About this file

This is Amendment 0004 to a Request for Quote (RFQ) issued by the 92nd Contracting Squadron at Fairchild Air Force Base, Washington, for a comprehensive cross-connection control survey of the installation's potable water system.

The solicitation (FA462026QA210) requires contractors to conduct a 100% physical survey of the entire water system, including approximately 490,000 linear feet of piping (distribution, transmission, and service lines) and an estimated 500 total connections. The scope encompasses approximately 305 buildings with potable water connections plus 12 additional irrigation device locations identified in Appendix D. Survey work must cover the main Fairchild AFB campus as well as six geographically separated units (Ft. Wright Wells & Cemetery at 10 miles, Geiger Reservoir at 7 miles, White Bluff at 9 miles, Ruby Creek at 90 miles, Tacoma Creek at 74 miles, and Clear Lake Recreation Area at 17 miles from the base). The survey is limited to physical visual inspection and does not include functional testing, water quality sampling, or device testing. The team lead must hold an active Washington State-issued cross-connection control specialist certification. Fairchild AFB will provide a Civil Engineering Water Shop representative to assist with building access and device location. Contractors may submit deliverables using a Microsoft Word version of AF Form 848 provided all required data fields are included. The RFQ due date has been extended from 08 May 2026 at 10:00 AM PT to 13 May 2026 at 10:00 AM PT. Amendment 0004 also updates the Combined Synopsis/Solicitation to include FAR Provision 52.222-90 addressing DEI discrimination requirements and replaces the Statement of Work with Revision 2 and the COMBO with Revision 2.

SUMMARY OF CHANGES

The purpose of this amendment is to provide additional responses to questions submitted by prospective contractors, to revise the Statement of Work, to revise the COMBO, and to extend the due date.

1. Responses to Contractor Questions All questions and answers are provided below. All applicable revisions have been incorporated into Attachment 1: Statement of Work (REV2).

Q1: Appendix C2 lists 305 buildings that will need to be surveyed with potable water. The solicitation states entirety of the premise (transmission and distribution lines). Please confirm the number of buildings contractors should base their quotes on.

A1: For bidding purposes, contractors must base their proposals on surveying the entire water system, not just a fixed number of buildings. The system includes approximately 490,000 linear feet of piping (distribution, transmission, and service lines) and an estimated 500 total connections requiring survey.

Of those 500 connections, approximately 305 are located within facilities (buildings). The provided appendices list known components but are not all-inclusive.

Q2: What must be surveyed at the GSU system? The entire system or only the containment assemblies (Point of service). What are the distances to each of the 5 Geographically Separated Units to account for travel and time from Fairchild AFB?

A2: The survey at the GSUs must cover the entirety of the water system, including both premise and in-premise protection, not just containment assemblies at the point of service. The approximate one-way travel distances from Fairchild AFB to the GSUs are as follows:

Ft. Wright Wells & Cemetery: 10 miles Geiger Reservoir: 7 miles White Bluff: 9 miles Ruby Creek: 90 miles Tacoma Creek: 74 miles Clear Lake Recreation Area: 17 miles

Q3: Is Appendix D included in these locations of 305 buildings? It appears 12 locations have DC assemblies that will need to be surveyed and inspected.

A3: The survey of the irrigation devices listed in Appendix D is required as part of the overall scope of work. However, these device locations are separate from the 305 facilities identified in Appendix C2.

Contractors should account for the Appendix D locations as part of the broader ~500 total connections across the entirety of the premise.

Q4: Will Fairchild AFB, WA provide a person to escort and assist in access to each building with potable water?

A4: Yes. In accordance with SOW Sections 4.4 and 6.1.2, a Fairchild AFB CE Water Shop representative will be available to aid in the location of devices and provide necessary access.

Q5: Can the attached Form 848 be used to conduct the cross-connection survey?

Amendment 0004 Page 3

A5: Yes. The provided Microsoft Word version of the AF Form 848 is an acceptable equivalent, provided that all data fields required by the official form are included and the final deliverable is compatible with Microsoft Office, as per SOW Section 5.2.4.

Q6: The SOW (Section 6.2.2) explicitly requires 'appropriate Washington State certifications.' A prospective contractor holds certifications from other states (Arkansas, Idaho, Iowa) as well as the American Backflow Prevention Association (ABPA). Please confirm if out-of-state or national ABPA certifications are acceptable, or if it is a strict requirement that the team lead holds a certification specifically issued by the State of Washington.

A6: No. In accordance with SOW Section 6.2.2, the team lead is strictly required to hold an active certification specifically issued by the State of Washington (e.g., Washington State-certified Cross- Connection Control Specialist). Please note that the State of Washington may offer reciprocity options for individuals holding out-of-state or national certifications, which contractors may pursue independently to meet this requirement.

Q7: When was the last time these similar surveys were conducted?

A7: The last verifiable comprehensive report was completed in 2008. Please note that the data provided in the solicitation appendices is informational. In accordance with the Statement of Work, the contractor is required to perform a 100% physical survey to field-verify all current devices, conditions, and connections across the entirety of the premise.

Q8: For the fire protection systems, is the scope of work limited to locating and inspecting the Backflow Prevention Devices (BFDs)?

A8: No. In accordance with SOW Section 3.2.3, the contractor must inspect the fire protection systems for all potential cross-connections, assess the associated BFDs for proper selection and installation, and determine the boundary for non-potable systems. The work is not limited to locating BFDs.

Q9: Is the overall project scope limited to surveying BFDs?

A9: No. As stated in SOW Sections 1 and 3, this is a comprehensive cross-connection survey of the entirety of the water system, including all piping and connections. The evaluation of existing BFDs is only one component of this comprehensive survey.

Q10: Is the contractor required to perform functional tests on devices or conduct water quality sampling?

A10: No. The scope is limited to a physical, visual survey. As stated in SOW Sections 3.2.1.2, 3.2.2.2, and 3.2.3.2, a "physical evaluation of devices' function is not necessary," and there is no requirement for water quality sampling or testing.

2. Revised Statement of Work

Attachment 1: Statement of Work (REV1) is hereby replaced with Attachment 1: Statement of Work (REV2). Attachment 1: Statement of Work (REV2) supersedes and replaces all previous versions of the Statement of Work. There is no change to the SOW Appendices.

3. Revised Combined Synopsis/Solicitation (COMBO) FAR Provision 52.222-90 Addressing DEI Discrimination by Federal Contractors has been added to the

COMBO.
