SCOPE OF WORK

WORK ORDER # PTFL 16-9002

Repair Bathroom & Locker Room

Joint Base M-D-L (McGuire)

1. General Requirements

The contactor shall provide and install all labor and materials to renovate the female latrines on the first floor of building 2216; located on Joint Base M-D-L, NJ. The summary of work shall consist of, but not limited to the following:

1.1. The Contractor shall ensure restrooms meet ADA requirements.

1.2. The Contractor shall provide for all demolition as noted in §2.4 & §2.5 (Existing Conditions).

1.3. The Contractor shall remove all fixtures and furnishings.

1.4. The Contractor shall provide necessary plumbing and venting to add 2 toilets and stalls.

1.5. The Contractor shall core drill 6”reinforced concrete block wall to tie into existing plumbing.

1.6. The Contractor shall provide and install wet wall in locker room area.

1.7. The Contractor shall provide and install floor leveling in locker room to match restroom. (build-up approx. 1.25”).

1.8. The Contractor shall provide and install all floor, wall and ceiling tiles, wall tiles shall terminate at 6’-3” above finished floor as noted on plan.

1.9. The Contractor shall provide and install all plumbing fixtures, to include; shower fixtures, sinks and toilets.

1.10. The Contractor shall provide and install toilet partitions, consisting of one (1) ADA Handicapped stall.

1.11. Contractor shall provide and install the following accessories: “two roll” cover dispensers, wall mounted sanitary receptacles in women’s restroom, and ADA grab bars (42” vertical bar (ADA stall & shower)), paper towel dispenser, soap dispenser, and automatic hand dryer.

1.12. The Contractor shall provide and install all mirrors and lighting (ceiling and vanities).

1.13. The Contractor shall reinstall lockers as per plan.

1.14. The Contractor shall provide and install locker room bench & pedestals.

1.15. The Contractor shall provide and install all paint.

1.16. The Contractor shall provide and install automatic hand dryer.

1.17. The Contractor shall ensure facility is fully functional and professionally cleaned.

2. Existing Conditions

Notes

2.1. Excavation-None

2.2. Landscaping

2.2.1. The contractor shall restore damage to grounds and plantings caused by contractor’s personnel or equipment.

2.3. Material Handling

2.3.1. Dumpsters and waste containers shall be kept fifteen (15) feet away from building.

2.3.2. The contractor shall remove all contract generated debris from the construction site and dispose of the debris off site at an approved landfill.

Disposal shall be in accordance with Local, State and Federal laws and regulations. In case of conflict, the most stringent law or regulation will apply.

All dumpster delivery tickets must be returned to the government.(See Material Submittals for detail)

2.4. Demolition (Remove and Dispose)

2.4.1. The Contractor shall remove and dispose of three (3) partitions (openings).

2.4.2. The Contractor shall remove and dispose of three (3) toilets.

2.4.3. The Contractor shall remove and dispose of thee (3) sinks.

2.4.4. The Contractor shall remove and dispose of five (5) mirrors.

2.4.5. The Contractor shall remove and dispose of 6’ bench and pedestals.

2.4.6. The Contractor shall remove and dispose of wall tile in restroom, locker room and shower.

2.4.7. The Contractor shall remove and dispose of ceramic floor tile in the rest room and shower.

2.4.8. The Contractor shall remove and dispose of VCT floor tile in the locker room.

2.4.9. The Contractor shall remove and dispose of shower fixtures.

2.4.10. The Contractor shall remove and dispose of ceiling tiles and grid.

2.4.11. The Contractor shall remove and dispose of light fixtures.

2.4.12. The Contractor shall remove and dispose of GFCI, duplex & switches within the bathroom and locker room.

2.4.13. The Contractor shall remove and dispose of three (3) soap dispensers

2.4.14. The Contractor shall remove and dispose of air supply/return diffusers, registers and grills.

2.4.15. The Contractor shall remove and dispose of vanity in restroom and locker room.

2.5. Demolition (Remove and Salvage)

The Contractor shall ensure all salvaged items and fixtures are removed with care as not to damage or destroy. All items salvaged are to be stored off the project until re-installation. The Contractor shall be responsible for all storage, transportation and damage for the duration of the project.

