COMBINED SYNOPSIS/SOLICITATION

FA460025Q0087

Building 301D Dock Leveler Replacement

Offutt AFB, NE

This is a combined synopsis/solicitation for commercial commodities prepared in accordance with (IAW) the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation: quotes are being requested and a written solicitation will not be issued.

This solicitation is being issued IAW FAR Parts 12 and 13 as a Request for Quote (RFQ). The RFQ number FA460025Q0087 shall be used to reference any written quote provided under this

RFQ.

This solicitation is for Small Business set aside under NAICS Code 332312. All prospective offerors must be registered in the System for Award Management (SAM) at www.sam.gov. Lack of SAM registration will make an offeror ineligible for award.

DESCRIPTION OF REQUIREMENT

55 CES requires the Dock Leveler in B301 to be removed and replaced with a new Leveler.

Project includes, but is not limited to, the following work elements. The Contractor shall furnish all required services, labor, tools, material, and equipment necessary to complete the installation and integration of the prime mission equipment in accordance with the approved site design and installation plan. No alterations, additions, or deletions shall be made unless by prior approval of the Contracting Officer (CO). All work will meet the International Building Code, Uniform Plumbing Code, National Electrical Code, AFI, UFC, UFGS, ASME, ASTM, ASHRAE, ETL, NFPA 72 and any other applicable code requirement. The contractor shall maintain the work site in a clean and orderly manner so that the safest possible site is provided for workers and AF members. Work shall be completed in a neat, substantial, and competent manner.

APPLICABLE LABOR LAWS

This solicitation is NOT subject to the Service Contract Labor Standards FAR 52.222-41.

SPECIAL INSTRUCTIONS

Offerors shall be required to return the attached solicitation, signed, and filled in appropriately, along with any additional information required to be submitted by FAR 52.212-2 Evaluation— Commercial Items Addendum and any additional fill-in provisions.

ADDENDUM TO 52.212-1, INSTRUCTIONS TO OFFERORS

To assure timely and equitable evaluation of the quotation, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitations.

Offerors must be currently registered in the System for Award Management (https://sam.gov) and agree to remain registered while performing the services on Offutt AFB. The offerors response shall consist of:

2. Technical Capability

3. Fill-In Clauses/Provisions

1. Price Quotation: The offeror is required to submit one electronic copy of the completed solicitation. The offeror must fill out the solicitation completely and submit a signed copy. In doing so, the offeror accepts the proposed contract terms and conditions as written in the solicitation, with attachments.

a. Offerors shall provide pricing for the entirety of each Line-Item.

b. Quotes must be valid for 60 calendar days after submission due date.

2. Technical Capability: Offerors are required to submit no more than 6 pages with the following:

a. Offerors shall submit a plan describing how they will meet the requirements outlined in the SON.

b. The offeror shall demonstrate that they can provide all content within the SOW.

Additionally, the plan shall describe how the Vendor will conform to the requirements outlined within the SOW.

3. Fill-In Clauses/Provisions: Offerors are required to complete all clauses/provisions that require fill-ins and submit them with their quotes.

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(1) Price: The quotations will be ranked according to their total evaluated price (TEP) as identified by contract line-item total pricing. The Government will evaluate offers for award purposes by adding the total price for all CLINs for the required period of performance.

The offeror's price quotation will be evaluated using one or more of the procedures and techniques defined in FAR 13.106-2 and FAR 13.106-3 in order to determine price reasonableness. For a price to be reasonable, it must represent a price that a prudent person would pay in the conduct of competitive business. Normally, price reasonableness is established through competitive quotations as described in FAR 13.106-3.

(2) Technical Capability: The Vendor shall submit a plan describing how they will meet the requirements outlined in the Statement of Work (SOW). The Vendor shall demonstrate that they can provide all content within the SOW.

Quotes will be evaluated to be technically acceptable or not technically acceptable based on the evaluation factors listed above.

WIDEAREA WORKFLOW PAYMENT INSTRUCTIONS (JAN 2023)

(a) Asused in this clause--Definitions.

"Departmentof Defense Activity Address Code (DoDAAC)" is a six position code that uniquelyidentifies a unit, activity, or organization.

"Documenttype" means the type of payment request or receiving report available forcreation in Wide Area WorkFlow

(WAWF).

"Localprocessing office (LPO)" is the office responsible for payment certificationwhen payment certification is done external to the entitlement system.

"Paymentrequest" and "receiving report" are defined in the clause at 252.232-7003 , Electronic Submission of Payment Requests andReceiving Reports.

(b) . The WAWF system provides the method to electronically processvendor payment requests and Electronicinvoicing receiving reports, as authorized by Defense FederalAcquisition Regulation Supplement (DFARS) 252.232-7003 , Electronic Submission of Payment Requests andReceiving Reports.

(c) . To access WAWF, the Contractor shall--WAWFaccess

(1)Have a designated electronic business point of contact in the System for AwardManagement at https://www.sam.gov;

and

(2)Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-stepprocedures for self-registration available at this web site.

(d) . The Contractor should follow the training instructions of theWAWF Web-Based Training Course and WAWFtraining use the Practice Training Site beforesubmitting payment requests through WAWF. Both can be accessed by selecting the"Web Based Training" link on the WAWF home page at https://wawf.eb.mil/

(e) . Document submissions may be via web entry,Electronic Data Interchange, WAWFmethods of document submission or File Transfer Protocol.

(f) . The Contractor shall use the following informationwhen submitting payment requests and WAWFpayment instructions receiving reports in WAWF for thiscontract or task or delivery order:

(d) Comptroller General Examination of Record. 5352.201-9101 ACC Ombudsman (Jul 2023)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

5352.242-9000 Contractor Access to Air Force Installations (Jun 2024)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Department of the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and <<1> >to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with citing the appropriate <<2>> paragraphs as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

(f) The contractor shall provide an after-hours contact number or after-hours email in the Emergency Mass Notification System (EMNS) for each of their personnel, whose normal place of duty is on a DoD installation or within a DoD facility. The contractor shall comply with any additional requirements in DAFMAN 10-206 for emergency operational reporting. Foreign Nationals may participate and may remove themselves from the Emergency Mass Notification System at any time. To update information, personnel can access the globe icon on their system desktop screens and choose the "Access Self-Service" option.
