving privileges.