2.5.1. The Contractor shall remove and salvage one (1) paper towel dispenser

3. Concrete

3.1. The contractor shall saw cut concrete slab and/or core drill wall for installation of new toilet drains.

3.2. The Contractor shall provide and install concrete floor leveling to ensure all finished floor grades are level with no need for transitions or thresholds between rooms.

6.1.

7. Thermal and Moisture Protection (Roofing and Siding) – NONE

7.1. The contractor shall apply a bead of silicone sealant around the back of each toilet and urinal.

7.2. The contractor shall insulate domestic hot water per ADA requirements.

8. Openings (Doors, Windows and Skylights) - NONE

9. Finishes (Flooring, Walls, Ceilings)

Notes

9.1. The Contractor shall provide and install partition /wet wall to accommodate the plumbing/ventilation of the new stalls.

9.1.1. New wall shall be finished on one side, NLB metal studs with backing, 8” wide, 5/8 water resistant gypsum board.

9.2. The Contractor shall provide and install (type/style/color) floor tile.

9.3. The Contractor shall provide and install (type/style/color) tile baseboard.

9.4. The Contractor shall provide and install (type/style/color) wall tile.

9.5. The Contractor shall provide and install (type/style/color).bull nose trim.

9.6. All (type/style/color) tile shall be constructed using (type/style/color) grout. All joints shall be 1/16” (inch).

9.7. The Contractor shall provide and install a 24”x24” vinyl-clad, moisture-resistant suspended ceiling.

9.8. The Contractor shall provide and install new ceiling-tile grid-work, and new wall angle in the bathroom.

9.9. All exposed wall areas shall be painted.

9.9.1. Painted areas shall include any and all surface repairs, primer, and 3 coats of paint.

10. Specialties – NONE

10.1. The Contractor shall provide and install floor mounted partitions for all toilet fixtures. One of the 5 stalls shall be designated handicapped and shall meet or exceed all ADA requirements.

10.2. Each of the partitions shall be equipped with a coat hook on the inside of the door.

10.3. The Contractor shall provide and install three (3) soap dispensers.

10.4. The Contractor shall provide and install one (1) paper towel dispenser.

10.5. The Contractor shall reinstall 20 of the removed 39 lockers as per the configuration as per manufacturers recommendation.

10.6. The Contractor shall provide and install a new 5 foot locker room bench with pedestals.

10.7. The Contractor shall provide and install five (5) mirrors. Three (3) shall be located at the sinks. One (1) shall be located at the locker room vanity. One shall full length and be located in the locker room, next to the lockers.

10.8. The Contractor shall provide and install a vanity/countertop/make-up table in the locker room.

The Contractor shall provide and install all labor, materials, tools and equipment required to install all fixtures according to local, state and federal code. No additional compensation shall be provided. This is inclusive of all pipe, drain, valves, traps and insulation.

22.1. The Contractor shall provide and install five (5) toilet fixtures with automatic flush valves. One of the five toilet fixtures and flush valves shall be designated handicapped and shall meet or exceed all ADA requirements.

22.2. The Contractor shall provide and install three (3) sinks with automatic faucets.

One of the 3 sinks shall be designated handicapped and shall meet or exceed all ADA requirements.

22.3. The Contractor shall provide and install complete shower head / mixing valves.

22.4. The Contractor shall provide and install any adjustment as required to connect sanitary drainage system to the existing venting configuration.

23. Heating, Ventilating, and Air-Conditioning (HVAC)

Notes

23.1. The Contractor shall provide and install all air exchange/supply/return diffusers, registers or grills.

23.2. The Contractor shall provide and install any and all duct work required to tie into existing HVAC system to match into the new suspended ceiling.

