52.212-1 Instructions to Offerors -- Commercial Items (Addendum) Paragraph (c) is tailored as follows: "The Offeror agrees to hold the prices in its offer for from the date specified for receipt of 64 calendar days offers."

Additional Instructions

a. Complete Blocks 12, 17a (to include CAGE code, Company name, doing business as name (if applicable) and address), 30a, 30b, and 30c in the solicitation. In doing so, the offeror accedes to the contract terms and conditions written in the solicitation.

b. Insert Offerors proposed unit and extended prices in the SCHEDULE OF SUPPLIES/SERVICES for base year plus four (4) option year CLINs.

The extended amount must equal the whole dollar unit price multiplied by the number of units, and total amount shall be entered into block 26 on page 1.

c. Complete the necessary fill-ins and certifications in provisions incorporated into the solicitation by full text (or annotate that offeror's representations and certifications in SAM are current) and return along with the offer.

d. Questions related to this solicitation are due no later than Offerors shall e-mail the Contract Specialist, . 20 September 2024 at 10:00 a.m. MDT Ms. Karmella Van Stockum, at karmella.vanstockum.1@us.af.mil and the Contracting Officer, Mr. Arthur Makekau, at arthur.makekau@us.af.mil with all questions related to this solicitation. Responses to the questions submitted will be answered and published as an amendment to this solicitation.

e. INFORMATION REGARDING SUBMISSION OF OFFER: Offers must be signed by an authorized company official and e-mailed to Ms.

Karmella Van Stockum and Mr. Arthur Makekau and must be received no later than the quote receipt date established in the solicitation. If amendments are issued, they must be signed by an authorized company official and submitted.

If no past performance questionnaires are received the Government reserves the right to use both the information provided in the offeror's Past Performance proposal volume, and information obtained from other sources available to the Government to include, but not limited to: The Past Performance Information Retrieval System (PPIRS); Federal Awardee Performance and Integrity Information System (FAPIIS); Electronic Subcontract Reporting System (eSRS), or other databases; and interviews/questionnaires with Program Managers, Contracting Officers and Fee Determining Officials, the Defense Contract Management Agency (DCMA) , and commercial sources.

h. Authorized company official shall sign and return the UCNI (Attach 2) upon award and prior to receiving any Controlled Unclassified information / documentation.

Evaluation Factors for Award

52.217-5 Evaluation of Options.

As prescribed in (c)17.208 , insert a provision substantially the same as the following:

evaluation of (July 1990) Options

Except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the option(s).

52.212-2 Evaluation--Commercial Products and Commercial Services.

As prescribed in 12.301(c),the Contracting Officer may insert a provision substantially as follows:

Evaluation--Commercial Products and Commercial Services (Nov 2021)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(i) Price: No additional information from the offeror will be required if the price is based on adequate price competition. In the event adequate price competition does not exist, the Contracting Officer will obtain information from the offeror to the extent required to determine the reasonableness of the offered prices.

(ii) Technical Capability: Provide acknowledgement that the Contractor is capable of meeting the salient characteristics and performance requirements identified in the Performance Work Statement (PWS

(iii) Past Performance: Vendors may provide past performance information for past/present performance and relevant past performance by submitting a copy of the attached Past Performance Questionnaire (PPQ) to no more than three (3) references with instructions to return the PPQ to Karmella Van Stockum at karmella.vanstockum.1@us.af.mil no later than 1400 MDT 22 Oct 2024.PPQ submission should be as recent as 3 years for the same or similar services to include contract numbers, points of contact with telephone numbers and other relevant information. It is the offeror's responsibility to ensure the reference submits the PPQ directly to the Contracting Office for consideration. The Government may also consider any additional information from its own files, databases such as the Past Performance Information Retrieval System (PPIRS), or from any other source it deems appropriate. The Government may survey references to verify the information provided. Past performance will be evaluated as Acceptable or Unacceptable based on past performance information obtained from the aforementioned sources.

Past Performance Evaluation Rating Descriptions:

Acceptable: Based on the offeror's performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown.

Unacceptable: Based on the offeror's performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.

(b) . The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic Options requirement. In addition, as part of the price evaluation, the Government will evaluate the Option to Extend Services under FAR clause52.217-8 by adding one-half of the offeror's final option period prices to the offeror's total price. Offerors are required only to price the base and option CLINs.

Offerors shall not submit a price for the potential one-half extension of services period. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer),whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
