About this file
This is a Past Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California for the procurement of Local Telecommunication Services. The questionnaire is designed to collect performance ratings from recent customers of contracting companies submitting quotations for this requirement.
Completed questionnaires must be submitted in PDF format to Mr. Don Salgado, Contract Specialist, at don_phillip.salgado@us.af.mil by May 18, 2026 at 12:00 PM PDT. The questionnaire requires respondents to provide information about the contractor (offeror), the respondent's organization, and detailed contract information including contract name/number, description, performance period, annual and total contract value, and whether the contractor performed as prime or subcontractor. Performance is rated on a 1-5 scale across six specific categories: quality of service, on-time delivery of service, management of service, communication, contract terms and condition adherence, and overall contractor performance. Additional evaluation items address whether the contractor received Corrective Actions Reports (CARs) or similar deficiency reports and whether the respondent would award another contract to the contractor. Narrative explanations are required for any ratings of 1, 2, or 5. The completed questionnaire becomes part of the official source selection records. Respondents should contact Mr. Salgado at 707-424-2014 to confirm receipt.