23.3. The Contractor shall ensure all existing air exchange fixtures are set in the ceiling tiles appropriately.

26. Electrical

Notes

26.1. The Contractor shall provide and install a total of: (5ea) 110-VAC GFCI receptacles in the bathroom.

26.2. The Contractor shall provide and install lighting at vanities.

26.3. The Contractor shall provide and install a total of: (7ea) 2’x2’ “LED” recessed light fixtures.

26.4. The Contractor shall provide and install automatic hand dryer, touch free, 1400w, 81,000 rpm.

26.4.1. The Contractor shall provide and install all necessary wiring, grounding and mounts for all automatic hand dryers.

I. Materials All work and materials shall conform to all applicable JOC specifications and standards.

All submittals shall be provided to the project manager for approval, 24 days prior to the start of any work unless otherwise specified. The contractor shall provide a project schedule generated by Microsoft project or equivalent. The schedule needs to be submitted to the government prior to the Pre-construction meeting.

A. See attached AF Form 66 Material Submittals.

B. Submit material approval with AF Form 3000 or ENG Form 4025-R.

II. Site Requirements

C. SAFETY COMPLIANCE:

i. The contractor shall comply with Installation safety regulations, Occupational Safety and Health Act (OSHA) (Public Law 91-596) and the portions of OSHA Standards 29 CFR 1910, 1926 applicable to your operations. The detailed provisions of the act for accident prevention are directly applicable to your operations. It is also the Contractor’s responsibility to ensure all Sub-Contractors comply with these requirements. Contact the Joint Base Safety Office for further

ii. Contractors have the responsibility to follow all industrial safety provisions. Contractors are also responsible for Hazard Communications Training to all contractors and subcontractors working for your company.

iii. The contractor must immediately report all contractor mishaps involving JB MDL personnel injuries and/or property/equipment damage to the contracting officer who is responsible to notify the 87 ABW Safety Office. Do not disturb the mishap scene until proper officials authorize movement. When the contracting officer becomes aware of any safety directives’ noncompliance by the contractor that could cause injury to JB MDL personnel or damage to JB MDL property or equipment, he/she will inform the contractor. The contractor must take immediate steps to correct the situation.

iv. The JB MDL reserves the right to enter and inspect, with or without prior notice and with the contracting officer’s concurrence, any and all facilities and (or) project locations on Joint Base McGuire-Dix- Lakehurst. 87 ABW Safety personnel may periodically monitor contractor activities, if necessary, only to ensure the safety of JB MDL personnel and protection of government property. If a safety procedure or condition exists requiring correction by the contractor, 87 ABW Safety will contact the contracting officer to initiate corrective action.

v. Prior to any excavation the contractor shall call and obtain approval from New Jersey State “Call before You Dig” program, 1-800-272- 1000 for underground utility identification. For locations outside of New Jersey, the contractor is responsible to contact and obtain approval from the appropriate authority. In-Addition, the contractor shall, prior to underground excavation, contact the 87 CES customer service (Dix Area Bldg. 5344 Phone: 562-6021/McGuire and Lakehurst Area Bldg. 3401) to initiate a work clearance permit. The contractor is responsible for updating the permit every 30 days; in addition the contractor shall maintain the site condition during the entire period of performance. A copy of this approval must be available for inspection at the excavation site. The contractor shall remove all identification Flag when excavation is complete or before final inspection of the project.

D. ENVIRONMENTAL COMPLIANCE:

i. The contractor is required to comply with all applicable State and Federal laws, State and Federal regulations, and Air Force instructions

PTFL 16-9002 – Repair Bathroom & Locker Room Page 9 of 14 in effect at the time of performance of this contract. The laws and regulations include, but are not limited to: environmental and occupational safety and health.

ii. The contractor shall be responsible for compliance with construction and operating permits, and also responsible for penalties, fines, or natural resource damage claims that may be required or assessed by the State of New Jersey, Federal or Local Governments resulting from contractor performance, or failure to perform, during the course of this contract. The Government will inspect for environmental compliance at their discretion, and will notify the Contracting Officer (CO) of any discrepancies found. In addition, the contractor may be subject to contractual sanctions including, but not limited to withholding, offset, termination, suspension, and debarment and adverse past performance assessment. iii. SOLID WASTE, HAZARDOUS WASTE AND HAZARDOUS MATERIALS: The contractor is responsible for the proper handling, disposal, and costs of solid and hazardous wastes and hazardous materials generated by or utilized during the course of its activities.

