Past Performance Questionnaire (PPQ)
F.E. Warren AFB is in the process of soliciting offers from companies capable of providing
Portable Latrine Services at the Launch Facilities assigned to F.E. Warren AFB.
The 90th Contracting Squadron at F.E. Warren AFB, WY is conducting an evaluation on the offeror that has submitted this questionnaire to you. We request you complete the questionnaire below and e-mail a signed, PDF file to Mrs. Karmella Van Stockum at karmella.vanstockum.1@us.af.mil, Contract Specialist, 90 CONS, F. E. Warren AFB, WY.
Questionnaires are due at quote submission, 22 October 2024, 2:00 p.m. MDT.
NOTE: For vendors to be considered, the Past Performance Questionnaires must be returned to Karmella Van Stockum at karmella.vanstockum.1@us.af.mil in its entirety along with quotes by the due date/time specified above.

S SATISFACTORY – Performance meets the contractual requirements. The contractual performance being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
U UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance being assessed contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
