About this file
This is a Past/Present Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California for the evaluation of laundry services contractors. The questionnaire is designed to gather performance feedback from customers of companies submitting quotations for the laundry services requirement. Responses must be submitted as PDF files to Mr. Clark Teninty, Contract Specialist, at clark.teninty.1@us.af.mil by March 31, 2026 at 12:00 PM PDT. Completed questionnaires will become part of the official source selection records. Respondents may confirm receipt by contacting Mr. Teninty at 707-424-7737.
The questionnaire collects information across three sections: Section A requests the offeror's company information including contractor name, address, point of contact, title, phone number, and email; Section B requires respondent organization details using the same contact information fields; and Section C documents contract specifics such as contract name/number, performance period, annual pounds of laundry services, annual contract value, total contract value, and whether the contractor performed as prime or subcontractor. Section D requires performance ratings on a 1-5 scale (Unsatisfactory through Exceptional) for quality of service, on-time delivery, service management, communication, contract terms adherence, and overall performance. Respondents must also indicate whether corrective action reports were issued and whether they would award another contract, with narrative explanations required for ratings of 1, 2, or 5. The questionnaire concludes with signature blocks for authorization.