Proper handling also includes recycling and material recovery. The terms solid and hazardous wastes, and hazardous materials, include, but are not limited to, those defined in the Federal Water Pollution Control Act, the Comprehensive Environmental Response, Compensation, and Liability Act of 1980, the Solid Waste Disposal Act, the Clean Air Act, and the Toxic Substances Control Act, the New Jersey Solid Waste Management Act, the New Jersey Water Pollution Control Act, the New Jersey Air Pollution Control Act and the regulations promulgated pursuant to those Acts. The contractor shall also be responsible for any enforcement action that may be taken by the State of New Jersey, other State or the Federal Government related to handling, transportation and the proper disposal of solid and hazardous waste and hazardous materials. If notification, certifications, documentation, manifests, permits or any other written materials are required by law to be obtained, prepared and/or submitted, the contractor shall prepare same and provide a written copy to the CO, and a copy to the Environmental Flight (787 CES/CEIE) within the timeframes set forth in the law and regulations governing the action. Hazardous waste manifests must be signed by the Environmental Flight Hazardous Waste Coordinator, (609) 754 6167 at Building 2404 for the Dix and McGuire Areas and (732) 323 5319 Building 5 for Lakehurst. JB MDL is not responsible for any hazardous materials brought on base by the contractor that may become hazardous waste during the course of performance.

Contractor shall remove those materials from the base at their own expense and risk.

iv. Source Separation of Solid Waste: Wastes shall be placed in closed metal containers that are emptied on a regular schedule. Solid waste shall be separated and placed in individual containers in a manner that facilitates recycling of the various categories of waste. Contractor is responsible for the handling and the associated costs of disposal for all solid wastes generated. Cardboard and all types of metal shall be source separated and recycled. Cardboard and metals shall be delivered to the McGuire AFB Recycling Center, building #2321.

v. Recycling: Concrete and asphalt shall be recycled where practicable.

Contractor shall deliver these recyclables to a permitted/authorized facility and report all quantities of materials recycled to the CO and to 87 CES/CEIE. All other waste shall be disposed of in accordance with paragraph 1.04A, above.

vi. Hazardous Material Handling: Any hazardous material or chemical to be used or stored by the contractor must be approved, in writing, by the CO prior to the contractor bringing it on the Base. The contractor must utilize an AF Form 3000, Material Approval Submittal, listing each container of hazardous material stored, kept, or utilized, to include Material Safety Data Sheets (MSDS). {Include on Material Approvals Submittal List} MSDS sheets must also be displayed on-site for access by contractor employees as well as the Government, per OSHA requirements.

vii. CLEAN AIR ACT PERMITS: JB MDL is responsible for obtaining and renewing all air permits for all stationary sources on Base that are owned by the Government and for mobile sources such as Government vehicles. Permits are required for boilers 1 MMBtu/Hr. and larger, generators larger than 100KW and tanks containing fuel or anything other than air distillates greater than 10,000 gallons. The contractor shall provide written notification to the CO and 87 CES/CEIE office sixty (60) days prior to bringing any equipment of this threshold size on base. It is the contractor’s responsibility to obtain air permits for all contractor-owned and installed air pollution sources. The contractor is responsible for compliance with the Government’s air permits and shall maintain all documentation and record keeping required to support those permits in a format acceptable to and approved by the CO. The documentation must be prepared and maintained by the contractor and readily available for on-site inspection by environmental regulators.

viii. LAND USE: Development, including building of docks, piers, bulkheads, bridges, pipelines, cables and filling, dredging or removing of sand or other materials near or on navigable waterways requires a permit. Activities that occur within 150 feet of a wetland may require a permit. JB MDL is responsible for selecting and securing the

PTFL 16-9002 – Repair Bathroom & Locker Room Page 11 of 14 appropriate approvals, unless the contractor is directed, in writing, to make permit determinations and acquire permits under this contract.

ix. WATERS AND WATER SUPPLY: It is not permissible to discharge any materials to the navigable waters of the United States without a permit. Accordingly, discharges to or in areas that may reasonably reach or impact wetland areas and all other watercourses, including ditches, is prohibited without a permit. All discharges to storm drains, including but not limited to, soils and sediments contained in water runoff or dewatering operations, are prohibited. An appropriate soil erosion control plan shall be developed and utilized by the contractor for construction projects to ensure that discharges do not occur. See appropriate engineering drawings of this contract for more specific soil erosion and sediment control requirements. Contractor shall comply with JB MDL area specific Storm Water Pollution Prevention Plan, a copy of which is available in 87 CES/CEIE, in building 2403.

1. Protection of Erodible Soils: Earthwork shall be planned and conducted in such a manner as to minimize the duration of exposure of unprotected soils. Earthwork brought to final grade shall be immediately finished as indicated and specified.

The rate of runoff from any construction site shall be mechanically retarded and controlled. Use of hay bale filtration, diversion ditches or other sound engineering device shall be used to retard and divert runoff from entering US waters.

2. Erosion and Sediment Control Measures: Temporary protection shall be provided on side and back slopes when rough grading is complete, or when sufficient soil is exposed, to require protection to prevent erosion. Protection shall be afforded by accelerated growth of permanent vegetation, temporary vegetation, mulching, or netting. Slopes too steep for stabilization by other means shall be stabilized by hydro-seeding, mulch anchored in place, covering with anchored netting, sodding or such combination of these and other methods as may be necessary for effective erosion control and to prevent a point source discharge.

x. LEAD PAINT AND ASBESTOS: If lead paint and/or asbestos materials are to be abated or removed pursuant to this contract, see Sections 02081 and/or 02080 respectively for contractor requirements.

If this contract does not contain those Sections, and lead paint and/or asbestos is found or suspected during the execution of this contract, the contractor, at their expense, shall have those substances tested by a certified laboratory in accordance with the protocol set forth in New Jersey and Federal law. If the contractor positively identifies these

PTFL 16-9002 – Repair Bathroom & Locker Room Page 12 of 14 materials at the action level set forth in the law and regulations, he must notify the CO by providing the laboratory results and cease work in those areas that might disturb those items until abatement issues are addressed and resolved. Work may continue as long as the suspected or identified materials are not disturbed.

xi. NOTIFICATION OF ENVIRONMENTAL SPILLS: In the event that the contractor spills or releases any unpermitted substance, hazardous substance or hazardous waste into the environment, the contractor shall immediately report the incident to the Fire Department, the CO, and the designated Government representative. The contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substance into the environment.

xii. FUEL STORAGE: Gas or diesel fuels shall not be stored on the construction site, or on Base, without prior written approval from the CO. Appropriate containers shall be utilized.

E. FIRE PREVENTION REQUIREMENTS

i. National Fire Alarm Code; A-5-3.5.1, Detectors shall not be located in a direct airflow, and not closer than 3 feet (1m) from an air supply diffuser and ceiling fans. When installing fans, the detector shall be moved not less than 3 feet or the fan shall be installed not less than 3 feet from the existing or new detector.

ii. The contractor shall be advised that all fire alarm and suppression system components, including all smoke detectors, heat detectors, horns, strobes, pull stations, control panels, sprinklers, fusible links, valves, gauges, etc. shall be protected from dirt, dust, paint, and damage during demolition and construction of the facility by covering them with a plastic or other viable covering to prevent these devices from inadvertently being set off or damaged by airborne dust and dirt.

Should it become necessary, the disconnection of the existing fire protection system and alarms requires prior approval of the Fire Prevention Office at the below telephone numbers.

iii. The contractor shall comply with the most current editions of the NFPA Codes and Installation Fire Prevention Standard JBMDLI 32- 2001. All open flame/hot work or disconnection of fire protection alarms or suppression systems requires prior approval of the Joint Base Fire Prevention Office: McGuire: (609) 754-2451; Dix: (609) 562- 5484/6430; Lakehurst: (732) 323-7580/7878. Any disconnection, connection or installation of fire protection alarms or suppression systems shall require an acceptance test performed by the contractor and witnessed by the Fire Prevention Office.

iv. Contractor will comply with all the requirements of the National Fire Code Standard 241 Safeguarding Construction, Alteration, and Demolition Operations 2002 edition. Contractor shall be very familiar with the requirements of Chapter 9 Safeguarding Roofing Operations of the National Fire Code Standard 241. The requirements can be viewed at the Joint Base Fire Prevention Office.

v. All hot work requires a hot work permit issued by the Joint Base Fire Prevention Office. The contractor shall contact the Joint Base Fire Prevention Office, to acquire a copy of a Hot Work Permit.

F. MISCELLANEOUS REQUIREMENTS

i. Compliance with the provisions of this contract by all sub-contractors shall be the sole responsibility of the prime contractor.

ii. It is imperative that a Pre-construction conference be conducted with all Government representatives involved, Prime Contractor, and Sub-contractors to discuss and resolve all potential issues and differences pertaining to the project prior to the start of actual work.

iii. Work shall normally be accomplished during the standard 8-hour day, 5-day week, and from 8:00am to 4:30pm, Monday through Friday, excluding federal legal holidays.

iv. Contractor shall verify all dimensions and materials required prior to ordering.

v. Contractor shall leave all work areas in a clean and orderly manner at the close of each business day, while the work is in progress, and upon completion of the project. The contractor shall be responsible for disposing of any installed equipment affected by the project. All contract-generated debris shall be removed from the government property and disposed of offsite. Disposal shall be in accordance with Local, State, and Federal laws and regulations. In case of conflict among these laws and regulations, the most stringent law or regulation will apply.

vi. Electrical power and water required in the performance of this contract will be provided by the government at no cost to the contractor.

Temporary extension of power and/or water lines, if necessary, shall be installed at the contractor’s expense.

1. Under No circumstances or conditions shall sub-contractors be permitted to tap directly into power supplies of existing HVAC units (outside) for source of power to operate their equipment.

vii. All work shall be performed by mechanics skilled in the type of work involved. When new work adjoins, connects or abuts existing work or facilities, the existing work or facility shall be altered as required and the connections made in an approved workman like manner. All existing work or facilities damaged by the contractor’s operations, other than facilities to be removed, shall be repaired or replaced by the contractor at no cost to the Government.

viii. Contractors will comply with all National Electric Codes (NEC) and National Standard Plumbing Codes (NSPC).

ix. Unless otherwise specified all materials and/or equipment used in the performance of this contract shall be new and unused. All materials stored on the job site must be protected from the elements.

x. The contracting officer and/or his Quality Assurance Evaluator (QAE) may inspect the work at any time during the contract period, and reserves the right to stop or terminate the contract if all work does not comply with the specifications provided. The presence or absence of the Contracting Officer or QAE shall not relieve the contractor from strict compliance with these plans and specifications.

xi. The contractor shall notify the QAE or Project Manager a minimum of 72 hours prior to commencing work, and prior to completion of work on this contract.

xii. Contractor shall use GFCI’s hardware within six (6) feet of any water source.

G. SUBMITTALS

i. Contractor shall provide 100% electronic As-Built drawings in the form of AutoCAD 2010 or later upon project completion.

1. UGeneral Requirements
The contactor shall provide and install all labor and materials to renovate the female latrines on the first floor of building 2216; located on Joint Base M-D-L, NJ. The summary of work shall consist of, but not limited to the following:
1.1. The Contractor shall ensure restrooms meet ADA requirements.
1.2. The Contractor shall provide for all demolition as noted in §2.4 & §2.5 (Existing Conditions).
1.3. The Contractor shall remove all fixtures and furnishings.
1.4. The Contractor shall provide necessary plumbing and venting to add 2 toilets and stalls.
1.5. The Contractor shall core drill 6”reinforced concrete block wall to tie into existing plumbing.
